The Add-Or-Subtract Rule, Run Before Any Thread

Before threads go anywhere near a jawline, Dr. Grandhige sorts the complaint into one of three answers: subtract, add or lift. Threads only answer the third.

The Formation Add-or-Subtract Rule exists because a blurred jawline looks the same in a photo whatever the cause. What reads as fat from the outside can come from fat under the skin, deeper fat beneath the platysma muscle, the salivary glands, the muscles under the chin, or the natural projection of the chin and jaw. Most necks show aging in more than one layer at once, so the rule works alongside the Four-Layer Neck Read: fat, the platysma muscle and its bands, skin, and the frame of the chin and jaw.

When The Answer Is Subtract

A soft, pinchable fullness under the chin is layer one, fat. A thread can’t remove it, and lifting a full neck only relocates the fullness. Past your mid-forties, expect the evaluation to test whether another layer is the real problem.
