What a PDO Thread Lift Does, and What It Does Not
A PDO thread lift treats mild to moderate skin laxity along the jawline, under the chin, and in the mid to lower face. It works two ways at once. Barbed threads grip and reposition loose tissue for an immediate lift, and the polydioxanone material sets off a healing response that builds new collagen over the following weeks.
Polydioxanone is not new. It is cleared by the FDA as an absorbable surgical suture and has been used in surgery for decades. In a thread lift, that same material is placed just under the skin rather than tied in a wound. The threads dissolve on their own in roughly six to nine months. The collagen they leave behind is what carries your result, which is why most people hold their improvement for about 12 to 18 months.
A thread lift also has clear limits. It does not remove significant loose skin, it does not replace lost facial volume, and it is not a substitute for weight loss. When someone needs any of those things, threads alone disappoint. Saying that up front matters more than filling a schedule.
The Formation Lift Ladder: Matching the Lift to Your Skin
Not everyone who wants a thread lift should get one. We use a simple model called the Formation Lift Ladder to place you at the right level of care, based on how much your skin has loosened and how much spring it still has. This is the backbone of our PDO thread lift program.
Rung one is early or minimal laxity. Your skin still snaps back, and a soft shadow along the jaw bothers you more in photos than in the mirror. Threads here are premature. We usually recommend waiting or working on skin quality first.
Rung two is mild laxity, and it is where threads do their best work. There is enough looseness to lift and enough elasticity to hold the new position. Most of our thread candidates sit here.
Rung three is moderate laxity paired with early volume loss. Threads can help, but on their own they leave the face looking pulled rather than restored. We often stage them with other support so the lift looks natural.
Rung four is advanced laxity or heavy tissue. The skin no longer has the elasticity to hold a thread, and pretending otherwise leads to a short-lived, disappointing result. Here the honest answer is a surgical lift or energy-based tightening, not threads.
Across the thread-lift consultations we ran in 2025, only about a third of the people who came in asking for threads actually landed on rung two. The rest we pointed toward a plan that suited their skin better. That ratio is the whole point of the ladder.
When a Thread Lift Is the Wrong Call
A thread lift is the wrong call when the main problem is loose skin, lost volume, or a result you expect to last for years. Threads lift. They do not excise skin or rebuild structure. If your neck has real skin excess, a surgical approach will serve you far better, and we will tell you so.
This matters more in 2026 than it used to. Aesthetic surgeons are seeing a wave of patients whose faces and necks have loosened after fast weight loss on GLP-1 weight-loss medications, a pattern documented in recent peer-reviewed aesthetic surgery work. Early, mild laxity from weight loss can respond well to threads. Heavier laxity usually needs more than a thread can provide, and combining threads with the wrong expectation is how people end up frustrated.
When threads are not the answer, they are often not the end of the conversation either. Some patients do better with non-surgical neck and jawline contouring, some with a staged surgical plan, and some with nothing at all for now. The goal of the visit is a clear recommendation, not a booked procedure.
How the Procedure Works at Formation

Gopal Grandhige, MD, a board-certified general surgeon who specializes in aesthetic surgery, personally evaluates every thread-lift candidate. He examines your skin, places you on the Lift Ladder, and explains what threads can and cannot do for your face. If threads are a good fit, you leave with a plan. If they are not, you leave knowing why.
The Treatment
The procedure is done in the office. We numb the area with local anesthesia, and an oral sedative is available if you want it. Using a fine cannula, threads are placed along the jawline and neck through very small entry points, then anchored to lift the tissue into a cleaner line. Most treatments take 30 to 60 minutes, depending on how many areas we address.
Recovery and Results
Expect mild swelling, tightness, and sometimes light bruising for a few days. Minor bunching at the entry points is normal and settles on its own over two to four weeks as the threads relax into place. Most people return to daily activities within a day or two, while strenuous workouts and facial massage wait about two weeks. You see a lift right away, and because the threads keep building collagen for weeks afterward, the result continues to refine before it holds for roughly 12 to 18 months.
