Skin is the other half of the equation. Firm, elastic skin drapes cleanly over the new contour once fat is gone. Skin that has thinned from age, sun, or fast weight loss can settle into slack instead. When that happens, Dr. Grandhige adds a skin-tightening step during the same visit, or he redirects you toward neck and jawline contouring approaches that address laxity directly.
Sometimes the answer is that neither one helps yet. He will say so. Roughly one in six people who come in asking specifically about neck liposuction leave with a different plan, and a smaller share leave with no procedure recommended at all.
The GLP-1 Question Most Pages Skip
If you have used semaglutide, tirzepatide, or a similar weight-loss medication, tell us at the consultation. It changes what your skin can do and it changes how the procedure is planned. It probably does not mean you have to stop the medication.
This has become the most common thing patients get wrong. The American Society of Plastic Surgeons recorded 24,000 submental and chin liposuction procedures among its member surgeons in 2024, a 1% increase over the prior year, while liposuction held its place as the most performed cosmetic surgical procedure. In its earlier reporting, ASPS noted that the jump in facial procedures and skin tightening might be connected to the effects of semaglutide drugs, while saying it was too early to confirm a direct link. The Three-Layer Neck Check

Layer One, Fat
Tilt your chin down slightly and pinch the tissue under your jaw. If you can gather a soft, movable pad between your fingers, that is subcutaneous fat, and it is what liposuction removes. This is the layer most people are actually bothered by.
Layer Two, Skin
Pinch a small section of neck skin, hold it for two seconds, then let go. Skin that springs back immediately handles fat removal well on its own. Skin that drifts back slowly or leaves a crease usually needs a tightening step in the same appointment. Reasonable elasticity generally holds through a skin age of about 25 to 55.
Layer Three, Structure
Look at your profile in a mirror and clench your jaw. If fullness stays put once the muscle tightens, what you are seeing may be muscle position, a recessed chin, or the angle of your jawbone. Liposuction does not change any of those. This is the layer that generic candidacy checklists ignore, and it is the most common reason someone is not a fit.

What Happens On Procedure Day
Plan on being at the office 70 to 100 minutes door to door. You are awake the entire time.
You take an oral sedative on arrival, then the area under your chin and along your jaw is numbed with local anesthesia. Dr. Grandhige makes small, discreet incisions under the chin and behind the ears. He removes fat to sharpen the jawline while preserving your natural angles, and he deliberately stops short of taking everything available. Over-resection is what creates hollow, aged, obviously operated necks, and it cannot be undone.
His framing is that this work is sculpture rather than fat removal. Every pass is planned around your muscle position, your jawbone, and how light falls across your lower face. When your skin needs help conforming to the new contour, the tightening step is added during the same visit rather than scheduled separately.
You leave in a chin compression garment and drive yourself home if the sedative has worn off, or have someone drive you. Your first follow-up happens within a few days. If your goals extend past the jawline, the same approach carries into submental liposuction combined with wider contouring work.
Larger surgical cases at Formation are performed in fully accredited Quad A certified surgical facilities. The neck procedure does not require that setting, which is why it stays in the office.
