Before anything gets planned, Dr. Grandhige reads your neck across four layers by hand. Each layer has its own answer, and the answers are not interchangeable. Most people arrive certain they have a fat problem.
Layer One, Fat
Chin down, jaw relaxed. A pinchable layer between skin and muscle means there is something to remove. Nothing to pinch means no amount of fat removal changes your profile. This is where neck liposuction does its work, and the fat cells taken out do not come back.
Layer Two, The Platysma And Platysmal Bands
The platysma is a thin sheet of muscle running from the chest up over the jawline. As its edges thicken and separate they read as vertical cords, called platysmal bands, and they blunt the jaw even on a thin neck.
Two answers here, depending on whether the banding shows up only when you contract or sits there at rest. For dynamic banding, a neurotoxin reduces the muscle’s pull. Botox, a botulinum toxin type A, received FDA approval in October 2024 for moderate to severe platysma bands, dosed at 26, 31 or 36 units by severity. That was its first approved use below the face. The published limitation matters as much as the approval: the effect is temporary, it gets repeated on a schedule, and it does nothing to fat or loose skin. It is the same mechanism behind Botox for turkey neck.
When the band is structural rather than dynamic, the answer is MyEllevate, a permanent suspension suture, paired with platysmaplasty. The suture is nonabsorbable and can be removed if it ever needs to be, unlike barbed threads that dissolve on their own. The lift is there the day it is placed rather than arriving over months. Layer Three, Skin And The Fibroseptal Network
This layer decides your entire plan. We test elasticity directly instead of guessing from your birthday. Skin in the 25 to 55 range clears this layer most often, but sun exposure, weight history and genetics move people in both directions. A 52 year old can pass cleanly. A 38 year old six months out from a fast 40 pound loss often cannot.
When skin will not retract on its own, Renuvion, a helium plasma energy device, or FaceTite in St. Petersburg, a radiofrequency contouring device, goes in over the new shape. The differentiator is depth of access, not the brand name. Both reach the deepest layer of the skin and the fibroseptal network beneath it, the connective tissue foundation that spans skin to muscle. Lasers and microneedling work from the top down and cannot get to that foundation. Both devices are 510(k)-cleared, which is a different regulatory category from an approved drug. Collagen keeps remodeling in that layer for months afterward, which is why the result is not finished at week four.
Layer Four, Chin Projection And Jaw Angle
Your frame sets the ceiling on your result. A recessed chin behind a full neck is a different problem from a strong jaw hidden by fat, and they need different plans. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, add projection along the bone. Neither removes fat or tightens skin. Volume added above a loose neck reads heavier rather than sharper, which is why sequence matters. Where the read lands: clear all four and liposuction alone usually does it, because the skin contracts over the new shape by itself. Pass layers one, two and four but sit borderline on three, and tightening gets added in the same visit. Fail layer three outright and contouring will not hold, which you will hear at the consultation rather than at six months.
If You Lost Weight On A GLP-1, Start At Layer Three
If your jawline got worse after losing weight on a GLP-1 medication, you are looking at a skin problem wearing a fat problem’s clothes. Treating it like a double chin makes it worse.
The story repeats almost word for word. The face thinned. The fullness under the chin mostly resolved. Then the skin along the jaw stayed loose, because volume left faster than skin could shrink to meet it. In the four-layer read these patients sail through layer one and stall hard at layer three. That is the whole mechanism.
It matters because the standard plan backfires here. Take out what little fat is left and you hand loose skin even less to drape over. The result gets hollower and looser at the same time. The plan for a post-weight-loss neck is usually inverted: conservative fat removal, sometimes none, with the real work aimed at the skin.
Timing is part of the plan. We would rather wait until your weight has held steady, because skin still catching up to ongoing loss is a moving target, and the neck you plan in March is not the neck you will have in July.
What Contouring Will Not Do
Four honest limits, because the wrong expectation ruins an otherwise good result.
It will not help sleep apnea. People ask, because neck circumference is a known risk marker. Neck liposuction removes subcutaneous fat, the layer between skin and muscle. The fat that narrows an airway sits deeper, near the tongue base and voice box, behind muscle and cartilage, and cosmetic contouring cannot reach it. This changes how you look, not how you breathe. Apnea is a sleep study and a medical workup, and we will say so.
It will not hold a neck that has passed what tightening can manage. That needs skin removed directly, which is not a procedure Dr. Grandhige performs. Formation stays in contact with the surgeons who do that work well and points you to the right one rather than selling you something smaller that will disappoint you.
It will not fix fullness that turns out to be an enlarged salivary gland or a thyroid issue. Those get evaluated medically before anything cosmetic happens.
And it will not produce a change people can point to. If you want an obvious result, this is not the practice for it.

What Recovery Looks Like
The First Few Days
Mild swelling, some bruising, a tight feeling under the chin. You wear a chin compression garment, walk early and often, and sleep with your head elevated. You will look worse before you look better, and that is swelling rather than the result.
Week One
Back to work and back to social plans. The chin garment stays on 12 to 24 hours a day through this stretch. Your first post-op visit lands inside this window. Most patients are back to training at one week.
Weeks Two To Four
Swelling settles and the liposuction result becomes visible. Garment hours taper.
Three To Nine Months
Skin tightening shows around three months, with a clearly sharper jawline. The final result usually lands between six and nine months, once tightening finishes.
Call us instead of waiting if swelling worsens after week two rather than improving, or if you notice a firm tender lump, a fever, or numbness spreading past the treated area. None of that is typical. Every case, in the office or the operating room, runs in a facility accredited by QUAD A with continuous monitoring and board-certified anesthesia professionals. Dr. Grandhige works with the same anesthesia provider and the same staff on every case, and his own physician assistant scrubs in on all of them.
What We Do Before You Ever Get Numb
Your health and diet get assessed before treatment, not after. Nutrition, supplements and peptides are adjusted in the window immediately before and immediately after, so your body builds the most structure it can while it is healing. Compression, lymphatic support and movement are written into the plan from day one. Dr. Grandhige follows patients for a full year.

The 60/40 Recovery Standard

Crossing The Bay From St. Petersburg
Our office sits at 1315 South Howard Avenue in South Tampa, 33606. Two routes get you here from St. Petersburg, and they fail at different times of day.
I-275 north over the Howard Frankland is the direct line, then east into South Tampa off the Kennedy corridor. Roughly half an hour when the bridge is behaving. The northbound span in the morning peak is the pinch point, which is a decent argument for a mid-morning appointment. The Gandy Bridge into South Tampa by way of Gandy Boulevard and Dale Mabry is the alternate, and it is often the calmer choice on the way home in the evening. Street parking along the SoHo stretch of Howard tightens up at lunch and after 7, though there is usually room a block off the main run.
Surgery day is the easy part. Because the procedure runs in our office under local anesthesia with an oral sedative, you are not booking a hospital day, an overnight, or weeks away from work. You drive home across the bay the same afternoon.
The follow-up cadence is what St. Petersburg patients underestimate. You will make that crossing several times over your first year, and some of those visits land while you are still swollen and wearing a chin garment. Map that out before you book. For patients who would rather not drive back the same evening, Formation has relationships with nearby hotels at discounted rates, and Dr. Grandhige visits patients in their rooms so they do not have to come into the office in those early days. Living in St. Pete, you probably will not need that. It is there if you do.
What Recovery Looks Like
Two ways in, and they are different things.
The in-office consultation with Dr. Grandhige carries a fee, because it is a full surgical assessment rather than a sales meeting, and that fee can be applied toward any procedure or product at the practice. And a recommendation on timing, which for post-weight-loss patients is often not yet.
We do not publish figures for this procedure, because the plan is not standardized. Whether tightening gets added, whether your case runs in the office or the operating room, and how much correction you need all move it. You leave with a real number for your plan.
