The abdomen can be one of the most stubborn areas to change, even with consistent diet and exercise.
For this patient, HD liposuction addressed the persistent abdominal fullness while creating more definition through the waist and midsection.
At 10 months post-op, you can see how much that one area affected the proportions of her entire torso.
If you’re dealing with stubborn abdominal fat despite diet and exercise, schedule a consultation with Dr. Gopal at Formation Tampa to learn whether liposuction may be appropriate for you.

Many surgeons abandon this lipo technique because it’s so physically demanding.
The approach to HD liposuction Dr. Gopal uses requires an extraordinary level of detail and precision.
Rather than simply etching lines, every contour and transition is shaped around the patient’s underlying anatomy, a process that can take several hours.
Dr. Gopal trained in this technique in Barcelona with one of the world’s leaders in HD liposuction and now teaches it to other physicians. It’s a demanding approach, but that level of precision is what allows for definition that looks natural to the individual.

“Glad I took my time researching.”
That line matters.
Choosing to have plastic surgery isn’t just about deciding you want a change. It’s about deciding who you trust to understand the result you’re looking for, especially when the goal isn’t simply removing fat, but creating a shape that still looks natural to you.
For our lovely patient, that meant a tighter stomach and waist, feeling more comfortable in fitted clothing, and most importantly, getting the result she wanted “without looking fake.”
That’s the goal of thoughtful body contouring: not to make you look like someone else, but to create a result that works with your body and your goals.

More definition doesn’t automatically mean a better liposuction result.
The goal of body contouring is to create definition that makes sense for your anatomy, not to create the most sculpted abdomen possible simply because we can.
Low-, medium-, and high-definition liposuction can create very different looks, from subtle contouring to a more athletic or highly sculpted appearance.
The right level depends on your existing muscle definition, proportions, surrounding areas of the body, and the result you want to achieve.
For Dr. Gopal Grandhige, a board-certified surgeon specializing in body contouring and high-definition liposuction in Tampa, the goal is balance: the abdomen, chest, back, arms and shoulders should look like they belong to the same body.
The result should feel cohesive, proportional, and natural to your anatomy.

One of the biggest misconceptions about liposuction? That the fat either “comes back” or simply moves somewhere else.
Liposuction permanently removes fat cells from the treated area. But it doesn’t remove every fat cell, and the cells that remain can still grow larger with weight gain.
That’s why long-term results aren’t just about what happens during the procedure. Maintaining a relatively stable weight helps preserve the contour created through liposuction.
Dr. Gopal Grandhige specializes in advanced high definition liposuction and body contouring, with an emphasis on creating results that complement each patient’s natural anatomy.

6 months post HD liposuction with moderate definition.
The goal was a leaner, more athletic contour with visible definition through the abdomen and torso, without pushing the result to the highest level of definition.
HD liposuction can be customized based on anatomy, muscle structure, and the degree of definition someone wants to achieve.
For this patient, moderate definition created the balance Dr. Gopal was looking for.

These lipo terms aren’t interchangeable.
Traditional liposuction, Lipo 360, and high-definition liposuction can all address unwanted fat, but the approach depends on what you’re actually trying to change.
Maybe it’s one stubborn area that hasn’t responded to diet and exercise.
Maybe the goal is creating better proportion throughout the entire midsection.
Or maybe you’re already lean and looking for more visible, athletic definition.
Those are different goals, and they call for different treatment plans.

Each treatment plan starts with your anatomy, your current body composition, and the result you want to achieve.

What Neck And Jawline Contouring Actually Does
Neck and jawline contouring removes the fat sitting between your chin and your neck, then tightens the skin over what remains, so the line of your jaw separates cleanly from your neck again. It runs about an hour in the office. Most patients are back at work and back in the gym one week later.
It isn’t a facelift and it isn’t weight loss. Dr. Gopal Grandhige performs it most often on people who are already fit and still carry fullness under the chin that training never touches. That fullness is largely genetic. It sits in a fat compartment that responds poorly to dieting, which is why the gym doesn’t fix it and why the same conversation happens in almost every consultation.
Results vary between practices for a reason, and it isn’t the equipment. It’s whether anyone looked carefully at which part of your neck is actually the problem before choosing a treatment. Four separate structures can make a neck look heavy. Each one needs a different answer. This page is the same procedure Formation performs as neck and jawline contouring in Tampa, written for patients driving in from Pinellas County.
The Four-Layer Neck Read
Every neck Dr. Grandhige evaluates gets read in four layers before anything is recommended. Most necks show aging in more than one layer at once, and treating the wrong one is how a patient ends up disappointed after a technically clean procedure.
Layer One, The Fat
The submental fat pad sits between the skin and the muscle. It is what most people mean by a double chin, and it is the layer that neck liposuction addresses. Fat comes out through very small incisions under the chin and behind the ears, with cannulas sized for the neck rather than the body. Removed fat cells don’t return, so this part of the result holds as long as your weight stays stable. Restraint matters more here than volume. Taking too much creates hollowing and surface irregularity that reads as older rather than sharper, and it is the most common way a neck gets over-treated.
Layer Two, The Platysma And Platysmal Bands
Beneath the fat is the platysma, a thin sheet of muscle running from the collarbone up to the jaw. With age its inner edges separate and stand up as two vertical cords, the platysmal bands. Removing fat from above them makes them more visible, not less, because the fat was hiding them. Where bands are present, MyEllevate, a permanent suspension suture, is used along with platysmaplasty to bring the muscle edges back toward the midline and support the jaw line from underneath. Unlike barbed threads that dissolve over months, this suture doesn’t absorb and can be removed if needed. The lift is there the day it is placed. A dedicated Formation page on MyEllevate is in progress.
Layer Three, The Skin And The Fibroseptal Network
Skin rests on a connective tissue foundation called the fibroseptal network, which anchors it to the muscle below. When that foundation loosens, skin stops springing back after fat is removed. FaceTite, a radiofrequency-assisted device, and Renuvion, a helium plasma and radiofrequency device, both work at that depth, from underneath the skin. Lasers and microneedling work from the surface downward and cannot reach the layer that is actually failing. Both devices are 510(k)-cleared, which means the FDA found them substantially equivalent to devices already on the market rather than approving them through new trials. That distinction is worth knowing the next time a clinic advertises something as FDA approved.
Layer Four, Chin Projection And Jaw Angle
Some necks are not heavy at all. The frame behind them is short. A recessed chin or a soft jaw angle makes an ordinary amount of fat look like a double chin, and removing that fat will not correct a proportion problem. Jawline fillers address this layer, using Radiesse, a calcium hydroxylapatite, for structural projection and Juvederm, a hyaluronic acid, for softer contour along the border. A superhero jawline is mostly a frame question. Patients whose fourth layer gets read correctly usually need less fat removed, not more.
Who This Works For, And Who It Doesn't
The best candidate is bothered by a double chin or a soft jaw line, has reasonable skin elasticity, and wants refinement rather than a dramatic change. Skin elasticity holds up well for roughly the 25 to 55 range, though the number on a birth certificate matters less than how the skin behaves when Dr. Grandhige tests it.
This is the wrong procedure for you if your neck skin no longer recoils. Removing fat from a neck that cannot retract leaves a looser neck than you started with. It is also the wrong procedure if your real issue is excess skin that needs to be cut out rather than tightened. Patients past what contouring and energy tightening can reach are referred outward. Formation stays in contact with the surgeons who do that work well and points you to the right one instead of talking you into something smaller that won’t hold.
Patients carrying fullness across the torso as well as the neck are often better served starting with high-definition liposuction and treating the neck as a second stage.
One representative outcome: a professional in his forties came in fit, otherwise happy with his body, and frustrated that his neck made him look heavier and older than he felt. He had always looked that way and had stopped appearing in photographs. After targeted neck liposuction with jawline contouring, colleagues told him he looked more rested. Nobody guessed why. He started getting in pictures again.
Why More People Are Treating Their Necks In 2026
The American Society of Plastic Surgeons released its 2025 procedural statistics this year, and the direction is clear. Neck lifts rose 21 percent year over year, facial fat grafting recorded the largest single increase of any cosmetic surgery procedure at 39 percent, and 82 percent of surveyed ASPS member surgeons reported consultation requests connected to GLP-1 medications. Overall cosmetic surgical volume was up 7 percent, and member surgeons pointed to restoration after weight loss, pregnancy and aging as the leading reason patients came in. You can read the 2025 ASPS report summary directly. plasticsurgeryplasticsurgery
Read that carefully before you assume it applies to you. A neck lift is a different, larger operation than neck contouring, and ASPS counts procedures performed by its member plastic surgeons, so it isn’t a complete picture of the market. What the data does say is that a lot of people finishing significant weight loss are discovering their neck didn’t follow the rest of them down.
What Procedure Day Looks Like
You stay awake. Neck and jawline contouring at Formation is done in the office under an oral sedative and local anesthesia, not general anesthesia in a surgical facility. Very small incisions go under the chin and behind the ears. Fat is removed with the jaw angles preserved rather than flattened, and then Renuvion or FaceTite is used to tighten the skin down over the muscle and bone underneath. You go home the same day wearing a chin garment.
Staying out of general anesthesia is not a marketing preference. It removes an entire category of risk from a procedure that doesn’t require it. The published safety record for neck liposuction under local anesthesia supports that: a case series of 987 neck liposuction patients treated by three surgeons reported five temporary marginal mandibular nerve issues, one submandibular gland ptosis and one arterial bleed, with no seromas, skin necrosis, scarring or hyperpigmentation. The honest limitation: that is a retrospective series from experienced hands, not a randomized trial, it doesn’t measure long-term skin laxity, and it doesn’t tell you what happens when the wrong patient is selected. Rare problems do happen. They are rarer when candidacy is honest.
For patients whose plan includes surgical work in an operating room, Formation uses fully accredited Quad A certified facilities with the same anesthesia provider and the same staff on every case.
Recovery Week By Week
First few days. Mild swelling, bruising and tightness. The chin compression garment goes on and stays on.
One week. Back to work and back to social life. Back to training. The garment is still worn 12 to 24 hours a day through this stretch, which is the part patients underestimate.
Two to four weeks. Swelling keeps settling and the results of the fat removal become clearly visible.
Three months. Skin tightening shows, with marked improvement along the jaw line.
Six to nine months. Final result, once the skin has finished tightening.
The gap between month three and month nine is where most online disappointment comes from. People post at seven weeks convinced it didn’t work. Swelling behaves like a tide, better some mornings and worse after a long day or a flight. Knowing the real curve in advance is most of the difference between a patient who is happy and a patient who panics.
How Formation Prepares Your Body Before And After
The device is the same everywhere. What surrounds it isn’t.
Before treatment, your health and diet are assessed, and nutrition, supplements and peptides are adjusted in the window immediately before and immediately after so your body builds the most structure it can while the skin is tightening. Protein intake and inflammation control get dialed in on purpose rather than left to chance.
Dr. Grandhige puts a number on it: the surgery is 60 percent of the result and the post-operative care is 40 percent. Poor aftercare can undo a well-performed procedure. That is why Formation keeps its own post-operative care in house rather than handing you a printout and a follow-up date, and why patients are followed for a full year rather than discharged at six weeks.
Any practice in Pinellas or Hillsborough County can buy the same equipment. The thinking around it is the part that doesn’t transfer.
The Surgeon Reading Your Neck
Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He is not a plastic surgeon, and he says so plainly. His certification is through the American Board of Surgery.
He went to medical school at the University of Michigan and matched into the six-year combined plastic surgery residency at Yale before changing course into a full general surgery residency, followed by fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. Fifteen years and several thousand operative cases later, that background is what shapes how he plans a neck: tissue behavior, wound healing and inflammation, not just what looks good on the table.
His position on this procedure runs against most of what you’ll read: more aggressive fat removal does not produce a sharper jaw line. Over-resection produces contour irregularity, unmasked bands and skin that ages badly. He removes less than patients expect, and he is willing to tell you not to do it at all. Some consultations end with a plan that isn’t surgery.
Coming To Formation From Clearwater
Formation is at 1315 South Howard Avenue in South Tampa, roughly 25 miles from downtown Clearwater. The direct route is Gulf to Bay Boulevard east onto State Road 60, across the Courtney Campbell Causeway over Old Tampa Bay, past Rocky Point and the airport onto Kennedy Boulevard, then south on Howard into SoHo. Figure 35 to 45 minutes outside rush hour, longer when the causeway backs up. The Howard Frankland Bridge on I-275 is the alternate when SR 60 is jammed.
Plan a driver. Because the procedure uses an oral sedative, you cannot drive yourself back across the bay afterward. Clearwater patients are close enough to come back for follow-up visits rather than staying over, which is the practical advantage Pinellas patients have over the patients Formation sees from Naples, Orlando and Atlanta.
Timing matters here more than most places. Pinellas fills up between January and April, and plenty of Clearwater residents leave for the summer. Work backward from the timeline above: visible tightening lands around month three and the final result around months six to nine. A patient who wants their jaw line settled before next season books in spring or early summer, not in December. Patients who show up in January asking to look different by March get told the truth about what is possible.
You can see neck and jawline results for men in the Formation gallery.
The in-office consultation with Dr. Grandhige carries a fee and is a full surgical assessment, and that fee can be applied toward any procedure or product at the practice. If you want to know whether the drive is worth making first, Formation also offers a complimentary virtual evaluation with patient coordinator Hanna, who reviews your goals and outlines a plan before you commit to coming in.
Booking Your Consultation
The decision worth making carefully isn’t which device to pick. It’s which layer of your neck is actually causing the problem, because the answer determines whether one treatment fixes it or three are needed. That reading takes an in-person evaluation, and it’s the whole point of the visit.
You’ll leave with a plan and a realistic timeline, or with a clear reason not to proceed yet. Both count as a useful outcome. Formation is a short drive across the bay from Clearwater, and Dr. Grandhige performs and follows every case himself.
Frequently Asked Questions
Most patients are back at work and back in the gym at one week, wearing the chin garment 12 to 24 hours a day. Swelling keeps improving through weeks two to four. Skin tightening becomes visible around three months, and the final result lands somewhere between six and nine months.
That is the design goal. Dr. Grandhige works conservatively and preserves natural jaw angles rather than carving lines that announce themselves. Patients commonly report that colleagues say they look rested or fitter without identifying why.
Possibly, but the plan changes. If skin no longer recoils, fat removal alone will leave you looser than you started, so tightening has to be planned from the beginning rather than added later. If the laxity is past what energy tightening reaches, you'll be told that and referred to a surgeon who removes skin.
No. It's performed in the office with an oral sedative and local anesthesia. You are awake and you go home the same day.
Fat cells removed during the procedure do not come back, so that portion is durable as long as your weight stays stable. Aging continues, but clearing excess fat in this area slows the appearance of future laxity.
No. The oral sedative rules that out. Arrange a driver for the trip across the causeway before your procedure date.
You'll be told during the consultation rather than after the procedure. Formation stays in contact with surgeons who handle skin-excision and facelift-level work and refers those patients outward.
Yes. The in-office consultation with Dr. Grandhige carries a fee and is a full surgical assessment, and that fee can be applied toward any procedure or product at the practice. The complimentary virtual evaluation with patient coordinator Hanna is a separate option if you want direction before making the drive.
