What Plastic Surgery At Formation Covers
Formation performs surgical contouring rather than a full cosmetic menu. The list is deliberately short.

Most patients receive a combination rather than a single procedure. The plan accounts for skeletal structure, fat quality and distribution, skin elasticity, training habits and how the result will age.
Dr. Grandhige does not treat liposuction as fat removal. He treats it as sculpture, designed around muscle shape and how light falls across the body. That distinction matters most in male contouring, where a small error reads as artificial rather than athletic.
Who He Operates On, And Who He Sends Elsewhere
Not every consultation ends with a surgical date, and that is the point.
Roughly speaking, the patients who do well are already close to their goal weight, active, and frustrated that visible definition never matched the work they put in. Liposuction is a finishing tool. It is not a weight-loss operation, and a patient who arrives expecting it to function as one gets told so directly.
Some patients need more than contouring or tightening can reach. Where the answer is a procedure Dr. Grandhige performs himself, including abdominoplasty, lower body lift, 360 contouring and revision work, he says so and schedules it. Where the answer sits outside that range, Formation points the patient to a surgeon who does that specific work well. Formation stays in contact with those surgeons. Nobody gets talked into the wrong operation because it was the one available in the building.
Dr. Grandhige holds certification from the American Board of Surgery and completed medical school at the University of Michigan. He began a combined plastic surgery residency at Yale before shifting to a full general surgery residency, then fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. That pathway is unusual in aesthetic work. It is also why his planning starts from wound healing and inflammation rather than from surface appearance. Dr. Ahmad Saad, a board-certified plastic surgeon, is part of the surgical team.
The 60/40 Recovery Standard
Formation treats surgery as 60 percent of the result and post-operative care as the other 40 percent. Incorrect aftercare can undo a well-performed operation. That ratio is why recovery is managed as a clinical phase rather than handed over as a printout.
What Gets Assessed Before Surgery
Health and diet are assessed before a surgical date is confirmed. Nutrition, supplements and peptides are then adjusted in the window immediately before and after the operation so the body builds as much structure as it can while it is healing. Protein intake, inflammation control and compression planning are dialed in during preparation, not improvised afterward.
What Gets Adjusted After
Lymphatic treatment begins at the first post-operative visit. Compression is continuous through week one. If a seroma forms it is drained rather than watched. Follow-up runs for a full year, and nearly all patients complete it.
For body work, most patients are back in the gym within one to two weeks and unrestricted between weeks two and four with the garment on. Abdominoplasty is different: six to eight weeks before unrestricted training. For the neck and jawline, patients return to training at one week.
Every practice in Tampa Bay can buy the same cannula and the same energy device. The instrument is identical everywhere. What surrounds it is not.
The Formation Crossing Plan For Pinellas County Patients
Three bridges connect Pinellas and Hillsborough counties. Which one a patient uses changes the arrival, and after general anesthesia it changes who is behind the wheel. Formation plans the crossing before surgery day rather than on it.
Decision One: Which Span You Use
The Howard Frankland carries I-275 across Old Tampa Bay and is by far the busiest of the bay bridges. FDOT put daily volume near 200,000 vehicles, more than the Courtney Campbell, Gandy and Skyway combined. The original 1960 span closed permanently in July 2025 and northbound traffic moved to the 1991 structure. As of August 22, 2026, the new express lanes are tolled electronically around the clock through SunPass, and they are restricted to passenger vehicles, vanpools and buses.
The route matters at the Tampa end. Coming south on I-275, a patient exits toward Armenia Avenue, because Armenia and Howard run as a one-way pair between Swann Avenue and Tampa Bay Boulevard. Howard carries northbound traffic in that stretch. Driving south on Howard from Kennedy is not possible. The turn is east onto Swann, then south onto Howard.
The Gandy is the quieter option and reaches the clinic differently. Howard Avenue begins at Bayshore Boulevard in Historic Hyde Park, so a patient landing off the Gandy comes north up Bayshore and onto Howard from the south end, clear of the one-way pair entirely. On a post-operative visit with swelling and a garment, that is the easier approach.
Decision Two: Whether You Go Home The Same Day
Downtown St. Petersburg sits roughly 22 miles from the clinic by road, about half an hour without traffic. That is a manageable drive on a normal day. It is a different drive at 48 hours post-operative, in compression, with a nine-mile open-water span and no realistic place to stop.
Formation holds rates at nearby Tampa hotels for patients who would rather not cross twice in two days. Dr. Grandhige visits those patients in the hotel room during the early days so they are not making the trip to the office at all. Pinellas patients use this more than local patients do, and for good reason.
Decision Three: Who Covers Week One
Someone else drives after general anesthesia. That is not negotiable, and it needs a name attached before surgery day rather than a plan to sort out later.
St. Petersburg housing shapes the rest. A single-story bungalow in Historic Kenwood makes week one simple. The two-story homes common in Historic Old Northeast put a staircase between a compressed patient and a bathroom. A downtown condo means an elevator, a parking garage and a walk. None of this is a reason to postpone. All of it is worth mapping before the date, and it is part of what the consultation covers.
What Florida Requires Of The Room You Operate In
Florida regulates office surgery more specifically than most states, and the rules apply the same way in Pinellas and Hillsborough.
Under Rule 64B8-9.009, a maximum of 4,000 cc of supernatant fat may be removed by liposuction in an office setting. When liposuction is combined with abdominoplasty, or is directly related to another procedure, it may not exceed 1,000 cc. Any office removing more than 1,000 cc must register with the Florida Department of Health, and registration is publicly checkable.
Accreditation matters here in a concrete way. Every office surgery facility takes a preregistration inspection. After that, a facility must submit to annual state inspections unless it holds current accreditation from a nationally recognized accrediting agency named in the rule. Formation operates in Quad A accredited facilities, which is one of those named agencies, so the accrediting body’s own inspection cycle applies instead of the state’s annual visit.
Being honest about the limit of that: accreditation replaces the annual inspection, not the preregistration one, and it is a facility standard rather than a statement about any individual surgeon’s results. It is worth verifying rather than taking on trust. Both certification and office registration can be looked up in a few minutes.
How A St. Petersburg Consultation Works
There are two ways in, and they are not the same thing.
The in-office consultation with Dr. Grandhige carries a fee. It is a full surgical assessment: anatomical evaluation, candidacy, an honest read on options and limits, and guidance on timing and recovery. The fee can be applied toward any procedure or product at the practice. Patients leave with a plan even when the plan is to wait.
Before committing to a drive across the bay, St. Petersburg patients can start with a complimentary virtual evaluation with patient coordinator Hanna. She reviews goals, explains what applies and what does not, and outlines a path forward. It costs nothing and it happens over video, which for a Pinellas patient answers the threshold question without a bridge crossing.
Three Real Cases, Three Different Answers

An active male physician trained consistently and ate well but never saw abdominal and flank definition match the effort. He did not want an etched look.
