FDA Cleared, Not FDA Approved

Almost every FaceTite page in Tampa says the device is “FDA approved.” That’s the wrong word, and the difference matters if you’re trying to read marketing honestly.

FaceTite went through the 510(k) premarket notification pathway, clearance number K151793, granted in February 2016 for electrocoagulation and hemostasis of soft tissue. Clearance means the FDA found the device substantially equivalent to something already on the market. Approval is a different, stricter route reserved for higher-risk devices, and it requires the manufacturer to prove safety and effectiveness from scratch.

Cleared is not a red flag. Roughly 99 percent of medical devices in the United States reach the market this way. But when a clinic page upgrades “cleared” to “approved,” that tells you something about how the rest of the page was written.

Dr. Grandhige says the distinction out loud in consultation, because a patient who understands the regulatory basis of a device asks better questions about everything else.

The Four-Point Jawline Check

Before anyone at Formation books a FaceTite date, Dr. Grandhige runs the same four-point assessment. It takes about five minutes in the room and it decides everything: whether you get FaceTite, a different tool, or a conversation about waiting.

Point One: Where The Fat Sits

Fat under the chin lives in two layers. The superficial layer, above the platysma muscle, responds well to suction and radiofrequency. The deeper subplatysmal layer does not, and chasing it with a cannula through small incisions creates contour irregularities that are hard to fix. Dr. Grandhige palpates for this with your jaw clenched. If the bulk is deep, FaceTite alone won’t give you the profile you want.

Point Two: How Fast Your Skin Snaps Back

Skin gets pinched at the jawline and released. Snap-back under two seconds is a strong signal. Two to four seconds is workable. Beyond that, tightening the envelope with energy alone tends to disappoint, and the honest answer is a different plan. Across Formation’s neck and jawline cases over the past two years, skin recoil, not fat volume, has been the single most common reason a case gets postponed.

The questionnaire answer Dr. Grandhige gives patients is a rough age band of 25 to 55 for reasonable elasticity, but the pinch test beats the birthday every time. Plenty of 58-year-olds pass it. Some 42-year-olds don’t.

Point Three: Whether The Bands Are Showing

Ask a patient to grimace. If two vertical cords stand up from the collarbone to the chin, those are platysmal bands, and they’re a muscle problem. Radiofrequency doesn’t address them. Removing the fat that partially camouflaged them can actually make them more visible, which is the outcome nobody wants and few pages mention.

Point Four: Jaw And Chin Projection

A short chin or a high hyoid bone limits how sharp any neck can look, because the underlying architecture sets the ceiling. Dr. Grandhige assesses this in profile before touching anything. If skeletal structure is the real constraint, tightening skin over it produces a modest change and a disappointed patient.

FaceTite, Renuvion, Liposuction, Or A Neck Lift

Formation runs more than one skin-tightening platform, which means the device gets picked to fit the neck rather than the neck getting talked into the device. Here’s how the call actually gets made.

When FaceTite Is The Right Tool

Moderate fat, moderate laxity, decent recoil, no visible banding. This is the sweet spot. The energy is delivered slowly and evenly across the treatment zone, which is what you want when the fibrous network is intact but stretched.

When Renuvion Gets Chosen Instead

Renuvion uses helium plasma with radiofrequency and delivers energy in a shorter, sharper burst. Dr. Grandhige reaches for it on thinner necks and on patients where the tissue plane is delicate and a longer thermal dwell time would be too much. Both devices are in the building. The choice happens at the four-point check, not at the booking desk.

When Liposuction Alone Is Enough

Younger patients with good recoil and a fat problem rather than a skin problem sometimes don’t need the energy step at all. Adding it is billable and unnecessary, and Formation’s position is that unnecessary is a bad reason to do something to a person’s face.

When You Need A Neck Lift Instead

Heavy skin laxity, significant banding, or skin that stays folded when released means an energy device will underdeliver. The right answer is a surgical neck lift with a different surgeon or a staged plan, and Dr. Grandhige will say so. If skin quality rather than skin volume is the concern, Morpheus8 sometimes fits better than anything invasive.

What The Largest Published Study Actually Found

Most FaceTite pages describe risk as “rare side effects.” There are real numbers, and they come from a multicenter retrospective review of 247 patients treated with radiofrequency for lower face and neck laxity.

Average improvement on the Baker Face and Neck Classification was 1.4 points. Ninety-three percent of patients said they’d do it again. Ninety-seven percent of cases were performed under local anesthesia, which lines up with how Formation runs them.

The complications were specific rather than vague. Prolonged swelling past six weeks in 4.8 percent of patients. Firm areas lasting beyond twelve weeks in 3.2 percent. Temporary weakness of the marginal mandibular nerve, which controls the lower lip, in 1.2 percent. All of it resolved without further treatment.

That last one is the reason technique matters. The marginal mandibular nerve runs close to where the probe travels along the jaw. Dr. Grandhige works in a defined plane and keeps the probe at a set depth across that zone, which is a decision made in the room rather than a feature of the machine.

Your Day At Formation, Start To Finish

You arrive without having eaten much, take an oral sedative, and wait about 30 minutes for it to settle in. Dr. Grandhige marks the treatment zones with you sitting upright, because a neck looks different lying down and marking a supine patient is how you end up with an uneven result.

Local anesthetic goes in next. Two or three access points get made under the chin and behind the ears. Fat comes out first, then the radiofrequency pass, with surface temperature monitored continuously throughout. The procedure itself runs roughly 45 to 60 minutes. A chin compression garment goes on before you stand up.

Total time from walking in to walking out is about two hours. You’ll be awake and talking for most of it, and you will not remember much of it. Your driver takes you home.

Across Formation’s neck and jawline cases over the past two years, 94 percent were completed exactly this way, in the office, awake, without general anesthesia.

The Real Recovery Timeline

Several Tampa pages claim a return to work in three to five days. Here’s the version Formation gives patients, which runs a little longer and is a lot more accurate.

First few days: Mild swelling, bruising, and a tight feeling under the chin. The compression garment stays on. Discomfort is manageable with what’s sent home with you.

Week one: Most patients are back at work and back to social activity. The garment is worn 12 to 24 hours a day through this stretch. Nine out of ten Formation jawline patients hit this mark on schedule.

Weeks two to four: Swelling keeps dropping and the liposuction result becomes clear. This is the point where people start taking photos again.

Three months: Skin tightening becomes visible, with a real change along the jaw. This is the milestone most patients are actually waiting for, and it’s why Formation asks people not to judge the result at week three.

Six to nine months: Final result. Skin contraction is complete and the contour has settled.

Who Gets Told No

Formation turns people away, and the reasons are consistent.

Anyone with skin that stays folded when released. Anyone with visible platysmal banding who wants the bands gone, because this doesn’t do that. Anyone whose weight has swung more than 15 pounds in the last six months, because the result won’t hold. Anyone expecting facelift-level change from an office procedure.

And anyone who describes their jawline as the problem but spends the consultation talking about their torso. That’s a different conversation, and it usually points toward high-definition liposuction rather than anything above the collarbone.

Being told to wait isn’t a rejection. It’s the same judgment call that keeps the results on this site looking like the people who had them.

Why Post-Op Care Is 40 Percent Of Your Result

Dr. Grandhige puts a number on it: the procedure is 60 percent of the outcome and post-operative care is the other 40 percent. Bad aftercare can undo good work, and that’s not a figure of speech.

Formation manages recovery directly instead of handing you a printout. Compression is scheduled, not suggested. Lymphatic drainage sessions start at the first post-op visit, and Formation patients average four sessions in the first month after a jawline case. Protein intake and inflammation control get specific guidance, because tissue that heals in a low-inflammation state contracts more predictably.

Follow-up runs a full year. Not a six-week discharge. That continuity is the core of the Formation philosophy, and it’s the part that’s hardest to copy.
