A PDO thread lift uses absorbable barbed sutures, passed under the skin through small entry points, to draw loose cheek and jowl tissue back toward where it used to sit. You see the lift the same day, with no incision and no general anesthesia. The variable that decides your result is candidacy rather than the thread itself, and at Formation a surgeon settles that question before anyone books you a treatment slot.
A thread lift runs on two clocks, and separating them explains most of the disappointment in this category.
The first clock is mechanical. Barbed sutures engage the tissue and reposition the cheek and jowl, and that part is visible before you leave. It fails when there was more loose skin than a suture could ever carry.
The second clock is biological. The threads are polydioxanone, an absorbable suture material used in surgery for decades, and as they break down over roughly six to nine months the body deposits fibrous tissue along the tracks they leave behind. That half fails when the skin doing the healing has little left to contribute.
Threads refine. They handle early to moderate laxity and they stop at the point where real skin excess starts, and a jaw angle that was never built in bone will not respond no matter how many go in. Formation plans a PDO thread lift in Tampa the way Dr. Grandhige plans contouring surgery, with your anatomy setting the pattern instead of a menu item.
A precision point before you start shopping. PDO threads are not FDA-approved. They are 510(k)-cleared Class II devices under 21 CFR 878.4840, the classification covering absorbable polydioxanone surgical sutures, and the indication that classification governs is soft tissue approximation. Clearance comes from showing equivalence to an earlier device, frequently on bench and animal testing alone, and several recently cleared barbed PDO systems carry indication language stating the device is for dermatological use only and is not intended for lifting and supporting tissues. Ask whoever treats you for the 510(k) number and the indication language of the specific threads going into your face.
Before Dr. Grandhige plans a thread pattern, he reads the jaw and neck in four layers and matches each one to its own solution. Threads answer exactly one of them. Most necks show aging in more than one layer at once, which is why a one-tool plan so rarely delivers what the patient pictured.
Layer one, fat. Fat under the chin and along the jawline is a subtraction problem. Threads take nothing away, so lifting skin that lies over a fat pad moves the appearance very little. This layer belongs to neck and chin liposuction.
Layer two, the platysma. When vertical platysmal bands stand up on animation, the muscle has separated and needs holding rather than draping. MyEllevate, a permanent suspension suture, paired with platysmaplasty does work a dissolving thread cannot. The contrast is worth understanding on this page: a nonabsorbable suture stays where it was placed and can be taken out, while barbed absorbable threads are built to disappear. Formation is building a dedicated MyEllevate page, so raise the comparison at consultation until it publishes.
Layer three, the skin. This is the thread layer. Skin that springs back when released holds a lift longer than skin that settles slowly, and the honest elasticity window runs roughly skin ages 25 to 55. Energy-based tightening with Renuvion, a helium plasma device, or FaceTite, a radiofrequency device, also works at this layer.
Layer four, the frame. Chin projection and jaw angle get assessed on their own terms. Suspension does not build bone. Structural filler answers this layer with Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, adding projection at the chin point and the angle. Patients who ask for a superhero jawline are almost always describing this layer rather than the skin above it. A page on structural filler for chin and jaw projection is on the way.
Get all four read before you buy any one of them.
Two published numbers should shape your consultation, and almost no clinic page in this market carries either one.
A 2026 multicenter cohort study in Aesthetic Plastic Surgery reviewed 2,000 PDO thread-lift cases performed at 105 clinics between November 2020 and December 2023. The overall complication rate came in at 12.4 percent, led by skin retraction at 6.35 percent, asymmetry at 2.9 percent and pain at 1.75 percent.
The valuable part is what predicted those events. Two variables held up as independent predictors. Thread count carried an odds ratio of 1.12 per additional thread, so risk accumulates gradually with volume instead of jumping at some threshold. Age mattered more. Measured against the study’s youngest patients, the odds of a complication ran 2.14 times higher at ages 40 to 49, 3.38 times higher at 50 to 59, and 5.08 times higher at 60 and above.
Now read that against layer three. The elasticity window starts closing over the same stretch of life where the risk curve climbs. That overlap is the real argument for treating earlier and treating lighter.
The authors built a risk score from those two variables and were candid about what it can do. Sensitivity reached 90.3 percent while specificity reached only 46.9 percent, so the model catches nearly everyone who will have a problem and also flags a large number of people who would have been fine. Treat it as a planning aid. The study is retrospective, which the authors grade as level III evidence.
Walk into any consultation with one question ready: how many threads are you planning, and what is the reasoning behind that number.
A separate 2026 meta-analysis pooling 26 studies and 2,827 patients put numbers on the events clinics file under the word mild. Swelling was reported in up to 34 percent of cases and bruising in 26 percent. Visible or palpable threads came in at about 10 percent and skin dimpling at 7 percent.
Sorting that list by timing is how Formation presents it in consultation, and the split is clean. Swelling and bruising arrive early and resolve on their own, closer to an expected tissue response than to a complication. Visible threads and dimpling persist, may need correcting, and declare themselves after the first month rather than in the first week.
The limitations belong here because they change how you shop. Heterogeneity between studies reached 92 percent for swelling. The pooled studies were conducted between 2002 and 2021 across patients aged 14 to 87, with follow-up running from four weeks to thirty months. Those percentages describe a very wide field of operators and techniques, so they tell you what can happen without telling you what will happen in a specific pair of hands. Placement depth, thread count and candidacy are what move your individual odds, and none of those arrive in the box with the threads.
Dr. Grandhige works from a rule he brought over from general surgery. The procedure accounts for about 60 percent of the result and the care surrounding it accounts for the other 40. Poor aftercare undoes well-executed placement, so recovery here gets managed rather than printed out and handed over.
That arithmetic runs backward as well as forward. Health and diet are assessed before treatment, then nutrition, supplements and peptides are adjusted through the window immediately before and immediately after, so the body lays down as much structure as it can while the material is still present to build around. Protein intake, inflammation control and lymphatic flow get dialed in as part of the plan, and every provider delivering that support was vetted personally instead of assembled from whoever answered the phone. You can read more about how Formation treats care as a system rather than a single appointment.
Any clinic can buy the same threads. The difference sits in the reasoning wrapped around them.
Expect mild swelling, some bruising and a tight pulling sensation. The soreness reads closer to a hard training session in the face than to sharp pain. Sleep on your back with your head raised so nothing presses on treated tissue, and leave facial massage alone entirely. Most patients are back at work and out socially at one week, and hard training waits that same week so the threads settle into position.
Early swelling and bruising have cleared by this point and the window that matters opens. Firmness keeps building as fibrous tissue forms along the thread tracks, and any late-onset issue announces itself during this stretch rather than in week one. Being seen during these weeks is where follow-up actually changes an outcome.
Formation sits at 1315 South Howard Avenue, roughly fifteen miles from Citrus Park whichever way you drive it.
Route one is the Veterans Expressway from Exit 9 at Gunn Highway, south past Hillsborough Avenue toward Memorial Highway, then east on Kennedy to Howard. About half an hour when the Veterans is moving. The Veterans is all-electronic, so there is no cash lane to pull into. You pay by SunPass transponder, or the system photographs your plate and mails a bill to the registered owner. Worth knowing if you plan to borrow a car for the drive home. The southbound express lane entrance sits right at the Gunn Highway interchange with its first exit at State Road 60, and it prices by congestion, which matters if your appointment sends you back north at five.
Route two is Gunn Highway southeast through Carrollwood to Dale Mabry, then straight south. No toll, more traffic lights, and it takes you past the same corridor that generates most of the thread advertising you have been seeing.
That distance is the local argument, and it ties directly to the section above. The events worth catching surface after week four, which is exactly the point where most practices have closed the chart. Fifteen miles turns a week-six or week-ten visit into a morning rather than a production, and Dr. Grandhige follows his patients for a full year rather than stopping at six weeks. Citrus Park also sits between Westchase and Carrollwood, two of the areas Formation patients already drive in from for jawline and neck contouring.
One more scheduling reality specific to this area. The Upper Tampa Bay Trail runs alongside the Veterans just south of the Gunn Highway interchange, and plenty of local patients ride or run it several mornings a week. Hard training waits one week after threads, and Florida sun on fresh bruising creates its own calendar problem. Plan backward from the ride you care about, not forward from the appointment.
The first step skips the drive entirely. Formation offers a complimentary virtual evaluation with patient coordinator Hanna, who reviews your goals and outlines a plan before you commit a morning to the road. The in-office consultation with Dr. Grandhige carries a fee because it is a full surgical assessment rather than a sales meeting, and that fee can be applied toward any procedure or product at the practice.
Threads reposition tissue. They do not add volume and they do not remove it.
That is the Formation Add-or-Subtract Rule, and it resolves most consultations quickly. If what bothers you in photographs is volume, whether that is too much under the chin or too little at the chin point, repositioning will not reach it. Fat comes out and frame goes in, while position is the only thing a lift can change.
Heavy skin excess in the lower face is the clearest disqualifier. Pull on excess skin and you create tension where you wanted shape, and tension is the quality people recognize as work. Patients past what suspension can reach get referred outward, because Formation stays in contact with the surgeons who do skin-excision work well rather than talking someone into whatever happens to be on the menu that week.
Where the skin itself is thin on recoil, energy-based tightening with FaceTite reaches a depth that threads do not.
One Formation patient, a fit professional in their forties who had disliked their own profile in photographs for years, had a jawline that never responded to weight loss because laxity was never the problem. Contouring answered it. Colleagues afterward said they looked more rested, without ever working out why.
Declining to treat is uncommon in aesthetics. A well-placed thread in the wrong face is still the wrong procedure.
Dr. Gopal Grandhige, MD is a board-certified general surgeon specializing in aesthetic surgery, with over 15 years of operating experience and several thousand surgical procedures behind him.
He completed medical school at the University of Michigan and matched into a six-year combined plastic surgery residency at Yale before redirecting toward a broader surgical education. He finished a full general surgery residency followed by fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery, and he holds certification from the American Board of Surgery. Years spent in settings where wound healing and tissue behavior carried immediate consequences change how a face gets read for a thread pattern.
Formation stays boutique deliberately. Surgical volume is limited on purpose so Dr. Grandhige evaluates every patient himself and designs every plan himself. Some patients get told to wait, and some get told the answer is a different layer or a different practice entirely.
Male faces need a different thread pattern, and male aesthetics is one of the areas Dr. Grandhige is specifically known for. Male skin is thicker, fat distributes differently, and a masculine jaw should read angular rather than lifted and softened. A vector designed around a female midface will feminize a male one, which is the single outcome most men walk in hoping to avoid.
The governing rule is the same one he applies in male body contouring, where small over-corrections read as artificial from across a room: do what is necessary, then stop. PDO thread lifts for men at Formation are planned to that standard.
There’s a story behind every decision.
Cosmetic surgery is a personal decision. Yet people who choose it often find themselves having to explain or defend that choice to others.
Some want to restore changes after pregnancy or weight loss. Others have worked hard to get in shape but still have areas they’d like to improve. And some simply want to feel more comfortable with their appearance.
The reasons are different for everyone. And they don’t have to make sense to anyone else.
At Formation, Dr. Gopal believes the decision to pursue cosmetic surgery should be guided by a patient’s personal goals, realistic expectations, and what’s medically appropriate for them.
Because ultimately, the person making that decision is the one who will live with it.

Our first four-legged patient. 🐾
Dr. Gopal takes his liposuction and body contouring very seriously.
Even when his patients have four legs.
From loose skin and neck liposuction to a jawline that deserves its own before-and-after reveal, this might be his most dramatic transformation yet.
And yes, he even got the patient’s permission to share the results. 😂
A little Halloween fun from Dr. Gopal and his two very patient pups! 🎃🐶
No dogs underwent surgery. Just two willing participants and one very committed surgeon.

What weight loss can’t fix.
After losing over 100 pounds, this patient had achieved significant weight loss, but was left with excess skin around his abdomen and back that weight loss alone couldn’t address.
During an 8-hour surgery, Dr. Gopal performed a circumferential body lift, a procedure that removes excess skin around the entire midsection to improve body contour.
In this video, Dr. Gopal walks through the procedure, explaining how he removed approximately 3 pounds of skin from the back and estimated another 4–5 pounds would be removed from the abdomen.
He’s also using a small amount of fat collected during the procedure to add volume to the upper chest, improving its contour without performing a separate chest lift.
For patients who have experienced major weight loss, body contouring can help address the excess skin that remains after the weight is gone.

Only 3 months after surgery. And still healing.
This patient had previously undergone a tummy tuck but wanted to improve his abdominal contour, create more definition, and address the appearance of his existing scar.
Dr. Gopal combined several body contouring techniques, including HD liposuction, skin tightening with Renuvion, fat transfer to the chest, and revision of the previous tummy tuck scar.
The result so far? A more defined abdomen, improved body contours, and a revised scar that’s still in the early stages of healing.
At just three months after surgery, these aren’t the final results. Swelling can continue to decrease, and scars typically soften and fade as healing progresses.

Your fat is liquid gold.
The fat you don’t want might be exactly what another part of your body needs.
During liposuction, the fat we remove doesn’t always have to be discarded. In fact, Dr. Gopal refers to it as “liquid gold.”
Why? Because that fat can sometimes be transferred to another area of the body to add volume, enhance natural contours, and create a more balanced shape.
For women, that might mean adding volume to the breasts, hips, or buttocks. For men, fat transfer can help enhance the chest or shoulders.
And because we’re using your own fat, we can potentially improve two areas during the same surgery.
Of course, fat transfer isn’t right for everyone. It’s something Dr. Gopal discusses with patients beforehand and incorporates into the surgical plan when appropriate.

Cosmetic surgery isn’t just cosmetic.
We often measure the success of plastic surgery by what we can see in before-and-after photos. But some of the most meaningful changes happen in ways a photograph can’t capture.
How someone feels about themselves. The confidence they carry into everyday life. The freedom of no longer feeling uncomfortable in their own skin.
For Dr. Gopal, that’s what makes this work so meaningful.
A patient who underwent body contouring in 2025 recently reached out to share how much the experience continues to mean to her.
She didn’t talk about measurements or how her body looked. She talked about how much better her days had become.
And that’s something Dr. Gopal has always believed: Cosmetic surgery isn’t simply about changing how someone looks. It’s about understanding what that change means to the person choosing it.

A flatter stomach without a tummy tuck? 👀
Pregnancy, and sometimes increased pressure from fat deep inside the abdomen, can cause the abdominal muscles to separate down the middle. This is called diastasis recti, and it can make your belly stick out even if you’re otherwise lean.
Traditionally, repairing that separation often meant undergoing a tummy tuck. But what if you don’t have much loose skin to remove?
For the right patient, Dr. Gopal can use robotic-assisted surgery to bring those separated muscles back together through three small incisions, without the long scar associated with a traditional tummy tuck.
That’s exactly what he did for the patient in these before-and-after photos. No tummy tuck. No additional procedures.
**But this approach isn’t for everyone.** If you have significant loose skin or excess fat, a different treatment may be more appropriate. That’s why understanding what’s causing your belly to protrude matters before deciding on surgery.

Why does fluid sometimes collect after surgery?
After certain surgical procedures, fluid can accumulate beneath the skin, forming what’s called a seroma.
In this video, Dr. Gopal demonstrates how he drains a seroma in a patient recovering from diastasis recti repair.
This is one of the reasons postoperative follow-up matters. It allows Dr. Gopal to monitor healing, identify any issues, and address them when necessary.

The fat is gone. But what takes its place?
When fat is removed during liposuction, it leaves behind space beneath the skin. As the body heals, fluid can sometimes collect in that space, creating what’s called a seroma.
And here’s why that matters.
If fluid continues to collect, it can potentially affect the final contour.
That’s why Dr. Gopal pays close attention to fluid buildup during recovery. His approach is to identify seromas early and address them quickly.

The mechanical lift shows the day it is placed. The polydioxanone breaks down over roughly six to nine months, and the fibrous tissue along the thread tracks keeps supporting the area after that. Follow-up across the pooled literature runs from four weeks to thirty months, so any precise figure quoted beyond that range is going past what has been published.
Yes, in two directions at once. The elasticity window for holding a lift runs roughly skin ages 25 to 55, and the 2026 cohort data shows complication odds climbing with each decade past 40. Both point the same way, toward treating earlier and treating lighter.
No. They are 510(k)-cleared Class II devices under the absorbable polydioxanone suture classification, and several cleared barbed systems carry indication language limiting them to soft tissue approximation for dermatological use. Ask for the 510(k) number and indication language of the specific threads being used on you.
Fewer than most offices quote. Complication odds rise about 12 percent with each additional thread, so thread count is a clinical decision rather than a package size. Dr. Grandhige sets the number from your anatomy and explains the reasoning.
Placed conservatively, no. Patients typically hear that they look rested rather than lifted. Over-tensioned patterns are what make a thread lift recognizable, which is the argument for restraint.
Usually, but disclose it. Fibrosis along old thread tracks changes how those tissue planes behave during surgery. One conservative round is a minor variable, while several stacked rounds are a larger one, and a surgeon who has operated in scarred planes will want to know beforehand.
Most patients return to work and social plans at one week, and hard training waits that same week so the threads settle. If the Upper Tampa Bay Trail is part of your routine, schedule around that week rather than through it.
