The Four-Layer Neck Read

Layer Two, The Platysma And Platysmal Bands
The platysma is the thin sheet of muscle across the front of the neck. Over time its edges separate and stand up as two vertical cords. Those are platysmal bands, and liposuction does nothing to them.
Where bands are present, the answer is MyEllevate, a permanent suspension suture, paired with platysmaplasty to bring the muscle edges back toward each other. The suture is nonabsorbable and can be taken out later if it ever needs to be, which isn’t true of barbed threads that dissolve on their own. The lift is there the day it’s placed rather than arriving over months.
Layer Three, Skin And The Fibroseptal Network
Skin quality is its own layer, and the tool for it is energy: Renuvion, a helium plasma and radiofrequency device, or FaceTite, a radiofrequency device. Both are 510(k)-cleared, which is a clearance pathway, not an approval.
What separates them from surface treatments is depth of access. Renuvion works at the deepest layer of the skin and the fibroseptal network beneath it, the connective tissue foundation that anchors skin to muscle. Lasers and microneedling work from the top down and can’t reach that foundation at all. Collagen change follows afterward, but the reason to choose one of these is where the energy actually lands.
Layer Four, Chin Projection And Jaw Angle
Bone is the frame everything else hangs on. A recessed chin reads as a heavy neck even when the fat underneath is normal, and no amount of liposuction fixes that.
Adding projection at the chin point changes the angle between jaw and neck without touching the neck. This is structural filler territory: Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed on bone rather than in soft tissue. For men who want the frame that gets called a superhero jawline, this is the layer doing that work, and jawline fillers are the tool, not a cannula.
When Neck Liposuction Makes A Neck Look Worse
Fat is a covering. Take it off and whatever sits underneath becomes more visible, not less.
The common version: a patient has mild platysmal bands hidden under a comfortable fat pad. Isolated liposuction removes the pad. The bands nobody could see before are now the most noticeable thing about the neck. That patient paid for a procedure and left with a new complaint.
The second version is a contour dip. Take too much directly under the chin without respecting the transition into the jaw and you leave a hollow that catches shadow in every overhead light.
Both are decided at the read, not in the procedure room. It’s why the four-layer check happens before anything gets scheduled, and why Dr. Grandhige tells some patients that liposuction alone is the wrong answer for their neck. Some are told to wait. Some get pointed at a different layer entirely. Patients whose skin and muscle have gone past what tightening and suspension can reach need excisional surgery, and Formation stays in contact with the surgeons who do that work well and sends those patients to the right one.
What Happens On Procedure Day
This is an office procedure, not an operating room case. You take an oral sedative for anxiety, the area is numbed with local anesthesia, and you stay awake through it. No general anesthesia, no breathing tube, no recovery bay.
Incisions are small and placed where they disappear: under the chin, behind each ear. Fat is removed, the contour is assessed, and then Renuvion or FaceTite tightens the skin down over the muscle and bone so the new shape has something to hold to.
You leave in a chin compression garment and you go home. For a Brandon patient that’s one ride back across the Selmon, not an overnight stay and not a second person taking a day off work to sit in a waiting room.
Recovery Week By Week
First few days. Mild swelling, some bruising, a tight feeling. The chin garment stays on.
One week. Back to work and back to social plans. Garment worn 12 to 24 hours a day. Back to training at this point.
Two to four weeks. Swelling keeps settling and the fat removal becomes clearly visible. This is when most people start getting comments.
Three months. Skin tightening shows up and the jawline change is obvious.
Six to nine months. Final result, once tightening has finished.
Dr. Grandhige puts the split at 60/40. The procedure is sixty percent of the result and post-operative care is the other forty. Bad aftercare can undo a well-executed procedure, which is the reasoning behind our philosophy of owning that phase rather than handing you a printed sheet and a follow-up date.
How Formation Prepares You Before And After
Health and diet get assessed before treatment, not after. Nutrition, supplements and peptides are adjusted in the window immediately before and immediately after the procedure so the body builds as much structure as it can while it’s healing. Protein intake, inflammation control and lymphatic flow all come with specific instructions instead of general advice.

Who Is A Candidate
Good candidates are bothered by a double chin or a soft jawline, sit at or near their goal weight, and have reasonable skin elasticity, roughly skin age 25 to 55. They want facial refinement without a facelift, and they want a sharp jawline rather than a pulled one.
Skin age isn’t the same as birthday age. A 52 year old who stayed out of the Florida sun can have better recoil than a 40 year old who didn’t, which is one reason candidacy gets decided by hand and not by a form.
Poor fits are easy to name. Anyone expecting this to substitute for weight loss. Anyone with skin excess that’s already past what tightening reaches. Anyone who wants a result that announces itself, since Dr. Gopal Grandhige builds toward the opposite.
For the right neck, this produces a large visible change from a small intervention. That ratio is unusual in aesthetic surgery and it’s the reason this procedure is worth doing well.
Driving In From Brandon
Brandon has no shortage of places to treat a double chin without surgery. Nonsurgical fat reduction, injectables and surface skin devices are all available inside the 33510 and 33511 corridor. What isn’t in Brandon is the surgical option. For actual fat removal you’re driving into Tampa.

The drive matters more for follow-up than for the procedure itself.
