The Formation Lift Ladder
Dr. Grandhige sorts every thread lift inquiry onto one of three rungs before discussing technique. The rung is set by how the skin behaves, not by age on a chart. Where the neck is the primary concern, the Four-Layer Neck Read governs which layer is driving the problem, and the Lift Ladder governs whether the laxity in that layer is still within reach of a thread.
Rung One, Early Laxity With Recoil
Pinch the skin along the jawline and release it. If it snaps back toward the bone without hesitating, the collagen scaffold is still intact and a thread has something to anchor into. Jowling is beginning to soften the jaw angle but nothing hangs when your face is at rest. This is the rung threads were built for, and it is where results look like nothing was done.
Rung Three, Skin That Hangs At Rest
Skin drapes below the jaw line when you look straight ahead, and the pinch test shows the skin returning slowly or not at all. No thread reaches this. Neither does energy. This is excisional territory, and Dr. Grandhige does not perform facelifts. Formation stays in contact with the surgeons who do that work well and points you toward the right one. That referral is the whole answer for a meaningful share of Largo inquiries.
Reasonable skin elasticity for this work sits roughly between skin ages 25 and 55. Skin age and birthday are not the same number, which is exactly why the ladder exists.

The skin recoils slowly, and there is fullness under the chin that has not moved with weight loss. Threads placed over unaddressed fat pull a heavier envelope and give up early. The sequence here runs the other way: the fat comes out first through neck and jawline contouring, the skin is reassessed at three months once tightening has declared itself, and threads become a finishing decision rather than a first move.

Dr. Grandhige turns down more thread lift candidates than he treats in the older half of the elasticity range. Sometimes surgery is not offered at all. That is the practice operating as designed.
Treatment Day And The Week After
You arrive, the entry points are marked while you are sitting upright so gravity is working the way it will after, and the areas are numbed with local anesthesia. The threads go in through fine cannulas. Total chair time runs 30 to 60 minutes depending on how many zones are treated.
Expect mild dimpling or gathering of the skin immediately afterward. It looks wrong and it is normal, and it settles as the tissue relaxes around the barbs.
The recovery arc for this cluster:
- First few days. Swelling, bruising and tightness. A chin compression garment is worn where the neck was treated.
- One week. Back to work, back to social activity, back to training. Garment 12 to 24 hours a day for neck-inclusive cases.
- Two to four weeks. Swelling settles and the placement result is clean.
- Three months. Collagen contribution is visible.
- Six to nine months. Final read, at the point the threads have fully absorbed.
Time this against the Gulf. Indian Rocks Beach is under six miles from central Largo, and direct sun on healing entry points is the fastest way to leave a mark you did not plan on. Two weeks of shade is a scheduling problem, not a medical one, but it is a real one in Pinellas County in July.
How Long The Lift Holds, And What The Literature Says
Results generally run 12 to 18 months. The threads dissolve in six to nine, and collagen deposition carries the remainder.
Now the honest part. A systematic review published in Plastic and Reconstructive Surgery looked at a decade of thread lift literature and concluded that little or no substantial evidence had been added to support the earlier optimism about barbed suspension sutures. More recent work in the aesthetic surgery literature raises a second point worth weighing: fibrosis from prior thread placement can make a later facelift more difficult and less predictable. If you are on Rung One and threads are a reasonable finishing tool, that risk is small. If you are on Rung Three and someone is selling you threads as a bridge, you are trading a harder operation later for a lift that will not hold now.
Regulatory language matters here too. PDO sutures are 510(k)-cleared, which is a finding of substantial equivalence to an existing device, not an approval. Cleared and approved are different words describing different reviews. Certain barbed PDO products carry a specific clearance for temporary face suspension; most PDO suture clearances cover soft tissue approximation only. Ask which product is going into your face and what its clearance actually says. Any practice that cannot answer that has told you something.
One more piece of current context. The American Society of Plastic Surgeons released its 2025 Procedural Statistics Report on September 1, 2026. Lift procedures grew broadly across the body, facial fat grafting posted the single largest gain at 39 percent, and neuromodulators and hyaluronic fillers together accounted for 80 percent of all minimally invasive procedures. Thread lifts are not a separately tracked line item in that report. Anyone citing an ASPS thread lift growth figure is citing something that does not exist.
Preparing The Tissue Before A Single Thread Goes In
A thread lift result is partly mechanical and partly collagen your own body has to build. That second half is not automatic, and it is where Formation spends its attention.
Health and diet are assessed before treatment. Nutrition, supplements and peptides are adjusted in the window immediately before and after, so the body builds the most structure it can while the threads are still present to build along. Protein intake, inflammation control and lymphatic flow are handled as part of the treatment rather than as advice handed over on the way out.
The 60/40 Recovery Standard governs this. The procedure is 60 percent of the result. What happens in active aftercare is the other 40 percent, and poor aftercare can undo well-placed threads. Dr. Grandhige limits volume specifically so that he remains involved through that window.
