Who This Works For, And Who It Disappoints

Patients with a fold that hangs at rest. If the skin will not recoil at all, no energy device closes that gap, and the honest answer is skin excision. Formation does not perform neck lifts. Dr. Grandhige stays in contact with surgeons who do that work well and will point you to one rather than sell you a smaller operation that will not hold.

The Four-Layer Neck Read
Formation runs a four-layer read on every neck consultation. You can do a rough version of it yourself in a mirror tonight. Each layer has its own solution, and picking the wrong one is how patients end up let down by an operation that went technically well.
Layer One: The Fat Under Your Chin
Tilt your chin down and pinch the tissue underneath. If you can hold a soft, mobile pad between your fingers, that is subcutaneous fat, and it is the layer liposuction owns. This pad is heavily genetic. Plenty of Formation patients are lean everywhere else and still carry it, which is why walking more and eating cleaner never moved it.
Dr. Grandhige removes this layer conservatively. Taking out too much is how a neck ends up hollow, irregular, and older than it started.
Layer Two: The Platysma Muscle And Its Bands
Tense your neck the way you would for a grimace. If two vertical cords stand up between your collarbone and your jaw, those are platysmal bands, and they are muscle. Fat removal does not touch them. Take the fat off a banded neck and the cords often become more visible, because the padding that was softening them is gone.
The answer at that layer is MyEllevate, a permanent suspension suture, usually paired with platysmaplasty to bring the muscle edges back together. That is a different operation from liposuction and it gets discussed as one.
Layer Three: Skin And The Fibroseptal Network
Pinch the skin under your jaw and let go. Skin that snaps back has recoil. Skin that takes a beat to return, or settles into a fold at rest, does not. Elasticity roughly tracks skin age 25 to 55, and it decides whether fat removal alone leaves you smooth.
When recoil is borderline, Dr. Grandhige tightens after the fat comes out using FaceTite, a radiofrequency-assisted device, or Renuvion, a helium plasma and radiofrequency device. Both work from underneath. They reach the deepest layer of the skin and the fibroseptal network beneath it, the connective tissue that anchors skin to muscle. Lasers and microneedling work from the top down and never get to that foundation.
Renuvion received 510(k) clearance in July 2022 for improving the appearance of loose skin in the neck and submental region. Read the clearance study before deciding it settles anything. Patients were followed for six months and results were graded from photographs by blinded reviewers, so it says nothing about how the tightening holds at year three.
Layer Four: Chin Projection And Jaw Angle
Look at your profile. A chin sitting behind the plane of your lower lip makes a neck look fuller than it is, because there is less bone for the tissue to drape over. No amount of fat removal creates a jaw angle you were not born with.
Where projection is the limit, structural filler does more than suction. Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed along the chin and jaw. That is the difference between a cleaner neck and something closer to a superhero jawline, and it is a separate conversation from the fat.
What Happens On Procedure Day
Neck cases run in the South Tampa office rather than a surgical facility. You take an oral sedative, the area is numbed with local anesthesia, and you stay awake. No general anesthesia, no breathing tube, no hospital day.
Incisions go under the chin and behind the ears, in creases where they fade. A thin cannula removes fat from the pad under the chin and along the jawline, and the natural angles of the jaw are worked around rather than through. Restraint is the whole technique here. Over-resection is what produces contour irregularities and the drawn look that ages badly.
Once the fat is out, Renuvion or FaceTite tightens the skin down over the muscle and bone underneath, so the envelope follows the new contour instead of sitting loose over it.

Plan for someone to drive you home. The sedative rules out driving yourself, which matters more when the drive is back down to Riverview than when it is six blocks.
Your Recovery, Week By Week
First few days. Mild swelling, some bruising, and a tight feeling under the chin. You wear a chin compression garment.
One week. Most patients are back at work and back out socially. The garment stays on 12 to 24 hours a day through this stretch. Training resumes at one week.
Two to four weeks. Swelling keeps coming down and the fat removal becomes obvious. This is when patients start noticing it in photographs rather than only in the mirror.
Three months. Skin tightening shows. Everything that changes from here comes from the skin contracting, not from fat that already left.
Six to nine months. Final. The tightening finishes and what you see is what holds.
Recovery here is a clinical phase, not a waiting period. Dr. Grandhige holds every case to the 60/40 Recovery Standard: the operation is 60 percent of the result and post-operative care is the other 40. Sometimes that means waiting, sometimes it means sorting out weight or health first, and sometimes it means a different procedure.
