What A PDO Thread Lift Does
A PDO thread lift repositions loose tissue along the jawline and lower face using absorbable barbed sutures placed under the skin. The lift is present the day it is placed. That immediacy is the main reason patients pick it over treatments that build slowly.
The threads are polydioxanone, the same absorbable suture material general surgeons have used for decades. Small barbs along each thread catch soft tissue and hold it in a higher position. Placement takes under an hour in the office under local anesthesia. Most Brandon patients drive themselves home the same afternoon.
Threads handle three complaints well. Early jowling. A jaw corner that has gone soft. Mild descent through the midface. At Formation, thread placement is treated as a select minimally invasive tool, used where the anatomy calls for it rather than as the default answer to a soft jawline.

The Four-Layer Neck Read
Most necks and jawlines show aging in more than one layer at once. That is why a single treatment rarely resolves the whole complaint, and why a patient can leave a thread appointment technically lifted and still unhappy. Dr. Grandhige reads the area in four layers. Each layer has its own answer.
Layer One, Fat Under The Chin
Submental fat sits between skin and muscle and blurs the border between jaw and neck. Threads do not remove fat. Reducing it is a liposuction question, handled in the office through small incisions under the chin during neck and jawline contouring. Pulling on a heavy chin with sutures relocates weight without reducing it. The result reads as strain rather than definition.
Layer Two, The Platysma And Platysmal Bands
The platysma is the broad sheet of muscle across the front of the neck. As its medial edges separate and tighten with age, they surface as vertical platysmal bands. Threads pass above this layer and cannot reach it. The answer at layer two is MyEllevate, a permanent suspension suture, combined with platysmaplasty, and it belongs in the plan only when bands are genuinely present. Plenty of patients who think they need banding treatment have layer one instead.
Layer Three, Skin Quality
Skin that has lost recoil will not hold a lift for long, whatever is placed beneath it. Energy-based tightening works at the deeper skin and the connective foundation under it, which is the territory of FaceTite, a radiofrequency contouring device. Threads travel through skin without changing its quality. Pinch the skin under your jaw and let go. If it drifts back rather than snapping, layer three is in play and threads alone will disappoint.
Layer Four, Chin Projection And Jaw Angle
Some jawlines look soft because of the bone underneath, not the tissue on top. A short chin or a shallow jaw angle needs volume built at the frame. Structural injectable fillers do that work: Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, both add projection where the skeleton runs short. No amount of pulling substitutes for a frame that was never there.
Read across all four, threads occupy a narrow band. Soft tissue that has descended slightly, on skin that still recoils, over a frame that is already adequate. That patient exists. That patient is not most patients, and saying so on a page selling the procedure is the point of the read.
Who The Threads Actually Fit
The candidate profile is specific rather than broad. Reasonable skin elasticity, which in practice means a skin age of roughly 25 to 55. Early to moderate descent rather than heavy laxity. A jaw and chin that do not need building. Someone after refinement without fillers or a more invasive operation.
Dr. Grandhige evaluates every patient personally and designs the plan himself. His fellowship training covered burn surgery, critical care, and minimally invasive metabolic and foregut surgery. That reads as unrelated to a facial suture until you consider what a thread lift actually asks: how tissue behaves under sustained tension while it heals. Dr. Grandhige spent years in environments where that question had immediate consequences.
He also declines. Sometimes a procedure is not offered at all, and the plan a patient leaves with is to address something else first. Hesitation from a patient is usually a sign they are taking the decision seriously, not a sales objection to overcome.
One piece of regulatory language is worth reading before you commit. The 510(k) record for barbed PDO sutures cleared for face suspension describes the device as fixating tissue in an elevated position temporarily. Temporarily is the word in the cleared indication, reviewed by the FDA. A thread lift is a holding action with a collagen response attached, not a structural repair.
When A Thread Lift Is The Wrong Tool
Three situations send a patient away from threads at Formation.
Heavy laxity with a standing fold of skin. Threads pull that skin tighter in one direction and leave it bunched in another. Patients past what tightening or suspension can reach get referred outward. Formation stays in contact with the surgeons who do excisional facial work well and points patients to the right one rather than shrinking the plan to fit the menu.
Bands rather than descent. Covered at layer two, and worth repeating because it is the most common misread.
Expecting permanence. Absorbable threads dissolve. The barbs release their hold as the material breaks down, and what remains is the collagen response along the thread tracks, which is subtler than the original lift. A nonabsorbable suspension suture behaves differently because it stays and can be removed. Both are legitimate. They are not the same product with different names.
Side effects deserve real numbers rather than reassurance. A pooled analysis of 26 thread lift studies published in Aesthetic Plastic Surgery reported skin dimpling in roughly 10% of cases and threads that became visible or palpable in roughly 4%, with swelling the most frequently reported effect and infection near 2%. The authors were explicit that they ran the review because reported complication rates across the literature remain contested, and the studies they pooled varied in technique, thread type, and follow-up. Preparation And Recovery At Formation
Health and diet are assessed before anything is placed.
