HD liposuction removes the thin layer of fat pressed against the underside of your skin so the muscle you’ve already built can show. It’s a finishing step for people who train and sit near their goal weight, not a way to lose weight. Formation performs it at 1315 South Howard Avenue in South Tampa, about six miles from Oak Park. That’s close enough to recover at home and come in for follow-ups.
Two planes of fat sit between skin and muscle. Standard liposuction works the deep one and makes an area smaller. HD liposuction also works the superficial one, the thin band that decides whether a muscle border reads through the skin. That’s what makes it unforgiving: a few millimeters separate a clean edge from a dent.
Most people who ask about it are already doing the work. They train and eat well, and one last layer over the abs, flanks or chest won’t budge. Dr. Grandhige’s take is blunt. The dieting it takes to strip that layer costs muscle, and the weight swings back. Removing it surgically lets you go back to building instead of cutting.
Zones are planned together. Define an abdomen and skip the flanks, and the result gives itself away from the side. That’s why most high-definition liposuction plans at Formation cover several zones at once, usually some mix of abdomen, flanks, back, chest and arms. Fat cells taken out don’t grow back, so how long your result lasts comes down to what your weight does afterward.
Technique comes second. The first question is whether fat is your problem at all, or whether it’s skin. The Formation Tighten-Or-Remove Line sorts that out with three checks you can do at home before booking anything.
Pinch a fold of skin over the area, hold it a second, and let go. Skin that flattens right away will shrink down over a slimmer contour. Skin that keeps the pinch mark has lost some spring, and taking fat out from under it won’t make it lie flat.
Stand in front of a mirror with your arms relaxed. Ignore anything that folds when you sit or bend. Tissue that hangs while you’re standing still is skin without support, and thinning the fat beneath it only gives it further to drop.
Run your fingers down the midline from your ribs to your navel and press. A soft gap between the two muscle columns is separation, which is common after pregnancy or a large weight change. It pushes the abdomen out from behind the muscle, where liposuction can’t reach.
Pass all three and fat is the issue. HD liposuction fits, and energy-based skin tightening is a reasonable finisher. Fail the second or third and the answer becomes skin removal or muscle repair. Dr. Grandhige performs abdominoplasty and revision cases himself, so a changed plan stays with the same surgeon in the same office.
Some plans fit in one operation and some need two. The Formation Staging Line decides using four inputs: total planned volume, how many zones the plan crosses, whether skin removal is involved, and whether your schedule handles one longer recovery better than two shorter ones.
State law adds two outer limits that matter for staging. The first is volume. Florida’s office-surgery rule caps liposuction in an office setting at 4,000 cc of supernatant fat. That means the settled fat layer once the aspirate separates, which is less than what fills the canister. Combine liposuction with an abdominoplasty or another directly related procedure and the cap falls to 1,000 cc. The rule leaves the right amount for each patient to the surgeon.
The second limit is time. The same rule caps the planned length of all surgical procedures combined in an office at eight hours. Detailed HD work is slow, and Formation puts a full HD day at six to eight hours from pre-op prep through the recovery room. A front, sides and back plan like HD Lipo 360 is done in one stage when the plan allows and split into two when it doesn’t.
Oak Park is an East Tampa neighborhood in ZIP 33605. It’s roughly bounded by Columbus Drive on the north, Broadway on the south and 50th Street on the west. Formation sits about six miles away in a straight line, on the far side of downtown.
Living this close changes the recovery plan. Patients traveling in from out of town can recover in a nearby hotel, where Dr. Grandhige visits them in their rooms during the early days. From Oak Park, you can rest at home and come in for the first post-operative visit and the lymphatic sessions after it. Surgery is under general anesthesia, so someone else drives you home that day, and work waits until week one is over. If home isn’t restful that first week, the hotel option is open to you too.
Your health and diet are assessed before a surgery date goes on the calendar, and Dr. Grandhige requires at least four weeks of preparation. In the weeks on either side of surgery, nutrition, protein intake, supplements and peptides are adjusted so your body has what it needs to build as much structure as it can while it heals. Inflammation control and lymphatic flow get dialed in the same way. Any surgeon in Tampa can buy the same cannula. The thinking around it is what doesn’t transfer.
The in-office consultation with Dr. Grandhige carries a fee. It’s a full surgical assessment covering your anatomy, your result on the three checks, an honest read on what surgery will and won’t change, and timing. That fee can be applied toward any procedure or product at the practice. If you’d like a read before coming in, Formation offers a complimentary virtual evaluation with patient coordinator Hanna. She goes over your goals and outlines a plan before you decide on an office visit.
Taking too much is how HD work ends up looking artificial, and it tends to age poorly too. Carve the superficial plane too hard and a body stops looking trained and starts looking drawn on. Lines show in a flexed photo and disappear when you move. Grooves land where no muscle sits, and the skin is left with nothing natural to drape over.
Dr. Grandhige treats HD liposuction as sculpture, not fat removal. Each contour follows your muscle shape, your frame, your fat quality and the way light crosses a body in motion. Formation uses power-assisted (PAL) high-definition technique, and where definition stops is decided before the first incision.
Male body contouring leaves the least margin for error. Chest, abdomen and flank work on men has to keep masculine balance. An over-resected chest leaves a saucer deformity that’s far easier to avoid than to repair. When it’s appropriate, gynecomastia surgery is combined with torso sculpting so the chest and core read as one shape.
One patient Dr. Grandhige describes is a physician who trained hard and ate carefully but never saw definition that matched his effort. He wanted clarity, not etching. His plan covered the abdomen, back, chest, armpits, arms and flanks, done conservatively. After recovery his core looked athletic standing still and moving, and nobody guessed surgery. He later sent Dr. Grandhige a Halloween photo of himself in a Superman costume. Results vary, and yours depend on your anatomy, your skin and how closely you follow the recovery plan.
The research supports screening carefully. A 2026 meta-analysis of 9,570 HD liposuction patients found complication rates weren’t tied to age, BMI or sex. Its authors also concluded that inconsistent selection criteria still prevent an evidence-based definition of the ideal candidate. The three checks, confirmed in person, are how that question gets answered for you.
Not every patient who wants surgery is ready for it.
One patient came to Dr. Gopal wanting a tummy tuck. Over the next 18 months, she returned multiple times, having lost more weight each time. And each time, the answer was still no.
Not because she couldn’t have the surgery, but because she wasn’t where she wanted to be yet.
Instead of rushing into surgery, the focus was on helping her reach a healthier, more sustainable weight through nutritional counseling, lifestyle support, and medical weight management.
By the time she had lost 80 pounds, she was ready, both physically and mentally.
For Dr. Gopal, being a good surgeon isn’t just about knowing how to perform a procedure. It’s also about knowing when not to perform one.
Sometimes the best result starts long before the operating room.

Six hours. Hundreds of decisions.
High-definition liposuction is as much about precision as it is about technique.
Dr. Gopal can spend 6–7 hours on a single HD liposuction procedure, carefully evaluating the body’s anatomy, refining muscle definition, and making sure each contour transitions naturally into the next.
And that level of attention doesn’t change as the hours go by.
Because the final result is shaped by hundreds of small decisions made throughout the procedure.

Which one surprised you the most? 👀
Liposuction is one of those procedures people have heard a lot about, but not everything they’ve heard is accurate.
From what liposuction can actually achieve to how long results take to develop, there are plenty of misconceptions that can lead to unrealistic expectations.
Dr. Gopal Grandhige, board-certified plastic surgeon at Formation in Tampa, breaks down some of the most common beliefs about liposuction and explains what’s fact and what’s fiction.

This is not your typical liposuction. 👀
Most people think liposuction is simply about removing unwanted fat. But high-definition liposuction takes a different approach.
Before surgery, Dr. Gopal carefully maps out the patient’s natural anatomy, marking the chest, abdomen, and waist to guide where fat will be removed and where it should be preserved.
Why does that matter?
Because removing fat evenly isn’t necessarily going to create definition. HD liposuction uses strategic fat removal to highlight the underlying muscle structure and create a more sculpted, athletic appearance.
And those lines? They’re part of the planning that makes this level of precision possible.
The photos show an actual patient immediately after surgery. Swelling is still present, and the final results will develop as healing progresses.

Does fat move after lipo?
It’s a common concern, especially when someone notices weight gain in an area that wasn’t treated.
Here’s what actually happens.
The fat cells removed during liposuction are gone permanently. But if someone gains weight afterward, the remaining fat cells can still grow, including in areas that weren’t treated.
The fat didn’t move. The body gained weight.
That’s why Dr. Gopal emphasizes being at a weight patients can realistically maintain before surgery. Not the lowest weight they can reach, but one they can sustain long after recovery.

Want a more sculpted body?
High-definition liposuction takes body contouring a step further. Instead of simply removing excess fat, it focuses on sculpting around the body’s natural muscle structure to create more visible definition and a more athletic contour.
Dr. Gopal Grandhige specializes in HD liposuction, customizing each procedure based on the patient’s anatomy, body shape, and desired level of definition.
Because the goal isn’t to give everyone the same body. It’s to create a result that works with their natural anatomy.

Don’t plan lipo 2 months before vacation.
If the goal is to enjoy a more sculpted, defined body next summer, the planning needs to start months before that.
Fall is one of our busiest seasons at Formation, and there’s a reason for it. Many patients are already thinking about how they want to look and feel next summer.
But getting there takes time.
At Formation, Dr. Gopal requires at least four weeks of preparation before surgery. That time is spent preparing patients physically and mentally for the procedure and recovery.
After surgery, swelling needs to go down, the skin needs to contract, and the tissues need time to settle into their new contour.
It can take 6–9 months for results to look and feel natural.
That’s why Dr. Gopal recommends planning liposuction 6–12 months ahead, especially when there’s a specific season or vacation in mind.
If next summer is the goal, now is the time to start the conversation.

How defined would you go?
Not everyone wants the same look from HD liposuction. Some prefer a smooth, subtle contour, while others want their abs to stand out.
Here’s how the three levels of definition differ:
01. Low definition: A leaner, natural-looking abdomen with subtle definition and light contouring around the underlying muscles.
02. Medium definition: More visible separation between the abdominal muscles, with added definition through the abs and obliques without a sharply etched six-pack.
03. High definition: More pronounced sculpting around the abs and obliques to highlight the underlying muscle structure and create a more chiseled, athletic look.
Dr. Gopal tailors his HD liposuction technique to the level of definition you’re looking for, while considering what’s achievable for your body.
Which would you choose: 1, 2 or 3? Tell us in the comments!

This isn’t traditional lipo. It’s HD lipo.
For this patient, getting smaller isn’t the goal.
He already works out. He already has the muscle. Dr. Gopal’s job is to use HD liposuction to bring out more of that existing definition and shape the areas around it so the body looks more athletic and defined.
That’s why the markings matter. Before surgery even begins, Dr. Gopal is mapping out how the abs, chest, arms and surrounding areas should work together in the final result.

This is the part of the job that never gets old.
There’s something special about knowing how different a patient’s life might feel on the other side.
Not just because their body may look different, but because of everything that can come with it.
Feeling more comfortable in their own skin.
Saying yes to plans without thinking twice.
Looking forward to summer instead of dreading it.
Plastic surgery can change a body. But sometimes, what matters most is how differently someone gets to experience their life afterward.
And that’s a pretty incredible part of what we get to do at Formation Tampa.

Gopal Grandhige, MD is a board-certified general surgeon who specializes in aesthetic surgery. He isn’t a plastic surgeon, and Formation says so up front.
He earned his medical degree at the University of Michigan Medical School and matched into Yale’s six-year combined plastic surgery residency. He then changed course to complete a full general surgery residency. Fellowship training followed in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. His board certification in general surgery comes through the American Board of Surgery. He’s been operating, and practicing in Tampa, for more than 15 years, with several thousand cases behind him.
His burn and critical care training is why tissue behavior leads his planning: how it swells, where circulation is fragile, how healing plays out over weeks. Formation also trains other physicians in its PAL high-definition approach.
Volume stays limited on purpose. He evaluates each patient himself, designs the plan, performs the operation and follows you for a full year. Cases run in Quad A accredited surgical facilities. The same board-certified anesthesia provider and his own physician assistant are in the room every time.
Dr. Grandhige splits the result 60 percent surgery and 40 percent post-operative care. That’s the 60/40 Recovery Standard, and it’s why Formation keeps aftercare in its own hands. Poor care after surgery can undo a well-performed operation.
Swelling, soreness and drainage fill the first 48 to 72 hours, and you’re encouraged to get up and walk early. Compression stays on around the clock. Lymphatic treatment starts at your first post-operative visit in the office, the first of several trips in from Oak Park. Light activity only, and no work this week.
Seroma, a pocket of fluid under the skin, is reported in both large HD reviews, and their numbers don’t match. A 2023 review of 6,964 HD patients found it in 3.3 percent, second only to temporary skin darkening. The 2026 analysis pooled it at 7 percent, with hematoma at 1 percent. Both sets of authors flagged inconsistent reporting, so treat any single figure with caution. At Formation, a seroma is drained in the office, and the lymphatic work in the following weeks is aimed at keeping new ones from forming.
Swelling drops steadily and the garment stays on full time. Most patients are back in the gym within one to two weeks. They’re training without restriction somewhere between weeks two and four, as long as the garment is on. Lymphatic sessions continue through this stretch.
Definition starts to show between 6 and 12 weeks as inflammation settles. Final contour arrives across 3 to 6 months. Skin pliability and normal sensation return over 6 to 12 months. With abdominoplasty in the plan, exercise waits six to eight weeks.
Formation is at 1315 South Howard Avenue in South Tampa, about six miles from Oak Park in a straight line. The usual route is I-4 west from Exit 3 at 50th Street, then I-275 south to Exit 42, then Armenia Avenue toward South Howard. The Selmon Expressway is the toll alternative.
No. It removes a thin superficial layer of fat so muscle you already have can show. The best candidates are active and at or near their goal weight. If weight loss is the goal, that work comes first.
Run the three checks: recoil after a pinch, tissue that hangs while you stand still, and separation along the abdominal midline. Failing the second or third points toward skin removal or muscle repair. Dr. Grandhige confirms the reading in person.
Florida's office-surgery rule allows up to 4,000 cc of supernatant fat in an office setting, or 1,000 cc when liposuction is combined with an abdominoplasty or a directly related procedure. The right amount for you depends on your anatomy and is set by the surgeon.
Seroma, a fluid pocket under the skin, appeared in 3.3 percent of 6,964 patients in a 2023 review. A 2026 analysis of 9,570 patients pooled it at 7 percent, with hematoma at 1 percent. Both reviews noted inconsistent reporting. Formation drains seromas in the office.
