The Formation Add-Or-Subtract Rule
Three checks decide whether your jawline needs something added or something taken away.
Check one, the pinch. Pinch the tissue under your chin. If you can gather a meaningful roll between two fingers, that is fat, and fat is a subtract problem. Filler does not address it.
Check two, the recoil. Pull the skin along your jaw gently outward and release. Skin that returns promptly still has working elastic support. Skin that drifts back slowly is a laxity problem, which points toward tightening rather than filling. Most people in the roughly 25 to 55 skin-age window still have enough recoil to work with.
Check three, the profile. Look at yourself from the side with your mouth closed and teeth together. If your chin sits well behind your lower lip, or the angle below your ear reads as a curve instead of a corner, that is a frame deficit. That is the one filler solves.
Clear check three but fail one or two and you are a candidate for a combined plan, not a filler-only plan. Fail all three and honest advice is to address fat and skin first, then reassess whether you still need projection at all.
One Formation patient, a fit professional in his forties, came in convinced his problem was a weak jaw. It was submental fat that had never responded to weight loss. He had liposuction with tightening instead of filler. Colleagues told him afterward that he looked more rested. Nobody guessed why. That result would not have happened with a syringe.
Dr. Grandhige is willing to say a procedure is wrong for you, which is one reason our philosophy limits surgical volume in the first place.
The Long-Term Question Most Pages Skip
If you plan to maintain a filled jawline for years, there is a finding you should know about, and almost no clinic page mentions it.
What The Research Found
A controlled trial published in the International Journal of Surgery in 2024 took CT scans before and after chin filler injection. Imaging showed discernible bone resorption at the chin in about 36 percent of patients, with a median resorption thickness ratio around 24 percent. The authors concluded that hyaluronic acid may induce bone resorption in this area and that patients should be informed of it.
What The Approval Data Shows
One regulatory distinction matters here and is regularly stated backwards online. Dermal fillers go through the FDA’s premarket approval pathway, so they are approved. Energy devices like FaceTite go through 510(k), so they are cleared. The words are not interchangeable.
Juvederm Volux XC is the first and only hyaluronic acid filler approved by the FDA for improving moderate to severe loss of jawline definition. In the pivotal trial, about 70 percent of treated participants showed improved jawline definition at six months, and 82 percent reported satisfaction with the appearance of their lower face and jawline through twelve months. The manufacturer’s own guidance is that the improvement lasted one year in the majority of participants, which is worth holding onto when you see pages promising two years. FDA + 2
Read the full patient labeling from the FDA if you want the side effect tables. Reported effects clustered around tenderness, swelling, lumps, firmness, and bruising at injection sites, mostly mild to moderate.
What The Research Did Not Settle
The same paper is careful about its own limits, and so are we. The prospective group was small, follow-up ran only six to twelve months, so whether bone regenerates once the filler fully degrades is unknown, and CT was the only imaging used. A widely repeated claim that larger injection volumes drive the effect did not hold up either. The subgroup comparison of more than 1 mL against 1 mL or less found no significant difference.
The authors’ own read is measured. The resorption thickness was far less than what is seen with mild chin implant erosion, no periapical lesions appeared during follow-up, and they advise interpreting the finding rationally rather than anxiously. You can read the full study yourself.
Why say this at all. Because a patient planning a decade of chin injections deserves to know the question exists before starting, and because it changes the calculus for someone weighing filler against a one-time structural fix. That conversation happens at Formation before treatment, not after.
How Formation Prepares You Before And After Injection
Your health and diet are assessed before treatment, not after a problem shows up. Nutrition, supplements, and peptides are adjusted in the window immediately before and immediately after so your tissue builds the most structure it can around whatever is placed.
The same principle carries through the surgical side of the practice, where surgery accounts for roughly 60 percent of a result and actively managed aftercare accounts for the other 40. Dr. Grandhige follows patients for a full year.
The syringe on the tray at Formation is the same syringe on the tray anywhere else. The thinking around it is not.
Getting To Formation From St. Petersburg
Formation is at 1315 South Howard Avenue, Suite 102, in Tampa. From downtown St. Petersburg that is roughly twenty miles, about half an hour outside peak hours, and noticeably longer if you cross at rush hour.
Two routes work. I-275 north over the Howard Frankland Bridge is the direct one, then east toward the Howard Avenue corridor in South Tampa. The Gandy Bridge is the quieter alternative and lands you closer to South Tampa without the interstate.
One recent change is worth knowing if you take I-275. FDOT’s new express lanes on the Howard Frankland run from 4th Street North in St. Petersburg to just north of the bridge in Tampa, and as of August 22, 2026 tolls on them are collected electronically around the clock through SunPass. Cash and toll-by-plate are not accepted, and the lanes are limited to passenger vehicles, registered vanpools, and buses. If you do not carry a transponder, stay in the general-use lanes. The Howard Frankland carries roughly a quarter million vehicles a day, so a filler appointment scheduled for late morning rather than 8 a.m. is a materially different drive.
Patients come to us from the Historic Old Northeast, Snell Isle, the Grand Central District, Kenwood, and the Beach Drive corridor near the St. Pete Pier. Because filler requires no meaningful downtime, most St. Petersburg patients drive over, get treated, and drive home the same afternoon.
What A Jawline Filler Visit Looks Like

Sometimes it is fat removal, tightening, or waiting. If your laxity is beyond what tightening or contouring can reach, Formation stays in contact with the surgeons who do excisional facial work well and will point you to the right one rather than sell you something that will not hold.

A Jawline That Matches The Rest Of You
Filler is very good at one job. It rebuilds a frame that was never there or has receded with age, and it does that in an afternoon with almost no interruption to your week. It is a poor substitute for fat removal and a worse one for skin tightening, and a practice willing to tell you that before taking your appointment is telling you something useful about how it operates.

If your plan includes liposuction or energy tightening instead, the neck and jawline timeline runs about a week back to work with a chin garment worn 12 to 24 hours a day, swelling settling over two to four weeks, tightening visible around three months, and the final result at six to nine months.
ย Wait. Facial fat changes with weight, and a jawline filled at a heavier weight often needs revisiting after a significant loss. Formation assesses weight stability before planning any structural work on the face.
