What This Procedure Fixes, And What It Doesn't
The neck has two separate problems and they need two separate answers. Fat under the chin comes out with liposuction. Loose skin over that fat needs energy-based tightening so it contracts down onto the new shape instead of hanging off it. Do the first without the second on the wrong patient and you trade a fuller neck for a looser one.
Some things this procedure cannot touch. A recessed chin changes the geometry of the whole area, and no amount of fat removal builds bone. A low-sitting hyoid, the small horseshoe bone at the top of your throat, sets a physical ceiling on how sharp the angle under your chin can get. Heavy platysmal banding, those two vertical cords that show when you clench your jaw, can look worse after fat comes off because there’s less padding hiding them. A prominent submandibular gland becomes more visible for the same reason.
Dr. Grandhige will tell you which of these applies to you before you book anything. If the honest answer is that neck liposuction alone gets you eighty percent of the way there, that’s the plan. The Four-Signal Fit Check
Formation screens every neck and jawline candidate against four physical signals. All four get checked at the consultation, in this order.
Signal One, The Pinch
Dr. Grandhige pinches the tissue under your chin. Roughly a centimeter and a half to two centimeters of fat between the fingers is the sweet spot. Much less and there isn’t enough to remove for the result to justify the recovery. Much more usually means the fat sits below the muscle, not above it, and surface liposuction won’t reach it.
Signal Two, The Recoil
He pulls the skin away from your neck and lets go. How fast it snaps back tells him whether the envelope will contract over the new contour or drape over it. Skin quality and elasticity are treated in the surgical literature as a prerequisite for this procedure, not a bonus, and the published work on managing the aging neck is direct about it. In practice at Formation this lands somewhere between a skin age of 25 and 55.
Signal Three, The Hyoid
He feels for where your hyoid bone sits. High and forward means a sharp angle is achievable. Low and back means you’ll get real improvement but not a knife-edge, and you need to hear that before surgery instead of at your three-month follow-up.
Signal Four, The Chin
Chin projection sets the reference line for the whole jaw. A weak chin can make a well-executed neck result look underwhelming. Sometimes the fix is jawline filler to restore the projection first, and sometimes a thread lift handles a mild laxity problem that doesn’t warrant surgery at all.
Here’s what the check actually screens out. Over the last twelve months Formation logged 84 neck and jawline consultations. 49 were cleared for in-office contouring. 21 were asked to wait, usually for weight to stabilize or for a skin question a few months would answer. 14 were sent somewhere other than the operating chair. That’s roughly one in six people who book a neck and jawline consultation here and leave without a surgical plan.
What Happens On Procedure Day
You take an oral sedative, Dr. Grandhige numbs the area with local anesthetic, and the work happens through very small incisions under the chin and behind the ears. Fat comes out selectively rather than aggressively, because over-resection under the chin produces contour irregularities and a hollowed look that ages badly. Then radiofrequency-assisted tightening runs under the skin so it contracts over the muscle and bone beneath it.
You leave with a chin compression garment on. You drive home, or somebody drives you the mile and a half back east. Nobody’s admitted anywhere.
Recovery Week By Week, And Where To Walk It
First few days. Mild swelling, bruising and a tight feeling under the chin. The garment stays on.
Week one. Most patients are back at work and back out socially. The chin garment goes 12 to 24 hours a day this week, and that number is the single biggest variable you control.
Weeks two to four. Swelling drops off steadily. The liposuction result becomes visible, though the skin hasn’t finished moving yet.
Three months. Skin tightening becomes obvious. This is where most people first see the jawline they came in for.
Six to nine months. Final result. Skin contraction has completed.
Early walking helps swelling clear, and Hyde Park Spanishtown Creek has the best recovery route in the city sitting on its eastern edge. Who Does The Procedure
Gopal Grandhige, MD is a board-certified general surgeon who specializes in aesthetic surgery. He went to medical school at the University of Michigan and matched into the six-year combined plastic surgery residency at Yale before shifting to a full general surgery residency, then fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. Fifteen-plus years operating, several thousand cases.
That path matters here specifically. Burn and critical care training is training in how skin heals, how inflammation behaves, and what tissue does under stress. That’s the entire mechanism of a neck contouring result. Board-certified plastic surgeon Dr. Ahmad Saad is part of the Formation team as well.
The same anesthesia provider works every one of his cases and the same staff run every room, which is how the team anticipates instead of reacting. Cases requiring general anesthesia are done in Quad A-accredited surgical facilities. Office-based procedures like this one are covered by Florida HB 1175, signed as Chapter 2026-89 and in effect since July 1, which directs the Florida Building Commission and State Fire Marshal to set safety design standards for office surgery suites by January 1, 2027. When Formation Says No
Four situations get turned down here regularly.
Your weight isn’t stable yet. Fat cells removed don’t come back, but the ones left behind still respond to weight gain, and operating mid-swing means operating on a moving target.
Your skin won’t cooperate. If the recoil test says the envelope won’t contract, contouring produces a looser neck, not a sharper one.
You want a facelift result from a neck procedure. This addresses the neck and the jaw edge. It does not lift the mid-face or erase jowling.
Your event is in six weeks. The liposuction result shows by week four, but skin tightening lands at three months and finishes between six and nine. If you’re getting married in October, this is a conversation for next year.
