The Four-Layer Neck Read

Dr. Grandhige reads every jawline in four layers before recommending anything. Each layer has its own cause and its own answer. Filler lives in exactly one of them.

Layer One, The Fat Under Your Chin

Pinch the tissue beneath your chin between two fingers. If you can gather a soft roll, fat is blurring the line between your face and your neck. Fat comes out. It does not get filled around. Neck liposuction removes it through small incisions under the chin and behind the ears, and it is one of the procedures Formation performs most often.

Layer Two, The Platysma And Its Bands

Clench your teeth and look at your neck in a mirror. If two vertical cords stand up from your collarbone toward your jaw, those are platysmal bands. Filler cannot reach them. A suspension suture placed with platysmaplasty supports the platysma from underneath and releases the bands. The suture is nonabsorbable and can be removed later if needed, which separates it from barbed threads that dissolve within months.

Layer Three, Skin Quality

Press the skin along your jaw and release it. Skin that snaps back has recoil. Skin that settles slowly has lost elasticity. The window where energy-based skin tightening does useful work runs roughly from skin age 25 to 55. Past that, tightening tends to under-deliver on what the patient pictured.

Layer Four, Chin Projection And Jaw Angle

Look at yourself in profile. Draw a vertical line down from your lower lip. If your chin sits well behind it, the bone is not projecting enough, and no amount of fat removal will fix that. This is filler’s layer. Juvederm Volux, a hyaluronic acid gel, and Radiesse, a calcium hydroxylapatite, both add structure here. The Formation Add-Or-Subtract Rule

Every jawline complaint resolves to one question. Are you short on structure, or carrying excess?

The FDA’s own trial data on Volux XC makes this concrete in a way no marketing page bothers to report. In the pivotal study, 81.5 percent of participants at six months were satisfied with how sculpted their jawline looked, compared with 12.2 percent at baseline. Strong result. But on the separate question of how smooth their lower face looked, meaning no jowls or folds of fatty skin, only 70.5 percent were satisfied at six months.

Eleven points of daylight between those two scores, measured inside the manufacturer’s own approval study. Filler did what filler does, which is sculpt. It did noticeably less for the fat. If your problem is layer one, the number that applies to you is the lower one.

Durability And Volume, Using The Numbers On The Label

The local market standard is a line about results lasting 12 to 24 months. The FDA labeling tells a more specific story, and it is a curve rather than a flat number.

Juvederm Volux XC is indicated for improvement of jawline definition in adults over the age of 21 with moderate to severe loss of jawline definition. It reached the market through the FDA’s premarket approval pathway, which requires a controlled clinical study. That is a different and higher bar than the 510(k) clearance route used by most energy-based tightening devices, and it is why filler durability data exists at all.

In that study, the responder rate ran 78.7 percent at one month, 76.0 percent at three months, 69.9 percent at six months, 64.9 percent at nine months, and 61.3 percent at twelve months. So roughly six in ten still held a measurable improvement at the one-year mark. Not zero, and not the same as “lasts two years.”

The volume numbers are the bigger surprise. Local pages talk about one to four syringes. The labeling states that the typical volume to achieve optimal improvement in jawline definition is 6.8 mL, the median total injected across initial and touch-up treatments combined, and that injection of more than 9 mL for jawline definition has not been studied. There is also an annual ceiling: patients should be limited to 20 mL of any Juvederm gel per 60 kg of body mass per year, because the safety of injecting greater amounts has not been established.

The Touch-Up Nobody Schedules For

Here is the operational detail that changes how a Bradenton patient should plan.

In the pivotal study, 129 of the 156 treatment group participants came back for a touch-up treatment about 30 days after the initial injection. That is more than four in five. A touch-up is not a sign something went wrong. It is the normal shape of the treatment, and the labeling builds it in.

For someone living in South Tampa, that is a short second visit. For someone driving up from Manatee County, it is a second round trip. Formation tells Bradenton patients this at the evaluation rather than at the follow-up, so the calendar and the drive get planned once.

Swelling behaves on a similar timeline. Per the trial diaries, most injection site responses were mild or moderate and lasted 14 days or less, though 35 percent lasted between 15 and 30 days. Lumps and firmness were among the longest-running. If you have an event, count backward four weeks, not four days.

Health and diet get assessed before treatment, not after. Nutrition, supplements and peptides are adjusted in the window immediately before and immediately after so the tissue is in the best state it can be in when the gel goes in and while it settles. Bruising and swelling respond to how inflamed you already were on the day. Most practices treat that as outside their scope.

The aftercare instruction that catches Florida patients out is in the labeling itself. Avoid makeup for 12 hours, and avoid strenuous exercise, extensive sun or heat exposure, and alcohol for the first 24 hours. On the Gulf coast in July, “extensive heat exposure” is not a theoretical restriction. Book around the beach day rather than before it.

Planning The Drive From Bradenton

Formation is at 1315 South Howard Avenue, Suite 102, in South Tampa. From Bradenton that is roughly 50 miles and about an hour when traffic cooperates.

Two routes work. I-75 north to the Selmon Expressway and west into South Tampa is the more predictable of the pair. I-275 across the Sunshine Skyway through St. Petersburg and over the Howard Frankland to the Howard Avenue exit is the prettier one and carries a toll. Patients coming from Lakewood Ranch usually take I-75. Patients coming from the west side, closer to Anna Maria Island or the Cortez corridor, more often take the Skyway.

When Filler Is The Wrong Answer

Some honest limits.

If you have real submental fat, filler will sharpen the bone line and leave the fullness sitting right underneath it. The jaw looks stronger and the neck looks the same. Addressing the fat first, then assessing whether structure is still short, produces a better result and often uses less product overall. Formation treats this through neck and jawline contouring rather than injectables.

If your skin has lost most of its recoil, filler adds weight to tissue that is already struggling to hold position. That tends to age poorly.

If you are past what contouring and tightening can reach and you need skin removed or a formal lift, Formation will say so and point you to the surgeons who do that work well. Staying in contact with those surgeons is part of how the practice operates. Questions Worth Asking Any Injector Before You Book

Ask who is holding the syringe and confirm their license through the Florida Board of Medicine. Ask what total volume they are planning and how that sits against the 9 mL study ceiling. Ask whether they inject with a needle or a cannula, and note that in the pivotal study the cannula responder rate was 73.5 percent against 66.7 percent for needle only, with a lower treatment-related adverse event rate as well, 6.5 percent versus 9.4 percent.

Then ask about vascular emergencies. The labeling is direct that the product must not be injected into blood vessels, and that intravascular introduction may lead to embolization, occlusion, ischemia, or infarction, with rare but serious reported events including vision impairment, blindness, cerebral ischemia or hemorrhage leading to stroke, and skin necrosis. It also instructs practitioners to keep reversal supplies immediately available per American Society for Dermatologic Surgery guidelines. An injector who cannot describe that protocol without checking is telling you something.

For context on overall risk, 16 of 198 participants, 8.1 percent, had a treatment-related adverse event, all mild or moderate, with none severe. The most common was difficulty chewing at 2.0 percent, followed by injection site nodules at 1.5 percent.

A Clearer Jaw Line, Read Correctly The First Time

The reason most jawline filler results disappoint is not the product. It is that the wrong layer got treated. Fat filled around instead of removed. Loose skin loaded instead of tightened. A recessed chin left alone while the jaw angle got all the attention.
