What Neck And Jawline Contouring Does
It is worth knowing what the procedure does not do. It does not lift a face. It does not fix a jaw that is set back skeletally. And it does not substitute for weight loss. It sharpens a border that fat and loose skin have blurred, which is why it changes how rested someone looks more than how thin they look. The Four-Layer Neck Read
Most necks show aging in more than one layer at once, and the reason so many people are unhappy after treatment is that only one layer got addressed. Dr. Grandhige reads every neck the same way before proposing anything. Four layers, each with its own answer.
Layer One: Fat Under The Chin
This is the layer most people can feel. Pinch under the chin. If there is a soft pad there that has not moved with dieting or training, that is submental fat, and it is the layer neck liposuction handles directly. Fat cells removed do not come back. This layer is also the one most heavily set by genetics, which is why a lean, fit person in Valrico can train five days a week at a gym on Lithia Pinecrest and still see fullness under the chin in every photo.
Layer Two: The Platysma And Platysmal Bands
Under the fat sits the platysma, a thin sheet of muscle spanning the neck. With age its front edges can separate and stand up as two vertical cords, called platysmal bands. Skin holds its shape because of the fibroseptal network, the connective tissue scaffold running between skin and muscle. Once fat is removed, skin has to shrink to the new contour. Whether it will depends on that scaffold. Renuvion, a helium plasma and radiofrequency device, and FaceTite, a radiofrequency device, both work by heating the deepest layer of the skin and the fibroseptal network beneath it. Both are 510(k)-cleared, which means cleared for use rather than approved as a drug. Lasers and microneedling work from the surface down and cannot reach that foundation, which is the whole reason a device that enters through a small incision does something a topical treatment cannot.
Layer Four: Chin Projection And Jaw Angle
The last layer is bone. A chin that sits back or a jaw angle that is soft creates a short-looking neck no matter how much fat comes out. This is the frame, and the frame is what people are describing when they say someone has a superhero jawline. Structural jawline filler addresses it: Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed at the chin point and along the angle. Fat removal without frame support is the most common reason a technically clean result still looks flat in profile.
Who Is A Candidate, And Who Is Not
Skin elasticity matters more than birthday. The window where skin reliably shrinks to a new contour runs roughly from skin age 25 to 55. Sun exposure, smoking history and past weight swings move that number in either direction, which is why the assessment happens in person rather than from a photograph.
Good candidates are close to a stable weight, bothered by fullness under the chin or a soft jaw edge, and want refinement rather than a dramatic change. The second is anyone whose skin has passed the point where tightening can reach it. For patients in that second group, the honest answer is skin excision, and Formation stays in contact with the surgeons who do that work well and points patients to the right one rather than talking them into something smaller that will disappoint.
How Dr. Grandhige Approaches The Neck
Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He is not a plastic surgeon, and he says so plainly. He attended medical school at the University of Michigan, matched into the combined plastic surgery residency at Yale, then changed course and completed a full general surgery residency followed by fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. He has been operating for more than 15 years, and his certification can be checked directly through the American Board of Surgery.
That path matters here specifically. Burn and critical care training is training in how tissue behaves when it is injured and how it heals afterward. The neck is unforgiving in exactly that way. Take too much fat and the skin drapes over irregularities that were not there before. Take too little and nothing changes. His stated rule is to do only what is necessary and to do it exceptionally well, and in the neck that usually means less than patients expect.
He evaluates every patient himself and designs the plan himself. He also limits how many procedures he performs, which is what makes the follow-up schedule below possible.
What Recovery Actually Looks Like
The published timeline for this procedure at Formation is specific rather than approximate.
First few days: mild swelling, bruising and tightness. A chin compression garment goes on.
One week: back to work and social activities, still wearing the chin garment 12 to 24 hours a day. This is also when training resumes.
Two to four weeks: swelling continues to settle and the results of fat removal become visible.
Three months: skin tightening is visible and the jawline is clearly improved.
Six to nine months: final result, once skin tightening has finished.
Marketing copy across this category tends to stop at “minimal downtime.” The honest version is that the visible part is short and the finishing part is long, and anyone planning around a wedding or a reunion should count backward from the three-month mark rather than the one-week mark.
The Preparation Window Most Practices Skip
Formation treats the surgery as 60 percent of the result and the aftercare as 40 percent. That number is a working standard, not a slogan, and it sits at the center of Formation’s philosophy.
Health and diet are assessed before treatment, and nutrition, supplements and peptides are adjusted in the window immediately before and immediately after so the body builds the most structure it can while the skin is contracting. Lymphatic treatment begins at the first post-operative visit. Dr. Grandhige then follows patients for a full year.
The instrument is identical at every practice that owns one. The thinking around it is not.
Planning The Drive From Valrico
Valrico sits roughly 20 miles east of the clinic on South Howard Avenue, and the practical route runs west on Brandon Boulevard or Bloomingdale Avenue to the Lee Roy Selmon Expressway, which is the direct connection between Brandon and downtown Tampa.
The Reversible Express Lanes changed schedule on July 13, 2026. The Tampa Hillsborough Expressway Authority eliminated the midday split and moved to a two-direction model after studying demand from Brandon, Riverview and southern Hillsborough drivers. The lanes now run westbound toward Tampa from 5 a.m. to noon Monday through Friday, an hour earlier than before, and eastbound from noon to 5 a.m.
That has a real effect on how a Valrico patient should book. A morning appointment means the express lanes are open in your direction on the way in. An afternoon appointment means they have flipped and are open in your direction on the way home. Booking across noon means driving the surface route at least once. Post-operative visits stack up in the first month because lymphatic treatment starts early, so the difference compounds.
For patients who would rather not make the drive at all in the first days, Formation has standing relationships with nearby hotels, and Dr. Grandhige visits patients in their room during the early recovery window rather than asking them to come to the office.
Where The Procedure Happens
Neck contouring with an oral sedative and local anesthesia is performed in the office. Cases that call for general anesthesia are done in a Quad A accredited facility, with the same anesthesia provider and the same staff on every case, plus a physician assistant Dr. Grandhige employs directly who assists on all of his surgeries.
Florida tightened the rules around this setting recently. House Bill 1175, signed in May 2026 and effective July 1, created section 553.884 of the Florida Statutes and directed the Florida Building Commission and the State Fire Marshal to write safety design standards for office surgery suites. The honest caveat is that the standards themselves are not due until January 1, 2027, so the statute currently sets the requirement rather than the specification. In the meantime, the checkable things are a physician office surgery registration and the accreditation status of the facility, and any patient can verify both before booking anywhere.
The Short Version For Valrico Patients
A neck that photographs heavier than you feel is usually a layered problem, and the layer nobody checked is the one that ruins the result. Read all four before anyone touches anything.
