The Formation Lift Ladder
Dr. Grandhige sorts every thread lift inquiry onto one of three rungs before discussing technique at all. The rung, not the request, decides the answer.
Rung One, Early Laxity With Good Recoil
Skin that still snaps back when you pinch and release it along the jaw. Volume loss is mild. The jawline is softening rather than sagging. Threads work well here, and so do structural fillers placed at the chin and jaw angle. Sometimes the better answer on this rung is not threads at all but volume: a calcium hydroxylapatite or hyaluronic acid product placed at the bone can rebuild a jaw angle that has flattened. Formation’s injectable options cover that path.
Rung Two, Moderate Laxity With Thinning Skin
Recoil is slower. Skin quality has changed, not just position. Threads placed into skin that has lost thickness are the most common source of the visible-thread and dimpling problems documented in the literature. On this rung Dr. Grandhige usually favors energy-based tightening, alone or before threads, because FaceTite treats the skin from underneath rather than pulling on it. Pulling weak tissue upward does not make it stronger.
Rung Three, Skin That Needs Removing
A standing fold of skin at rest along the jaw or under the chin. Recoil is minimal. Nothing that suspends or tightens will reach this. Threads on rung three produce a short, disappointing result and leave tissue changes behind. Formation says no here and refers outward. Dr. Grandhige does not perform facelifts, and he stays in contact with surgeons who do that work well so he can point you to the right one rather than sell you something adjacent.
The decision rule is short. Threads reposition tissue. They do not remove it, restore its thickness, or rebuild bone.
Why The Palm Harbor Age Profile Changes The Answer
Palm Harbor’s median age is 50.9 years, and 54.4 for women. The elasticity range where jawline and neck work holds up well runs roughly from skin age 25 to 55. How Dr. Grandhige Places Threads
Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He trained at the University of Michigan Medical School, matched into a combined plastic surgery residency at Yale before changing paths, and completed a general surgery residency followed by fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. That background is unusual in aesthetics, and it is why he reads tissue behavior and healing biology first and technique second. His full training history is on his profile.
The treatment itself is short. The entry points are numbed with local anesthetic. Threads are introduced through a fine needle or cannula along planned vectors, the tissue is repositioned, and the ends are trimmed. Most patients describe pressure and tugging rather than pain.
He personally evaluates every patient and designs the plan himself. He also limits how much he takes on. Formation is a boutique practice, not a volume clinic, and Dr. Grandhige follows patients for a full year after treatment rather than closing the file at six weeks.
What The Published Data Says About Side Effects
Most pages about this treatment say minimal downtime and stop. There is better information available.
A 2026 meta-analysis in Frontiers in Surgery pooled 26 studies covering 2,827 thread lift patients. The most useful thing it did was split findings by timing rather than lumping them together.
Early events, inside the first four weeks, were common and mostly self-limiting. Swelling appeared in 34 percent of cases, bruising in 26 percent, and pain in 11 percent. Swelling generally settled within two to four weeks and bruising within one to three. The authors argue these are better understood as expected tissue responses to threads being passed through soft tissue than as complications.
Late-onset findings, persisting past four weeks, matter more because they can require correction. Visible or palpable threads appeared in 10 percent of patients, skin dimpling or asymmetry in 7 percent, altered sensation in 6 percent, thread exposure in 5 percent, and infection in 2 percent. Dimpling resolved on its own within three to six months in several of the included studies, though a subset of patients needed massage or a minor revision.
The published limitation is significant and should be stated alongside those figures. The authors did not run subgroup analysis by thread material, so PDO, polycaprolactone, poly-L-lactic acid, and permanent polypropylene threads are pooled together. Statistical heterogeneity was high, above 90 percent for swelling and bruising. The pooled numbers describe thread lifting as a category, not PDO specifically, and the authors themselves note that biodegradable materials likely carry a different profile from permanent sutures.

A 2025 retrospective series in Aesthetic Plastic Surgery examined facelifts performed between January 2021 and December 2023 on ten patients aged 51 to 68, all of whom had undergone two or more absorbable thread lifts beforehand. Intraoperatively, the surgeon documented fibrosis, tissue distortion, and fusion between anatomical layers that normally separate cleanly during dissection.
The limitation matters here too. Ten patients, a single surgeon’s series, level five evidence. Other experienced facelift surgeons report that prior thread fibrosis is minimal and dissected through without affecting the result. The honest summary is that the profession disagrees, the finding is real but not universal, and it appears more often in patients with multiple prior thread rounds than a single one.
What it means practically: if a facelift is somewhere in your ten-year plan, that belongs in the conversation now, not later. Threads are frequently sold as a bridge to surgery. In some patients they narrow the bridge. Formation’s position is that this is exactly the kind of trade-off a patient should hear before consenting, not after, and it is a large part of why some Palm Harbor inquiries end with a recommendation to wait.
Preparation And Recovery, And The 60/40 Standard
Formation treats placement as 60 percent of the result and the weeks around it as the other 40. That ratio is not a slogan. It is why the practice does not hand off aftercare.
Health and diet are assessed before treatment. Nutrition, supplements, and peptides are then adjusted in the window immediately before and immediately after, so the body builds as much structure as it is capable of building along the thread tracks. Inflammation control and protein intake get specific attention, because collagen deposition is the part of the result that outlasts the threads themselves. Sleeping position, avoiding facial massage, and holding off on strenuous exercise for the first two weeks all protect the mechanical lift while the tissue settles.
