Skin removal alone can leave a flat abdomen sitting between two untouched flanks, which reads as boxy. Adding contouring is often what makes the result look like a body rather than a repair.
Extended Abdominoplasty
The incision continues around the hips to address skin that wraps toward the back. This is the version most often needed after major weight loss or bariatric surgery.
Who Is a Candidate, and Who Gets Told No
The best candidates are at or near a stable goal weight, in good general health, and finished having children. Loose skin, a lower abdominal pouch that will not respond to training, or a visible midline bulge are the physical signals.
Chronic conditions do not disqualify you. Uncontrolled ones do. Diabetes, hypertension, and thyroid disorders are all workable when they are managed and your primary physician signs off. Poorly controlled blood sugar, active nicotine use, and an unstable weight are the three things that reliably turn a good surgical plan into a wound healing problem.
Dr. Grandhige declines cases regularly, and this is worth saying plainly because most practice websites will not say it. He will tell you no if your weight has moved more than a few pounds in either direction over the past six months. He will tell you no if you are planning a pregnancy. He will tell you to wait if your goal is a specific date that does not leave room for a real recovery. And if the abdomen bothering you is firm rather than loose, he will tell you that surgery cannot reach it and send you back to your physician instead.
Men considering the procedure are working toward a different result. The goal is a flat, squared midsection with the flanks kept in proportion, not the narrowed waist that defines a female contour. Quad A accreditation covers infection control, emergency protocols, staff credentialing, and continuous safety oversight for outpatient operating rooms, and it is the standard worth confirming before any elective surgery anywhere.
Dr. Grandhige makes a horizontal incision from hip to hip, placed just above the pubic hairline so it sits under a waistband and under swimwear. Excess skin and fat are removed from the lower and mid abdomen. The rectus muscles are sutured back to the midline. A second small incision around the navel repositions the belly button so it sits naturally on the tightened abdominal wall. Where the plan calls for it, liposuction is performed in the same operation to shape the flanks and smooth the transitions.
Surgery takes four to seven hours depending on how much correction is involved. Patients are discharged within two to three hours afterward.
The operating room team is the same team on every case. Same anesthesia provider, same nurses, same physician assistant. Dr. Grandhige built it that way because a team that knows a surgeon’s methods anticipates rather than reacts, and that shortens time under anesthesia.
You will not sleep alone the first night. A nurse stays with you for the full 24 hours after surgery, at home or at the recovery hotel if you are coming in from out of the area.
The 60/40 Rule: Why Recovery Decides Your Result
Dr. Grandhige puts a number on it: the operation is 60 percent of your result and post-operative care is the other 40 percent. Bad post-op care can ruin a well-executed surgery. That is why Formation owns the recovery rather than handing you a printout and a follow-up date.

Compression garments go on in the operating room and stay on continuously through week four, then during the day through week six. Lymphatic treatments begin at your first post-operative visit and continue through the early weeks. Their job is to move fluid out before it pools. When a seroma does form, it gets drained in the office rather than left to reabsorb on its own, which is how a small fluid collection turns into a contour irregularity six months later. Whether drains are placed at all depends on the technique used, and drainless abdominoplasty is one of the things to settle in your consultation rather than on surgery day.
Walking starts the first day. Short, frequent, hunched forward at the waist. It is not optional and it does more for your recovery than any medication.
Protein intake matters more than most patients expect. Muscle repair is a healing wound under tension, and underfeeding it slows everything down.
Timeline in plain terms. Rest and gentle movement through week one. Most patients with desk jobs return to work at 10 to 14 days. Physically demanding work waits for six weeks. Exercise without restriction resumes at six to eight weeks. Swelling improves steadily through months three to six, and skin pliability and normal sensation return between six and twelve months.

Recovering at Home in Palmetto Beach
Three things about this neighborhood change how you should plan your recovery, and no general Tampa page is going to mention any of them.
- Your house is probably older than your surgeon. Much of Palmetto Beach’s original housing stock is early 1900s bungalows and shotgun homes, and many of them are raised on piers with front porch steps. For the first week after a tummy tuck you cannot stand fully upright, and stairs are genuinely difficult. Before surgery, walk your own entry. If your bedroom is up a flight, set up a sleeping spot on the main floor. If your bed is high, you need a plan for getting in and out of it that does not involve engaging your core. Most people sleep bent at the hips for the first ten days or so, which usually means a recliner or a wedge pillow setup. Small bungalow bedrooms do not always fit a recliner, so measure before you buy one.
- The neighborhood is flat, and that is an advantage. You need to walk from day one, and Palmetto Beach gives you level sidewalks and short blocks. These are the questions that separate practices, and they apply whether or not you end up at Formation.
Ask for the quote in writing, itemized. A number given over the phone is not a quote. The line items that should appear separately are the surgeon’s fee, anesthesia, the facility fee, your compression garments, your post-operative visits, your lymphatic treatments, and the policy on revisions. If any of those are missing from the page, they are either bundled or they are not included, and you need to know which. A figure that looks unusually low is usually a surgeon’s fee with anesthesia and facility charges still to come.
Verify board certification yourself. Dr. Grandhige is board certified in general surgery by the American Board of Surgery, with fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. He is not certified in plastic surgery, and Formation would rather you hear that here than find it later. What that background brings to an abdominal operation is a decade and a half of work inside the abdominal wall itself: tissue handling, wound healing under tension, inflammation, and complication management in settings where the margin was thin. Whoever you consult with, look up their certification directly rather than taking a website’s word for it.
Ask who is in the room and who sees you afterward. Ask whether the surgeon uses the same anesthesia team on every case. Ask how many post-operative visits are included and who conducts them. Ask what happens at month six if something is not settling correctly.
Ask what they will not do. A surgeon who has never declined a case is a surgeon who is selling.
