The Four-Layer Neck Read
Before any tool gets chosen, the neck gets read in four layers. Each layer has its own cause and its own answer, and the plan is built from whichever layers are actually involved rather than from whichever device the practice happens to own.
Layer One, Fat Under The Chin
Submental fat is the layer most people can see and pinch. It is heavily genetic, it shows up in patients who are otherwise lean, and it resists diet and training because the body does not draw down fat by location on request.
Precise removal is the answer here. Dr. Grandhige treats it as sculpture rather than volume reduction, taking fat where it blurs the jaw and leaving it where removal would create a hollow or an irregular edge. Over-resection in this area does not read as a better result. It reads as premature aging, and it is difficult to reverse. The fat cells that come out do not come back, so the result holds as long as your weight stays stable. The same principles carry over from the practice’s work in neck liposuction across Tampa.
Layer Three, Skin And The Fibroseptal Network
Skin quality decides how much of a result the first two layers can deliver. Beneath the skin sits the fibroseptal network, the connective tissue scaffold that anchors skin to the structures underneath. When that scaffold loosens, skin drapes instead of contracting.

Removing fat over separated bands makes them more visible, not less, which is why the muscle has to be assessed before anything is taken out.
Where bands are present, MyEllevate, a permanent suspension suture, can be placed with platysmaplasty to bring the muscle edges back toward the midline and hold them there. The suture is nonabsorbable and can be removed if it ever needs to be, and the lift is there the day it is placed. Booking against that schedule instead of ignoring it removes the worst of the drive on both ends.
One logistical point specific to this procedure. Because the neck is treated with an oral sedative and local anesthesia, you cannot drive yourself home. Riverview patients need someone to make the round trip with them, which is a different calculation than it is for a patient living two miles away.
