Because the procedure runs with you conscious, it works the same way awake liposuction does elsewhere on the body: you can respond during contouring rather than waking up to the outcome.
Layer Two, The Platysma
The platysma is the flat sheet of muscle running up the front of your neck. When its edges separate with age, they show as two vertical cords, the platysmal bands, that stand out when you clench your jaw. Suction does nothing to them. Where bands are the problem, the answer is suspension: MyEllevate, a nonabsorbable suture that lifts and holds the muscle, combined with platysmaplasty. Layer Three, The Skin And Fibroseptal Network
Under your skin sits the fibroseptal network, the connective tissue scaffold anchoring skin to the structures below it. That scaffold decides whether skin redrapes once fat is gone. Energy-based tightening reaches it. FaceTite, a radiofrequency-assisted device, and Renuvion, a helium plasma device, both work from beneath the skin at that depth. Lasers and microneedling work from the surface down and cannot get there. Both devices are 510(k)-cleared rather than FDA-approved, and that distinction is worth knowing when you read anyone’s marketing.
Layer Four, Chin Projection And Jaw Angle
Some necks look full because the chin sits back, not because there is extra tissue. A recessed chin shortens the distance between jaw and neck and makes an ordinary amount of fat read as a double chin. Structural filler addresses that: Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed at the chin point and along the jaw angle. Fat removal alone will not build a frame that was never there. Layer four decides whether you finish with a superhero jawline or just a thinner neck.

What The Numbers Show
The largest published series on this procedure followed 987 neck liposuction patients treated under tumescent local anesthesia. Five developed temporary weakness of the marginal mandibular nerve, about half of one percent. One had submandibular gland ptosis. One had arterial bleeding. Seroma, skin necrosis, scarring and hyperpigmentation did not occur in that group at all.
Read the limitation next to the number. That series is retrospective, it has no control group, and all 987 cases came from three high-volume surgeons using microcannular tumescent technique. It tells you what the procedure looks like in experienced hands. It does not tell you what it looks like in every pair of hands, and it does not cover cases where an energy device is added.
A second series of 320 neck cases reported no serious complications, with four patients needing closer follow-up. Its conclusion is the one to carry into a consultation: neck liposuction calls for more monitoring than body liposuction, not less. The structures at risk sit millimeters from the working plane.
Florida sets the outer limits for office liposuction. The Board of Medicine requires a physician to register the office with the Department of Health once a case removes more than 1,000 cubic centimeters of supernatant fat, and 4,000 cc is the ceiling for any office setting. Neck volumes sit far below both. As of July 1, 2026, HB 1175 added another layer, directing the Florida Building Commission and the State Fire Marshal to write safety design standards for office surgery suites by January 1, 2027.
When Neck Liposuction Is The Wrong Tool
Dr. Grandhige says no to this procedure more often than people expect. Four situations account for most of those conversations.
Skin without recoil. Pinch the skin under your jaw and release it. If it returns slowly or stays creased, taking the fat out from beneath leaves you with the same skin over less volume. Elasticity generally holds in the range of skin age 25 to 55.
Bands as the main complaint. If what bothers you in the mirror is two vertical cords, suction will not touch them. That is a layer two problem with a layer two answer.
A recessed chin. Removing fat from a short neck with no chin projection can leave the profile weaker rather than sharper. Some patients in this position do better with non-surgical neck and jawline contouring first.
Skin that needs excision. Some necks need tissue removed, not tightened. That is a neck lift, and Dr. Grandhige does not perform it. Formation keeps working relationships with the surgeons who do that operation well and sends patients straight to the right one. Being told no is part of what the assessment is for.
The 60/40 Recovery Standard
Dr. Grandhige’s rule at Formation is that the operation accounts for sixty percent of your result and the care afterward accounts for the other forty. Post-operative care done badly can undo surgery done well.
That ratio shapes what happens before you ever reach the table. Your health and diet get assessed first. Nutrition, supplements and peptides are adjusted in the window right before and right after the procedure so your body builds as much structure as it can while it heals. Lymphatic treatment starts at your first post-operative visit rather than weeks down the line. The same anesthesia provider and the same staff work every one of his cases, and his own physician assistant assists on all of them. Follow-up runs a full year, and nearly every patient stays for it.
The cannula is the same instrument in every practice in Tampa. What differs is the thinking around it, which is the short version of our philosophy.
Your Recovery, Week By Week
First few days. Mild swelling, bruising and tightness. You wear a chin compression garment.
One week. Back to work, back to social plans, back to training. The garment stays on 12 to 24 hours a day.
Two to four weeks. Swelling keeps settling and the result of the fat removal becomes clear.
Three months. Skin tightening shows, and the change along the jawline is substantial by this point.
Six to nine months. Final result, once tightening has finished.
Getting Here From New Suburb Beautiful
Howard Avenue is your eastern boundary. Formation sits on it, roughly a third of a mile south of the Morrison Avenue corner.

New Suburb Beautiful was platted in the 1920s inside the Hyde Park district, and it is small: 340 households at the last census, all in ZIP 33629. Discretion carries weight in a neighborhood that size. The openings used for neck liposuction sit under the chin and behind the ears and heal to almost nothing. The visible part of your recovery is swelling, not scarring, which is easier to see in the before and after gallery than to describe.

Neck liposuction is a small operation that changes how your face reads from the side. It works when fat is the problem, your skin still recoils, and your chin gives the jaw something to sit on. When those things are not true, a good surgeon tells you so and points you somewhere better.
The published data on neck liposuction under local anesthesia is reassuring in experienced hands, and the honest reading of it is that experience is the variable. Dr. Grandhige spent more than fifteen years across general surgery, burn surgery and critical care before concentrating on aesthetics, and he limits how many cases he takes so he can stay involved through the year that follows.
