Formation reads the neck in four layers. Each one has its own solution. Before a procedure gets named, the question is which layers are actually producing what you see in photographs.
Layer One, The Fat Under The Chin
Submental fat. It sits below the platysma in most people, which is why crash dieting rarely touches it and why a fit forty-five-year-old can carry a full double chin. The solution here is neck liposuction, done through small incisions under the chin and behind the ears. Energy does not solve layer one. Removal does.
Layer Two, The Platysma And Its Bands
The platysma is the flat sheet of muscle covering the front of the neck. With age its edges separate and stand forward as two vertical cords, the platysmal bands. You can see them when you clench your jaw. No amount of skin tightening flattens them, because they are muscle, not skin. Layer Three, Skin And The Fibroseptal Network
Beneath the skin sits the fibroseptal network, the connective tissue scaffolding that anchors skin to the structures under it. When it loosens, the skin drapes instead of holding. This is the layer FaceTite treats. It’s also the layer that surface treatments cannot get to, which is the whole argument for putting the energy underneath.
Layer Four, Chin Projection And Jaw Angle
Sometimes the neck is fine and the frame is short. A recessed chin makes any amount of submental fullness look worse, because there is no forward point for the jawline to run to. Removing fat from a weak frame does not build a superhero jawline. It just makes the recession more obvious. Why Energy Delivered Beneath The Skin Reaches Deeper
The difference between FaceTite and the tightening treatments advertised at every medspa on US 19 is not brand. It’s depth of access.
FaceTite works from underneath. The probe travels in the plane where fat, skin and the fibroseptal network meet, and the heat lands on that foundation directly. Lasers and microneedling work from the top down. They pass through the epidermis and dermis and stop well above the network that actually holds the neck up. They can improve texture and tone. They cannot lift a foundation they never reach.
Collagen response follows the heat, and it keeps working for months. That’s real, and it’s the reason results keep improving long after the swelling goes. But it is the consequence, not the mechanism. The mechanism is where the energy is placed.
One point of accuracy, since local pages get this wrong constantly: the radiofrequency platform behind FaceTite is 510(k)-cleared, not FDA-approved. Clearance means the device was shown substantially equivalent to something already on the market, which is a different and lower bar than the premarket approval pathway used for high-risk implants. You can look up any device’s clearance record yourself in the FDA 510(k) database. Anyone writing “FDA-approved” is either careless or hoping you don’t check.
What Most FaceTite Pages Leave Out
Three things, and all three matter more than the device specs.
- There are incisions. Small ones, placed under the chin and behind the ears where they sit in shadow and in the crease. That is not the same as none. “Scarless” is marketing shorthand for well-hidden. If your hair is short and you shave clean, ask to see where the access points go before you book, not after.
- Removing fat without a skin response can make the neck look worse. This is the outcome nobody advertises. Take out the padding, get no contraction from the layer above it, and what’s left is a thinner neck that hangs more than it did before. Some of those patients end up needing surgical correction. It’s the reason Formation screens skin quality first and treats roughly skin ages 25 to 55 as the working window for energy tightening. Past that window, tightening is not the honest answer. Patients beyond what tightening and contouring can reach get referred out, and Formation stays in contact with the surgeons who do that work well so the handoff is to a specific name rather than a search result.
- The marginal mandibular branch of the facial nerve runs through this territory. Aggressive heat near it can cause temporary weakness at the corner of the mouth. It’s uncommon and usually resolves, but it is documented and it is technique-dependent, which is why the operator matters more here than the equipment. Dr. Grandhige is board certified in general surgery with fellowship training in burn surgery and critical care. Thermal injury and how tissue behaves under heat is not a new subject for him.
Renuvion Or FaceTite, And Why Formation Doesn't Pick First
Formation runs both. Renuvion uses helium plasma energized by radiofrequency; FaceTite uses radiofrequency between two poles. Both deliver energy under the skin. They behave differently in different necks.
Most practices own one. Which means most pages you’ll read recommend one, and the recommendation was made before anyone saw your anatomy. That’s backwards, and it’s the single most common reason people end up with a treatment that was right for the clinic’s inventory rather than their neck.

What The Day Looks Like
Neck and jawline work at Formation is an in-office procedure. You take an oral sedative, the area is numbed with local anesthesia, and the whole thing runs through very small incisions under the chin and behind the ears. Fat comes out first, carefully, preserving natural jaw angles rather than stripping the area flat. Then the energy goes in to tighten the skin down over the muscle and bone underneath.
You go home the same day in a chin compression garment.
Coming from Palm Harbor, plan for two practical things. You’ll need a driver, because of the sedative. And the drive to our South Tampa office runs about forty minutes across the Courtney Campbell outside rush hour, longer if you hit the causeway wrong. The instrument is identical at every practice that owns one. You can buy the same handpiece. What differs is what happens in the two weeks on either side of the appointment.
