What A PDO Thread Lift Does, And What The FDA Actually Cleared
A PDO thread lift places absorbable polydioxanone sutures under the skin to reposition soft tissue and provoke a collagen response along the thread tracks. That is the whole mechanism. Two effects, one mechanical and one biological.
Now the part almost no page in this market will tell you. PDO sutures are cleared by the FDA through the 510(k) premarket notification pathway for soft tissue approximation, meaning wound closure. Cosmetic lifting is an off-label use of a cleared device. That is normal and legal in aesthetic medicine. It also means the FDA has not reviewed thread lifting for cosmetic efficacy the way it reviews a Class III implant.
Read any local clinic page advertising “FDA-approved” thread treatments. That phrase is wrong. Clearance and approval are different regulatory events, and the difference matters when you are deciding what to put in your face.

The Three-Layer Jawline Check
Before anyone at Formation discusses threads, your jawline gets sorted into one of three layers. We built this screen because thread lifts fail predictably, and they fail when the wrong layer is treated.
Layer One, The Fat Layer
If there is measurable fat under the chin or along the jaw, threads will not fix it. Threads reposition tissue. They do not remove volume. Pulling a fat pad upward with a suture moves the problem, it does not solve it.
This is the single most common reason we say no. A patient sees a soft jawline in photos, assumes the skin is loose, and books a thread consultation. On exam the skin is fine. The fat is the issue. That patient needs neck liposuction in Hyde Park Spanishtown Creek, not sutures.
Layer Two, The Skin Envelope

Over the past 12 months, 61 patients came to Formation asking specifically about a thread lift. Fourteen of them came from the 33606 zip code, which covers Hyde Park, Spanishtown Creek, and the blocks around Hyde Park Village.
Here is where those 61 landed after the three-layer check. Thirty-four were better served by neck liposuction with skin tightening. Eighteen were reasonable thread candidates. Nine were told to wait, either because the laxity was too mild to justify the risk or because a planned weight change would have altered the result.
The Hyde Park and Spanishtown Creek subset skewed harder than the average. Nine of the 14 had measurable submental fat on exam. Threads alone would have disappointed nearly two thirds of the people in this neighborhood who asked for them by name.
We think there is a reason for that. This is a walking neighborhood full of people who are already fit. Bayshore Boulevard is four and a half miles of continuous sidewalk, and the people who use it daily tend to arrive lean, active, and convinced that a soft jawline must be a skin problem because everything else responded to training. Submental fat is largely genetic. It does not care how many miles you log. Dr. Grandhige has been operating in Tampa for over 15 years and this pattern is consistent enough that we now screen for it first. The Complication Numbers Most Thread Lift Pages Leave Out
Local pages list side effects. They rarely give you rates. That age finding is why we cap thread count. We do not place more than four barbed threads per side of the lower face. More threads means more entry points, more tissue trauma, and more chances for one of them to dimple or become palpable. A provider who quotes you a large number of threads is selling volume, not planning a result.
Dimpling deserves its own note. Local pages call it temporary and normal. Sometimes it is. Persistent dimpling usually means a thread sat too superficial, and it needs the provider who placed it, not self-massage and patience. The intended plane is the subcutaneous fat, not the dermis.
How Long Results Last, And Why The Range Is So Wide
You will find two different answers across Tampa. One practice tells you six to nine months. Another says 12 to 18. Both are quoting real outcomes. Neither explains the gap.
The threads themselves hydrolyze and disappear within roughly four to six months regardless of who places them. What lasts beyond that is the collagen laid down along the tracks, and that response varies by skin thickness, baseline collagen quality, sun history, and age. Thicker skin with better collagen lands at the long end. Thin, sun-damaged skin lands at the short end, and in a city where the UV index sits high most of the year, plenty of Tampa patients belong in that second group.
In our own follow-up, the 18 patients we treated averaged 13 months before asking about a second session. The clearest visible change showed up between weeks eight and 16, not on day one. Dr. Grandhige follows every patient for a full year, so these are observed intervals rather than manufacturer estimates. Recent multicenter data on facial thread lifting points the same direction, toward technique and patient selection as the variables that matter most.

The Question Nobody In This Neighborhood Is Being Asked
The collagen response that makes threads work also leaves fibrosis along the thread tracks. That tissue change is permanent in a way the threads are not.
If you have a facelift in your future, that scar tissue makes the dissection less predictable. Planes that should separate cleanly do not. A surgeon operating on a face with a history of repeated thread sessions is working through altered tissue, and the surgical literature on prior minimally invasive treatments now includes specific perioperative guidance for exactly this scenario.
No clinic page in Tampa mentions this, because it complicates the sale. Threads are marketed as a low-commitment bridge. For a 38-year-old with mild laxity and no surgical plans, that framing is fair. For a 52-year-old who will likely want a lift in five years, stacking three or four thread sessions between now and then is a decision with a cost attached.
We raise it in the consultation because it belongs there. That approach is not specific to threads, it runs through how Formation practices. Dr. Grandhige limits volume specifically so he can have this conversation instead of skipping it.
What The Procedure And Recovery Look Like At Our South Howard Office
The procedure runs 30 to 45 minutes in our office at 1315 South Howard Avenue, in the yellow brick building across from Fresh Kitchen. Local anesthetic only. You are awake. From most of Spanishtown Creek and Hyde Park that is a five minute drive or a walk of about 15 minutes, and you can get yourself home afterward.
Expect some dimpling or puckering at the entry points on day one. It settles over several days as tissue relaxes around the threads. Swelling and tenderness are normal for the first few days.
