The Four-Layer Neck Read

Most necks show aging in more than one layer at once. That is the reason two people with the same complaint need different procedures. Formation uses a four-layer read to decide what actually needs treating.

Layer One, Submental Fat

This is the pad that sits between your skin and the platysma muscle under your chin. Pinch under your jaw. If you can grab a soft roll that moves independently, that is preplatysmal fat, and it is what liposuction removes. Fat cells taken out do not grow back. Layer Two, The Platysma And Platysmal Bands

Layer Three, Skin And The Fibroseptal Network

Under the skin sits the fibroseptal network, the connective scaffolding that anchors skin to the tissue beneath it. When that scaffolding loosens, skin stops retracting after fat comes out. FaceTite, a radiofrequency device, is 510(k) cleared and works from underneath the skin rather than across the surface. Renuvion, a helium plasma device, reaches the same depth. Lasers and microneedling treat from the top down and never get to the scaffolding at all. At Formation one of these follows the liposuction in the same appointment when the read calls for it.

Layer Four, Chin Projection And Jaw Angle

Some necks are not heavy. They are short. If the chin sits behind the plane of the lower lip, there is nowhere for the jawline to land, and no amount of fat removal creates a line that the skeleton does not support. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, add projection at the chin and along the jaw angle. This is the layer that decides whether someone ends up with a soft profile or a superhero jawline, and it gets missed constantly because it is the only layer that is not about removing anything.

Who This Procedure Fits

The candidates who do best have real submental fat, skin that still snaps back, and one layer to treat rather than three. Skin elasticity holds up reasonably well through roughly the mid twenties to mid fifties. Inside that window, skin usually retracts on its own once the fat is gone. Past it, retraction gets unreliable and skin tightening stops being optional.

Two honest limits. If your neck is mostly loose skin with little fat, liposuction takes away the volume holding that skin out and can leave things looking worse. And if the read finds fat below the platysma or a submandibular gland sitting low, that is beyond what suction and tightening reach. Formation refers those patients out rather than operating past the limit of the tool, and stays in touch with the surgeons who handle that work. For patients who need less than surgery, non-invasive neck and jawline contouring is a separate route. Dr. Grandhige turns patients away regularly. That is the point of paying for an assessment instead of a sales call.

How The Procedure Runs

The whole thing happens in the office. You take an oral anxiolytic, the area is numbed with tumescent local anesthesia, and two or three incisions go in under the chin and behind the ears, each small enough to disappear once healed. Fat comes out through microcannulas. Skin tightening follows in the same sitting if layer three needs it. Awake liposuction keeps you responsive throughout, which means positioning and symmetry get checked while you are sitting up rather than guessed at while you are flat.

The safety record for this approach is better documented than most pages let on. A 2018 case series of 987 neck liposuction patients treated under tumescent local anesthesia reported five cases of temporary marginal mandibular nerve weakness, one submandibular gland ptosis, and one arterial bleed, with no seromas, skin necrosis, or scarring. The limitation matters as much as the numbers. It is an uncontrolled series from three surgeons, so it reflects their technique rather than a field average. Temporary nerve weakness looks like a slightly uneven smile for a few weeks. Any surgeon who tells you the risk is zero is not reading the same literature.

Formation operates in a Quad A accredited facility with the same anesthesia provider and the same staff on every case, plus a physician assistant employed directly by the practice who assists on all surgeries. Office-based surgery in Florida got stricter this year. House Bill 1175 took effect July 1, 2026. It created section 553.884 of the Florida Statutes, which sets safety design standards for office surgery suites, and the implementing rules are due by January 1, 2027. If you are comparing offices around USF or Fowler Avenue that advertise in-office procedures, ask which are accredited and registered rather than simply open.

Recovery Week By Week

The first few days bring swelling, bruising, and a tight feeling under the jaw, managed with a chin compression garment. At one week most patients are back at work and back to social life, still wearing the garment 12 to 24 hours a day. Training also resumes at one week.

Between weeks two and four the swelling settles and the fat removal becomes visible. At three months the skin tightening shows. The final result lands somewhere between six and nine months, once contraction finishes.

The stretch nobody warns you about is days three through six. Swelling peaks, the garment feels worst, and plenty of patients hit a short window of wondering whether they made a mistake. It passes once the contour starts appearing. The before and after gallery shows where this lands.

Lymphatic treatment starts at the first post-operative visit and runs through the swelling phase. Formation keeps discounted rates with nearby hotels and Dr. Grandhige visits out-of-town patients in their rooms during the early days. A Temple Terrace patient needs none of that, which is the quiet advantage of living inside the county.

The Preparation Window Most Practices Skip

Surgery is sixty percent of the result. Active aftercare is the other forty. Formation treats the weeks on either side of the procedure as part of the procedure.

Health and diet get assessed before a date is set. Nutrition, supplements, and peptides are adjusted in the window immediately before and immediately after so the body has what it needs to build structure while it heals. Protein intake, inflammation control, and lymphatic flow are dialed in rather than left to whatever the patient was already doing. Dr. Grandhige follows patients for a full year, and nearly all of them stay through it.

The cannula is the same instrument in every office in Hillsborough County. What differs is the thinking around it.

Getting To Formation From Temple Terrace

Formation is at 1315 South Howard Avenue, Suite 102, in South Tampa. From Temple Terrace the run is I-275 south to Exit 42, which serves the Armenia and Howard couplet.

One detail worth knowing before you drive it. Armenia Avenue runs one way southbound and Howard Avenue runs one way northbound between Tampa Bay Boulevard and Swann Avenue, so you cannot turn south onto Howard from Kennedy Boulevard. Take Armenia south to Swann, turn east, and pick up Howard there. South of Swann the couplet ends and Howard returns to two-way traffic, which is where Suite 102 sits. Navigation apps route this correctly, but the first-time approach surprises people.

Temple Terrace is its own city, not a Tampa neighborhood. It is one of only three incorporated municipalities in Hillsborough County alongside Tampa and Plant City, incorporated in May 1925 and covering ZIP codes 33617 and 33637. That matters for a practical reason. A search for a surgeon near you pulls the USF and Busch Boulevard corridor, most of it medical spas rather than surgical practices. Accreditation and who holds the instrument is the difference.
