Neck liposuction removes the fat pad under your chin through two or three very small incisions. The result is a clearer line between the jaw and the neck. What Neck Liposuction Actually Removes
It removes fat. That is the whole mechanism, and it is worth being blunt about because fat is only one of the four things that shape a jawline.
The fat under the chin sits in two compartments, one above the platysma muscle and one below it. Liposuction reaches both. Once that fat is gone, it does not come back, because the cells themselves are removed rather than shrunk. What liposuction cannot do is tighten loose skin, tighten a slack muscle, or add bone where the chin sits back.
This matters more than most pages admit. A patient whose only problem is fat gets a large improvement from a small procedure. A patient whose fat is hiding a muscle problem can look worse after fat removal, because taking the padding away makes the bands underneath more visible. The same is true of the submandibular gland, which sits just under the jaw. Remove too much fat above it and the gland becomes a visible bulge that no amount of further liposuction will fix.
So the question at a consultation is never “can we take fat out.” It is “how many layers are actually involved.”
The Four-Layer Neck Read
Most necks show aging in more than one layer at once. Dr. Grandhige screens every neck against four layers before recommending anything, and each layer has its own answer.
Layer One, The Fat
This is the layer liposuction treats. Fat under the chin is heavily influenced by genetics, and in a lot of patients it never responds to weight loss at all. People who are otherwise lean carry it, train around it for years, and still see it in every photograph.
If the fat is the only finding, neck liposuction alone is the answer, and it is a short procedure with a mild recovery.
Layer Two, The Platysma And Platysmal Bands
The platysma is the thin sheet of muscle running up the front of the neck. When its edges separate and slacken, you see two vertical cords running from the jaw toward the collarbone. Those are platysmal bands, and no amount of fat removal touches them.
Where bands are present, the answer is MyEllevate, a permanent suspension suture, paired with platysmaplasty to bring the muscle edges back together. The suture is nonabsorbable and can be removed later if needed, which separates it from barbed threads that dissolve and take the correction with them. Layer Three, Skin And The Fibroseptal Network
Underneath the skin sits the fibroseptal network, the connective tissue scaffolding that holds skin against the muscle below. When that scaffolding loosens, skin stops recoiling.
Depth of access is what separates the tools here. Radiofrequency devices such as FaceTite and Renuvion, a helium plasma and radiofrequency device, are placed beneath the skin and treat its deepest layer along with the fibroseptal network under it. Surface treatments, meaning lasers and microneedling as a category, work from the top down and cannot reach that foundation. Both device types are 510(k)-cleared rather than FDA-approved, which is a lower regulatory bar and worth understanding before anyone quotes clearance at you as proof of results. Skin quality sets the ceiling. Roughly speaking, skin ages 25 to 55 respond well. Outside that window the honest answer is often that tightening will not be enough.
Layer Four, Chin Projection And Jaw Angle
Some double chins are not fat problems. They are frame problems. When the chin sits back relative to the lower lip, there is nowhere for the soft tissue to drape, and the neck reads as full no matter how lean the person is.
Structural chin and jawline filler addresses this. Radiesse, a calcium hydroxylapatite, builds projection along bone. Juvederm, a hyaluronic acid, refines the border. How The Procedure Is Done At Formation
Neck liposuction here is an office procedure, not a hospital case. You take an oral sedative, the area is numbed with local anesthetic, and you stay awake and comfortable throughout.
Access is through very small, discreet incisions under the chin and behind the ears. Fat is removed carefully to sharpen the jawline while preserving the natural angles that make a neck look unoperated. Over-aggressive removal is deliberately avoided, because it produces surface irregularities and a neck that ages faster, not slower. Renuvion or FaceTite is then used to tighten the skin down over the muscle and bone underneath.
Dr. Gopal Grandhige performs every case himself. He is a board-certified general surgeon specializing in aesthetic surgery, certified by the American Board of Surgery, with fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. He personally evaluates every patient and designs the surgical plan. His physician assistant, whom the practice employs directly, assists on all of his surgeries, and he works with the same anesthesia provider and the same staff on every case, so the room already knows how he operates. Who This Works For, And Who It Does Not
The strongest candidates are bothered by a double chin or a soft jawline, have reasonable skin elasticity, and want facial refinement without fillers or a more invasive operation. They want a sharp jawline, not a tight one.
Four situations where liposuction alone is the wrong answer, and where Dr. Grandhige will say so:
You have visible platysmal bands. Removing the fat over them makes them more obvious. The muscle has to be addressed in the same sitting or the result looks worse than the starting point.
Your chin sits back. Fat removal will sharpen a line that still has no frame behind it. Structure comes first.
Your skin has lost recoil past what tightening reaches, usually after major weight change or beyond the elasticity window. Energy-based tightening has a ceiling and pretending otherwise is how patients end up in revision. Patients who need skin excised are referred outward. Formation stays in contact with the surgeons who do that work well and points you to the right one.
Your fullness is glandular rather than fatty. Aggressive fat removal above a prominent submandibular gland leaves the gland exposed as a visible bulge. That is a permanent contour problem and it is why the assessment matters more than the technique.

Recovery Week By Week

Two to four weeks. Swelling keeps settling and the fat removal becomes clearly visible. This is when most patients start hearing comments.
Three months. Skin tightening is visible and the jawline improvement is obvious.
Six to nine months. Final result. The tightening component keeps refining across this whole window, which is why the neck you see at three months is not the neck you keep.

What Formation Does Before And After That Most Practices Skip

Aging continues regardless. What removing excess fat in this area does, when it is done with restraint, is reset the aging clock in the neck and slow the appearance of future laxity. Done aggressively, it does the opposite.
