What HD Liposuction Actually Removes
HD liposuction takes different amounts of fat from different depths and different spots, leaving it where a natural shadow belongs and clearing it where a muscle border should read. Standard liposuction takes volume out of a region. That is the whole difference, and it decides who the procedure helps.
It is a finishing tool for someone already close to their goal weight. It is not a route to getting there.
Dr. Grandhige puts it plainly: the last layer of fat over an abdomen or a flank is often genetically stubborn, and the amount of dieting it takes to strip it usually costs muscle. People lose the muscle, rebound, and start over. Removing that layer surgically ends the cycle and lets you train to build instead of dieting to reveal. The areas treated most often are the abdomen, flanks, back and chest, alone or in combination. The Formation Tighten-Or-Remove Line
Three checks decide whether the honest answer is contouring, tightening, or removing skin. Dr. Grandhige runs them in this order at every body consultation, and they are the reason some Valrico patients leave with a surgical date and others leave with a different plan.
Check One, Recoil
Pinch the skin over the area, lift it, let go. Skin that snaps back toward the body has enough elastic recoil to follow a new contour. Skin that drifts back slowly is telling you the surface will not cooperate with fat removal alone.

Stand upright and relaxed. No bending, no leaning forward. Look for skin that hangs at rest. A fold that is present standing still is a removal problem, and removal means abdominoplasty, which Dr. Grandhige performs himself rather than sending you elsewhere for.

Liposuction does not touch the abdominal wall. Tighten your abs and feel along the midline for a gap. If the bulge is muscle separation, taking fat off the top makes the separation easier to see, not harder. That case needs wall repair.
Clear all three and HD liposuction alone can carry the result. Fail check two or three and fat removal becomes the smaller half of the plan. Skin tightening enters the conversation as an add-on for borderline recoil, not as the main event.
How The Sculpt Is Planned
Dr. Grandhige treats liposuction as sculpture rather than extraction. Each contour is designed around muscle shape, skeletal structure, fat quality, and how light actually falls across a body in motion.
Male contouring gets a different eye than female contouring, because the errors are different. Over-resect a male chest and you get a saucer deformity. Etch too aggressively across an abdomen and the lines announce themselves in every photo and read as artificial in person. He is known for flat, clean masculine chest contours and for stopping short of the look social media rewards. Definition that only works in a flexed still image is a failure with a good photograph attached.
That restraint shapes case selection too. He evaluates every patient personally and designs the plan himself, and he caps surgical volume so he can stay closely involved before, during and after. What The Published Data Shows, And What It Does Not
A 2023 systematic review in Plastic and Reconstructive Surgery pooled 21 studies covering 6,964 high definition liposuction patients and calculated an overall complication rate of 14.4 percent, with 0.2 percent of complications classed as major. The most frequent were transient hyperpigmentation at 3.8 percent, seroma at 3.3 percent, and temporary soft-tissue fibrosis at 2.7 percent. Reported satisfaction came in at 92.6 percent, and the pooled patient averaged 38 years old with a body mass index around 25. Florida Board of Medicine
Read the limitations alongside the numbers, because they matter. The authors note there was no uniform reporting standard for satisfaction across the included studies, so that 92.6 percent is softer than it looks. The systematic review also pools cases globally without separating results by surgeon, technique variant or region, so it says nothing about any one practice. One number does change how Formation runs recovery. Where The Surgery Happens, And The Florida Rule Behind It
HD liposuction at Formation is performed under general anesthesia in a Quad A-accredited surgical facility, not in a treatment room. Dr. Grandhige works with the same anesthesia provider and the same operating room staff on every case, and his own physician assistant assists on all of his surgeries.
Florida sharpened this question in May 2024. Under CS/HB 1561, an office where a physician removes more than 1,000 cubic centimeters of supernatant fat, temporarily or permanently, must register with the Department of Health, and that procedure cannot be performed anywhere other than a registered office surgery setting or a facility licensed under chapters 390 or 395. The Department inspects an office before registering it, then inspects registrants annually unless the office holds national accreditation. That threshold is low enough for a multi-area case to reach it. So ask any practice you are considering where your operation will physically happen and what that setting’s office surgery registration or accreditation status is. The answer should come back immediately and without qualifiers.
The Preparation Window Most Practices Skip
Your health and diet get assessed before a surgical date is set, not after. In the four weeks before surgery and the six weeks after, nutrition, supplements and peptides are adjusted so your body builds as much structure as it can while it is healing. Protein intake, inflammation control and lymphatic flow are all dialed in against that window rather than left to a printed handout.
The cannula is the same instrument in every operating room in Hillsborough County. What differs is the thinking around it, and most of that thinking happens before the first incision and after the last stitch.
Recovery And Return To Training
Dr. Grandhige’s position on this is blunt. The operation is 60 percent of the result and post-operative care is the other 40. Incorrect after-care can undo a well-executed surgery, which is why Formation keeps that phase in-house instead of handing it off.
- First 48 to 72 hours: swelling, soreness and drainage are expected. You are walking early, on purpose.
- Week one: compression stays on continuously. Light daily activity is fine, work is not. Lymphatic treatment begins at your first post-operative visit, and a seroma gets drained if one forms.
- Weeks two to four: swelling improves steadily and compression continues. Most patients are back in the gym within one to two weeks and unrestricted between weeks two and four as long as the garment stays on.
- Six to 12 weeks: definition starts emerging as inflammation subsides.
- Three to six months: final contour matures.
- Six to 12 months: skin pliability and sensation return to normal.
Follow-up runs a full year, with visits at day three, week one, week three, week six, and months three, six and 12. Nearly all patients complete the whole year.
Timing is worth thinking about from Valrico specifically. Valrico sits inland, away from the bay breeze that South Tampa gets, and wearing a compression garment continuously through a Hillsborough County July is a real thing to plan around. When HD Liposuction Is The Wrong Choice
Some of the people who search for this procedure should not have it, and saying so is faster than a consultation that ends in disappointment.
A standing fold of loose skin is a removal case. Fat removal will make it hang more, not less. Muscle separation is a wall repair case for the same reason. If your weight has moved more than a few pounds in either direction over the past year, or a pregnancy is planned, the honest answer is to wait, because HD liposuction sets a contour against the body you have on surgery day.
Chest tissue that is firm and glandular rather than soft and fatty will not respond to liposuction the way patients expect. That is gynecomastia surgery, a different operation with a different plan.
And if what you want is the heavily etched look that circulates on social media, Dr. Grandhige will tell you no. Over-etched contours age badly, they read as surgical from across a room, and they cannot be undone.
