Threads lift tissue that has dropped. They cannot remove fat, repair muscle, or build a jaw you were not born with. Knowing which of those four things is actually softening your jawline is the entire decision, and it takes about ten minutes to find out.

This layer is answered with structural support, using Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed at the chin and along the jaw angle. Read all four and the plan usually writes itself. Threads alone are right for a narrower group of people than the search results suggest, and Formation would rather tell you that at the consultation than after.
Dr. Grandhige’s stated position is that sometimes a procedure is not offered at all. He limits volume specifically so he can personally evaluate every patient and design the plan himself, and he is willing to say no when the answer is no. In his words, the role is not to sell a procedure. It is to guide a patient to the decision that gets them where they want to be, even when that means waiting or changing course.
For threads, the common no’s look like this. A visible fat pad under the chin. Skin that does not recoil when you release it. Real platysmal bands. A chin that sits back. Someone who wants a facelift result from a fifteen-minute in-office procedure.
One Formation patient, a professional in his forties, came in convinced his neck made him look heavier and older than he felt. He was otherwise fit, and the area had never responded to weight loss. He had assumed he had always simply looked that way. The read showed fat, not descended tissue. He had targeted neck liposuction with jawline contouring instead. Colleagues told him he looked rested. He started getting in photographs again.
That is the difference between a read and a menu.
The phrase you will see everywhere is minimal downtime. It is technically accurate and practically misleading, because downtime and restriction are not the same thing.
You will look presentable quickly. Expect mild swelling, some bruising at the entry points, and a tight or tugging sensation for several days. Most people go back to work within a couple of days.
The restrictions run longer, and this is the part the ranking pages compress into one sentence. Thread aftercare instructions, at Formation and everywhere else, ask you to limit wide mouth opening for two to four weeks. That means soft foods, no big yawns, no dental cleanings, and yes, being careful when you laugh hard. Barbed threads hold by anchoring in tissue, and aggressive stretching of the treated area can pull one loose. You sleep on your back for the first week or two. You skip facial massage, rollers, and any energy-based facial treatment for weeks.

None of that is a problem if you plan for it. It is a problem if you booked threads on a Thursday because you have a wedding Saturday and nobody mentioned the soft-food window. Ask for the written restriction list before you schedule, not after.
The thread is a commodity. Any licensed injector in Pasco or Hillsborough County can buy the same box. What differs is what happens in the two weeks on either side of it.

First, the device status. PDO threads are 510(k)-cleared Class II medical devices, and specific thread brands carry clearances for soft tissue approximation and, in some cases, facial suspension. They are not FDA-approved in the sense a new drug is approved. Ask who is placing the threads. Ask what happens if one migrates or becomes palpable. If the answer is vague, keep looking.
There are two ways in.
The in-office consultation with Dr. Grandhige carries a fee. It is a full surgical assessment, not a sales appointment: anatomical evaluation, candidacy across all four layers, honest discussion of what will and will not help, and guidance on timing. That fee can be applied toward any procedure or product at the practice. If threads are wrong for you, you will hear it in that room.

The commonly reported range is twelve to eighteen months, though the material itself absorbs sooner than the visible effect fades. Skin quality, sun history and how much tissue needed to move all shift where you land in that range. Threads are a maintenance procedure, not a one-time fix, and it is fair to ask what the plan is when they go.
The area is numbed with local anesthetic first. Most people describe pressure and a pulling sensation rather than pain during placement. Soreness and tightness for the first few days afterward are normal and manageable without anything strong.

If a thread becomes palpable or visible, it can be addressed, which is another reason to have this done by someone who can manage the complication rather than refer it away.
