The Formation Thread Threshold
Four checks you can run on yourself before you book anything. They take about two minutes in front of a mirror in good light.
Check One, The Lift And Hold
Place two fingers on your cheekbone and lift the skin toward your temple. Note how far you had to move it before the jowl and the nasolabial fold looked the way you want. A quarter inch of movement sits inside thread range. Half an inch or more does not, and threads will read as a partial result you paid for twice.
Check Two, The Release
Pinch the skin just below your jaw, hold for two seconds, and let go. Watch how it returns. Skin that snaps back holds a thread vector. Skin that settles slowly or stays creased has lost the recoil the procedure depends on, and threads placed into it tend to migrate or dimple. Formation uses a working range of roughly ages 25 to 55 for skin quality on facial contouring work, though the number on your license matters less than what your skin does in this test.
Check Three, Descent Or Volume
Look at yourself in a photo taken five to ten years ago. If the change is that everything moved down, threads address that. If the change is that your midface looks emptier, you have lost volume, and lifting empty tissue makes the hollow more obvious rather than less. Threads move tissue. They do not add it back.
Check Four, The Five Year Question
Ask whether you expect to want a surgical lift within five years. If the answer is probably yes, this is the check that should change your plan, and the reason is in the next section.
What The Complication Data Shows
Reported complication rates for thread lifting are higher than the marketing language around the procedure suggests, and the honest number is not close to zero.
A 2026 meta-analysis in Frontiers in Surgery pooled 26 studies covering 2,827 patients and found bruising in 26 percent of cases, swelling in 16 percent, pain in 11 percent, altered sensation in 10 percent, dimpling or asymmetry in 7 percent, visible or palpable threads in 6 percent, thread exposure in 5 percent, and infection in 2 percent. The authors flag high variability between studies, which means technique and patient selection move these numbers more than the averages imply. A separate series of 160 PDO cases reported an overall early complication rate of 34 percent, most of it barbed suture displacement and transient redness.
Almost none of this is dangerous. Most of it resolves. The reason to read it before booking is that “minimal downtime” and “a 26 percent chance of visible bruising” describe the same procedure, and only one of them helps you schedule around a wedding.
The longer-term finding is the one that should govern the decision. A 2025 study in Aesthetic Plastic Surgery reviewed facelifts performed on patients who had two or more prior absorbable thread procedures, average age 59. Intraoperatively the surgeon found fibrosis, tissue distortion, and fusion of anatomical layers that normally separate cleanly. The threads had long dissolved. The tissue changes had not. Dissection was harder and less predictable as a result.
That is why Check Four exists. Threads are a reasonable bridge for someone who is not ready for surgery. They are a poor choice for someone who is two years out from wanting one.
Larger multicenter work has also found that patient age and the number of threads placed both predict dissatisfaction, defined as returning to the clinic because the result was not enough. More threads is not a stronger result. It is a longer list of places something can go wrong.
When Threads Are The Wrong Tool
If your laxity sits past thread range, Formation will say so and route you rather than sell you a smaller procedure that will not hold.
For patients whose skin quality is the limiting factor rather than position, in-office skin tightening addresses the tissue directly instead of pulling on it. For excess fat under the chin obscuring the jaw, fat removal comes before any question of lifting, and Formation’s approach to neck and jawline contouring combines careful fat removal with skin tightening over the underlying structures. For lift-level and complex facial cases, Dr. Ahmad Saad, a board-certified plastic surgeon at Formation, handles the work that belongs in an operating room.
How Placement Works Here
Placement takes under an hour under local anesthesia, and the plan is set before a single thread goes in.
Dr. Grandhige maps vectors against your own anatomy rather than a template: where the tissue actually needs to travel, which entry points reach it, and how few threads will do the job. Most Formation cases land in the eight to twelve thread range. The restraint is deliberate, and the dissatisfaction data above is the reason for it.
Formation treats aftercare as part of the result, not a postscript. Dr. Grandhige puts the split at roughly 60 percent procedure and 40 percent what happens afterward for surgical work, and for threads the aftercare share is arguably larger, because there is no dissection doing the structural work for you. That means no strenuous exercise or facial massage for two weeks, a follow-up inside the first couple of days to catch dimpling while it is still correctable, and honest expectation setting about the swelling curve. Dr. Grandhige follows facial patients for a full year, and nearly all of them stay through it.

Thread placement itself is an office procedure and does not require that setting, which is worth knowing rather than glossing. Formation’s practice philosophy is built around limited volume and saying no when no is the answer. Consultations are by appointment only, carry a fee, and run as a full assessment rather than a sales conversation. Patients leave with a plan even when the plan is to wait.
Scheduling A Thread Lift From New Tampa

That drive matters for two reasons specific to this procedure. The early follow-up visit falls inside the first 48 hours, which is a second trip down Bruce B. Downs during a week you may have planned to stay home. And New Tampa is built around being outside. The paved seven-mile loop at Flatwoods Park off Bruce B. Downs, the courses at Hunter’s Green Country Club and Tampa Palms Country Club, and the training schedules at the New Tampa Family YMCA all sit inside the two weeks you are asked to hold off. Patients in the 33647 ZIP code make up close to one in nine facial consultations at Formation, and the scheduling conversation with them is usually about the calendar rather than the procedure.
Cumulative sun exposure is the other local factor. Skin that has spent years on a Florida golf course or a Flatwoods loop at midday behaves differently under tension than skin that has not. It shows up in Check Two, and it is a reason two patients the same age can get different answers about candidacy. Deciding Whether Threads Fit Your Timeline
A thread lift is a good answer to a narrow question: mild descent, decent skin recoil, no surgery on the horizon. Outside those conditions it tends to disappoint, and in one specific case, the patient two years from wanting a lift, it can make the next step harder.
