The Four-Signal Jawline Fit Check
Formation runs every neck and jawline candidate through four signals before scheduling anything. Three of four usually means the procedure will do what you’re hoping for. Two or fewer means something other than fat is driving the problem.

Signal One, Pinch Versus Push
Sit upright and pinch the tissue under your chin between two fingers. If you can gather a soft roll, that’s subcutaneous fat and liposuction reaches it. If the area feels firm and pushes back instead of gathering, the fullness sits deeper than a cannula goes.
Signal Two, The Cord Test
Clench your jaw and look at your neck in a mirror. If two vertical cords stand up along the front, those are platysmal bands. Fat removal won’t soften them. In some necks it makes them more obvious.
Signal Three, Skin Recoil
Pinch the skin along your jawline, hold two seconds, then let go. Skin that snaps back will redrape over a new contour. Skin that settles slowly needs energy-based tightening at the same visit, or a lift instead. Skin roughly in the 25 to 55 range tends to recoil well enough.
Signal Four, The Weight Line
Your weight should have held within a few pounds for at least three months. Neck fat tracks weight closely. Operating on a moving target gives you a result that shifts underneath you.
About one in four people who come to Formation asking for neck liposuction leave with a different plan, because the fit check points somewhere other than fat. Dr. Grandhige would rather tell you that in a consultation than have you find out at month six.
How The Procedure Runs At Formation
Neck liposuction at Formation is an office procedure. You take an oral sedative, the area is numbed with local anesthetic, and the work happens through very small incisions under the chin and behind the ears. Fat comes out in controlled passes that deliberately preserve a thin subcutaneous layer, because stripping that layer is what creates the hollow, skeletonized neck that ages badly. An energy-based skin tightening device, FDA-cleared for use in the neck and submental region, is then used under the skin to help it contract over the new contour.
No general anesthesia. No hospital. You go home the same afternoon with someone else driving.
Why Fat Volume Decides Where Surgery Happens
Florida regulates office surgery by volume, not by body part. Any office where a physician removes more than 1,000 cubic centimeters of supernatant fat has to register with the Florida Department of Health, pass a preregistration inspection, and be inspected annually unless it holds national accreditation. The ceiling for liposuction in any office setting is 4,000 cc. Surgeons also have to keep a formal log of every case above the 1,000 cc line. A neck case does not come close to those numbers. At Formation, a typical submental and jawline case removes somewhere between 45 and 90 cc. That’s the reason this procedure is comfortable under local anesthetic with an oral sedative while a Lipo360 case is not.
The framework around office surgery is tightening this year. Florida House Bill 1175 took effect on July 1, 2026, creating section 553.884 of the Florida Statutes and directing the Florida Building Commission and the State Fire Marshal to write safety design standards specific to office surgery suites by January 1, 2027. Those standards will work as an alternative to ambulatory health care occupancy rules and will cover offices treating up to six patients at a time on an outpatient basis. The bill text is available through the Florida Senate’s record for HB 1175. If you’re comparing Tampa offices this year, asking how a practice is preparing for those standards is a fair question.
Recovery And The 60/40 Standard
Dr. Grandhige treats surgery as 60 percent of the result and aftercare as 40 percent. That isn’t a line about attentiveness. It’s a claim that a well-executed procedure can be undone by poor compression and uncontrolled inflammation in the first month. Recovery at Formation is managed, not waited out. Days One Through Seven
Mild swelling, bruising, and tightness. You wear a chin compression garment. Most people are back at work and social activity inside a week, and roughly eight in ten Formation neck patients are back at a desk within five days.
Weeks Two Through Four
Swelling keeps improving and the liposuction result becomes readable. The garment stays on 12 to 24 hours a day. Lymphatic treatment starts at your first post-operative visit and continues through this window, which is the part that separates a clean recovery from a lumpy one.
Month Three
Skin tightening becomes visible and the jawline shows real improvement. This is usually when other people start noticing something changed without being able to say what.
Months Six Through Nine
The final result. Optimal skin tightening is slow and it does not finish at three months. Anyone quoting you a final answer at six weeks is describing a marketing timeline, not a biological one.
Who Performs The Procedure
Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He attended medical school at the University of Michigan and matched into a six-year combined plastic surgery residency at Yale before shifting into a full general surgery residency, followed by fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. He’s certified by the American Board of Surgery and has been operating for over 15 years.
Dr. Ahmad Saad, a board-certified plastic surgeon, is also on the Formation team.
Surgical cases run in Quad A accredited facilities with the same anesthesia provider and the same operating room staff every time. Dr. Grandhige employs his physician assistant directly, and that PA assists on all of his surgeries. Formation limits surgical volume deliberately, which is what makes the year-long follow-up protocol possible rather than aspirational.
What The National Numbers Show
ASPS member surgeons performed 24,000 submental and chin liposuction procedures in 2024, a one percent increase over 2023. Liposuction overall remained the most common cosmetic surgical procedure in the country at 349,728 cases. Neck lifts came in at 22,445, up two percent. Two things stand out. Submental liposuction and neck lift volumes are nearly identical, which tells you how often the choice between them comes down to anatomy rather than preference. And demand held steady through a period when GLP-1 medications were widely expected to reduce interest in targeted fat removal. It didn’t happen. Weight loss changes volume, not the shape of the fat compartment under your chin.
