What A Tummy Tuck Actually Repairs
Abdominoplasty was the third most common cosmetic surgical procedure in the United States in 2024, with 171,064 performed, a one percent increase over the year before. That volume is not driven by vanity. It is driven by two events that structurally change the abdominal wall: pregnancy and significant weight loss.
The American Society of Plastic Surgeons has tied a growing share of that demand to patients finishing weight loss on GLP-1 medications and then facing skin that will not retract. Ninety-eight percent of those 171,064 procedures were performed on women.
Here is what the operation addresses, in the order a surgeon evaluates it. Excess skin is removed. The rectus muscles are brought back together if they have separated, a condition called diastasis recti. The remaining tissue is redraped. Liposuction may be added to refine the transitions at the flanks and waist.
The result is not only cosmetic. Patients frequently report better posture and less back discomfort after muscle repair, because the abdominal wall is load-bearing structure, not decoration.
The Formation Four-Layer Check
Most consultations start with a photograph. Formation starts with four layers, because a tummy tuck touches all four and the answer changes depending on which ones are the problem.
Layer One: The Skin Envelope
Stand up straight and look. If the loose skin hangs when you are upright and does not tighten when you flex, the skin envelope has lost elasticity and will not recover on its own. Skin that only creases when you sit is a different problem and may not need excision at all. This single distinction decides whether a full abdominoplasty or a mini abdominoplasty is the right operation.
Layer Two: The Fat Layer

Patients who carry weight deep in the abdomen are told to address that first. This is the most common reason Dr. Grandhige tells a patient to wait.
Layer Three: The Abdominal Wall
Lie flat and lift your head. If a ridge or a gap appears down the midline, that is separation. It does not respond to core work. Crunches often make the bulge more visible rather than less. Muscle repair is the part of the procedure that liposuction cannot substitute for, which is why some patients need both the excision and contouring in one operation, and why a tummy tuck carries a longer recovery than a contouring procedure alone.
Layer Four: The Recovery
Formation treats this layer as clinical, not administrative. Dr. Grandhige puts it plainly: the surgery is sixty percent of the result and the post-operative care is forty percent. Poor recovery can undo good surgery. If you cannot protect the first two weeks, the honest answer is to schedule later.
What Changed In Abdominoplasty Technique
Most neighborhood pages describe recovery in terms that were accurate twenty years ago. The practice patterns have moved.
A member survey published in 2025 compared current abdominoplasty practice against a benchmark survey from 2006. Drain use fell from ninety-eight percent of surgeons to 82.5 percent. Progressive tension sutures, which close the dead space internally instead of relying on suction, are now used by 49.8 percent. Tranexamic acid appears in 38.3 percent of cases. Power-assisted liposuction as an adjunct rose from fourteen percent to 50.7 percent.
That matters because a separate 2024 review in the Aesthetic Surgery Journal found progressive tension suturing significantly reduced seroma rates compared with drains, with no increase in hematoma or infection. It is the reason drain-free approaches have become viable for a wider range of patients than they were a decade ago.
So ask specific questions at any consultation, here or elsewhere. Do you use progressive tension sutures. What is your current drain rate and how long do drains stay in. Do you use tranexamic acid. Will liposuction be combined with the excision. A practice that cannot answer those in plain language is describing a procedure rather than performing one.
Recovering In Old Seminole Heights
Old Seminole Heights presents one recovery problem that South Tampa does not, and one advantage that most of Tampa lacks.
The problem is the housing. Old Seminole Heights is Tampa’s bungalow district, largely built between the 1910s and the 1930s, and the original craftsman and Spanish revival layouts almost always put the front door at the top of a raised porch. For the first week after abdominoplasty you will walk bent slightly forward at the waist and you should not be negotiating porch steps repeatedly. Patients from the neighborhood are advised to set up a ground-floor recovery space before surgery, keep one entrance clear, and arrange help for the first several days. A nurse is provided for the first twenty-four hours.
The advantage is walking. Early movement is not optional after this operation, it is what protects against clots and moves fluid. Rivercrest Park at 4802 North Boulevard has a flat loop of roughly seven-tenths of a mile along the Hillsborough River, which is close to ideal for a week-one walk. It is short enough to turn around, level the whole way, and the historic canopy over the neighborhood streets means you are not walking in open Florida sun. Most patients from the area work up from a single lap to several over the first two weeks.
The drive matters too. Post-operative visits at Formation are frequent, and lymphatic treatment begins at the first visit. From Old Seminole Heights, the run down Florida Avenue or Nebraska Avenue to South Howard is about twenty minutes outside of rush hour. That pathway is unusual and it is directly relevant here. Abdominoplasty is abdominal surgery. A surgeon whose background includes complex abdominal operations and critical care is reading tissue behavior, healing biology, and risk differently than someone trained only in cosmetic technique. Dr. Ahmad Saad, a board-certified plastic surgeon, practices alongside him.
Dr. Grandhige personally evaluates every patient, designs the plan, performs the operation, and follows patients for a full year. Surgical volume is limited on purpose. All procedures are performed in Quad A accredited surgical facilities with the same anesthesia provider and the same operating room team on every case.
One more thing, and it applies to every practice you are considering. Florida publishes license status and disciplinary history for every physician in the state, free, through the Department of Health license verification portal. Look up any surgeon before you book, including ours.
