The Four-Layer Neck Read

Most necks show aging in more than one layer at once. Liposuction treats one of them. The Four-Layer Neck Read is how Dr. Grandhige sorts out what is actually causing the fullness before he chooses a tool, and each layer has its own solution.

Layer One: Fat

This is the fat between the skin and the platysma muscle, and it’s the only layer liposuction reaches. It responds well when it’s the main problem. One catch Dr. Grandhige points out: fat sitting beneath the platysma, and the salivary glands under the jaw, can look exactly like layer-one fat from the outside. Neither comes out with liposuction above the muscle. If either is driving the fullness, liposuction alone won’t create the contour you’re picturing.

Layer Two: The Platysma And Its Bands

The platysma is the thin sheet of muscle across the front of the neck. As it loosens with age, it can show as vertical platysmal bands. Liposuction doesn’t change the muscle, so bands stay where they are after the fat is gone. Where bands are present, the layer-two solution is MyEllevate, a permanent suspension suture, paired with platysmaplasty to tighten the muscle itself.

Layer Three: Skin And The Fibroseptal Network

Once the fat is out, the skin has to shrink back over the new contour. Dr. Grandhige rates skin elasticity a 10 out of 10 for how a liposuction result turns out. Procedure Day In The Office

Florida draws its office-surgery lines by how much fat comes out and how deep the anesthesia goes. The Florida Board of Medicine classifies liposuction that removes less than 1,000 cc of supernatant fat, under local or topical anesthesia with no more than minimal pre-procedure tranquilization, as Level I office surgery. Risks And What Neck Liposuction Won't Do

The complication that matters most in neck liposuction usually isn’t infection. It’s taking too much. Over-resection leaves irregular contours and a neck that ages early, and because removed fat cells don’t return, waiting won’t fix it. Restraint is the safety feature.

Published numbers help, as long as you read their limits. A retrospective series in Aesthetic Plastic Surgery, published in January 2026, reviewed 435 women treated with office-based neck liposuction under local anesthesia in 2023. Complications occurred in 3.45% of them. Four had a temporary injury to the marginal mandibular nerve, which moves the lower lip, and all four recovered.

The limits are real: it’s a single series from Saudi Arabia, every patient was female, the technique was ultrasound-assisted, and 78 of the 513 eligible patients didn’t take part. Read it as what careful selection can look like, not a forecast for any office, this one included.

Liposuction isn’t weight loss either. The best candidates are already close to their goal weight, and the procedure refines shape rather than moving the scale.

Recovering From Dunedin

You’ll spend most of your recovery at home.
