The Four-Layer Neck Read
Most necks show aging in more than one layer at once. That is why two people with what looks like the same complaint need different treatments, and why a practice that only sells one thing will tell you that one thing is the answer. Dr. Gopal Grandhige, a board-certified general surgeon specializing in aesthetic surgery, reads every neck in four layers before recommending anything.
Layer One, Fat
Fat under the chin blurs the line between jaw and neck. It responds to direct removal and almost nothing else, which is why people who lose twenty pounds often find this area unchanged. Layer Two, The Platysma
The platysma is the flat sheet of muscle across the front of the neck. When its edges separate with age, they show as two vertical cords running from jaw to collarbone. These are platysmal bands, and no amount of energy tightening will fix them, because the problem is muscle position, not skin laxity. Bands need a suspension approach, MyEllevate paired with platysmaplasty, which repositions the muscle rather than heating the tissue above it. Layer Three, Skin And The Fibroseptal Network

When FaceTite Is The Wrong Call For You
There are four situations where we will tell you no.
Skin that has lost its recoil entirely is the first. If the skin no longer springs back when released, taking out the fat beneath it removes the only thing filling it out, and the neck can read looser afterward than it did before. This is the single most common reason people end up unhappy with energy tightening, and it is predictable at the consultation.
Heavy platysmal banding is the second. Bands are a muscle problem. Heating the skin above them does not move them.
True facelift-tier laxity is the third. When there is enough excess skin that it needs to be removed rather than tightened, no device reaches that.
