The Four-Layer Neck Read
Most necks show aging in more than one layer at once. Treating one and ignoring the other three is how patients end up disappointed after a technically clean surgery. Before Dr. Grandhige quotes a plan, he reads all four.
Layer One: The Fat
Pinch under your chin. If you can grab a distinct pad and it moves, that’s subcutaneous fat, and liposuction addresses it directly. This layer responds best and fastest. If your neck is mostly a layer-one problem and your skin snaps back when released, you are the ideal candidate and you probably don’t need anything added.
Layer Two: The Platysma And Platysmal Bands
Tip your chin up and clench your jaw. If two vertical cords stand out down the front of your neck, those are platysmal bands, and no amount of fat removal touches them. Remove the fat over a banded platysma and the bands often look worse, because you’ve taken away what was hiding them.
Bands need the muscle addressed. MyEllevate, a permanent suspension suture, combined with platysmaplasty, is the repair when the muscle edges have separated. For mild banding in a patient who isn’t ready for surgery, Botox, a botulinum toxin, softens the cords temporarily. It doesn’t fix them.
Layer Three: The Skin And Fibroseptal Network
Underneath your skin sits a web of collagen fibers, the fibroseptal network, that anchors skin to the tissue below. Age, sun and weight swings break those fibers down, and once they go, skin hangs regardless of how much fat is removed.
This is the layer that separates a good submental result from a hollow one. Renuvion, a helium plasma and radiofrequency device, and FaceTite, a radiofrequency contouring device, work from underneath the skin and reach that fiber network.
