The Four-Layer Neck Read
Formation screens every neck and jawline patient against four layers. Most necks show aging in more than one layer at once, and using the wrong answer for the layer you actually have is how people spend a year on treatments and still dislike their profile.
Layer One, Fat Below The Chin
This is the layer most people assume they have, and it is the one treated first by default across the Brandon market. A pocket of fat below the chin sits in front of the muscle and hides the jaw behind it. It is heavily genetic. Plenty of patients carry it at a normal weight and have carried it since their twenties.
Fat responds to direct removal. Neck liposuction at Formation is performed through very small incisions under the chin and behind the ears, usually with an oral sedative and local anesthesia rather than general anesthesia. Fat cells removed do not return. Over-resection is the classic error here, because a neck stripped too aggressively looks hollow and ages faster, so restraint matters more than volume.
Layer Two, The Platysma And Its Bands
The platysma is a thin sheet of muscle across the front of the neck. When it loosens and separates, its edges show as vertical cords running from the jaw toward the collarbone. Platysmal bands drag a jawline down visually even when almost no fat is present, and fat-focused treatment plans miss this layer completely.
Two answers apply, and they are not interchangeable. For bands driven by muscle activity, Botox, onabotulinumtoxinA, has carried an FDA approval for moderate to severe platysma bands since October 18, 2024, dosed at 26, 31, or 36 units by severity. The published limitation belongs beside that figure. Across the two phase three trials, 65 percent and 62 percent of treated patients reported being satisfied or very satisfied with their neck and jawline at two weeks, against 12 percent on placebo, and the effect is temporary. It relaxes a muscle. It removes no fat and tightens no skin.
For bands that are structural rather than movement-driven, MyEllevate, a permanent suspension suture, paired with platysmaplasty repositions the muscle itself. The suture is nonabsorbable and can be removed, which separates it from barbed threads that dissolve, and the lift is present the day it is placed.
Layer Three, Skin And The Fibroseptal Network
Skin is the layer that decides whether removing fat helps or backfires. Beneath the surface sits the fibroseptal network, the connective tissue scaffold that anchors skin to the structures underneath. When that scaffold slackens, skin stops conforming to whatever shape you create below it.
FaceTite, a radiofrequency-assisted device cleared through the FDA 510(k) pathway, works from underneath. It reaches the deepest layer of the skin and the fibroseptal network beneath it. Lasers and microneedling work from the top down and cannot get to that foundation, which is the whole reason a surface treatment can feel like it is doing nothing for a slack neck. Collagen response follows, but depth of access is the actual difference. Renuvion, a helium plasma device, is used the same way. Skin roughly between the ages of 25 and 55 tends to respond best.
Layer Four, Chin Projection And Jaw Angle
The fourth layer is not soft tissue at all. A recessed chin or an obtuse jaw angle produces a soft profile no matter how much fat is removed, because there is no frame for the tissue to drape over. Patients often arrive convinced they need fat gone when their measurements say they need projection.
Structural fillers build that frame. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, are placed along the chin and jaw angle to give the soft tissue somewhere to sit. A modest amount of projection at the chin is frequently what turns a decent result into a superhero jawline, and it is the cheapest thing to test first because hyaluronic acid is reversible.
Why Sessions Stack Up And Nothing Changes
The Brandon and Riverview corridor is well served by injectable and device-based clinics, and their menus lean toward fat reduction and surface tightening. That default is right for a genuine layer-one patient. It goes sideways for everyone else.
Here is the pattern worth naming. A patient with a layer-two or layer-four problem gets treated for layer one, sees a small change, and books another session. Then another modality gets added. Sessions accumulate, downtime accumulates, and eighteen months later the underlying cause has never been addressed. A thread lift, a filler syringe, and a fat-dissolving series can each be appropriate and still add up to the wrong plan when the sequence was never diagnosed.
Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He is not a plastic surgeon and is direct about that distinction. He trained at the University of Michigan, matched initially into the combined plastic surgery residency at Yale, then completed a full general surgery residency with fellowship training in burn surgery, critical care, and minimally invasive surgery. He evaluates every patient personally and designs each plan himself.
Formation caps surgical volume so that happens. The same anesthesia provider works every case, the same operating room staff, and Dr. Grandhige’s own physician assistant is present for every procedure. Surgeries take place in Quad A accredited facilities.
Driving In From Brandon
Brandon patients have a routing detail worth knowing before an early appointment. The Lee Roy Selmon Expressway carries Reversible Express Lanes between Brandon and downtown Tampa, and THEA moved the westbound opening an hour earlier effective July 13, 2026. Westbound now runs 5 a.m. to noon Monday through Friday, and the lanes flip eastbound from noon to 5 a.m.
The useful part is counterintuitive. The express lanes exit at Meridian Avenue downtown, which means they drop you northeast of the office and leave you doubling back through downtown to reach South Howard. For a morning appointment, the mainline lanes and Exit 4 at Willow Avenue land you far closer. Save the express lanes for the ride home, when they run eastbound from noon and your driver is taking you back to Brandon Parkway.
That matters more than it sounds. Neck procedures here are done under oral sedation, so you are not driving yourself home, and whoever brings you is making the return trip in the afternoon.
Recovery And The Preparation Most Practices Skip
The neck timeline is its own thing and does not match body contouring recovery. Expect mild swelling, bruising, and tightness for the first few days, with a chin compression garment on.
At one week most patients are back at work and back to training, with the garment worn 12 to 24 hours a day. Between two and four weeks swelling settles and the results of fat removal become visible. Skin tightening shows around three months. The final result lands between six and nine months as tightening completes.
Recovery is not a waiting period. Surgery accounts for roughly 60 percent of a result and active aftercare accounts for the other 40, which is why Formation owns the post-operative phase rather than handing it off. Every patient is followed for a full year.
Health and diet are assessed before treatment, and nutrition, supplements and peptides are adjusted in the window immediately before and after so the body builds as much structure as it can while it heals. The instrument is identical at every practice that owns one. The thinking around it is not.
Who This Is Not For
Honest candidacy screening means saying no. A neck with heavy skin excess, deep vertical creases, or laxity beyond what tightening can reach needs skin removed, not tightened. Formation refers those patients outward and stays in contact with the surgeons who do that work well.
Rapid weight loss deserves its own note, because Brandon has seen the same GLP-1 wave as everywhere else. Published work describes skin losing its ability to retract when weight comes off quickly, with sagging most visible in the face and neck. These patients are usually deflated and lax rather than fatty, so removing more volume makes them look worse. Formation applies a weight-stability window and waits for weight to hold steady before contouring anything.
What Results Look Like And How Long They Hold
Male and female necks are not read the same way. A masculine jawline needs clean angles, a flat neck, and a preserved jaw-to-neck transition, and even small over-corrections here read as feminized or obviously operated. Formation does a high proportion of male contouring, and the technical approach differs enough that treating a male neck with a female template is its own category of error.
Results from the fat layer are permanent, since removed fat cells do not regenerate, provided weight stays stable. Skin tightening holds for years but continues to age alongside the rest of you. Filler at the chin and jaw angle is the shortest-lived of the four and needs maintenance. Muscle relaxation is measured in months, not years. Knowing which parts of your result are permanent and which are maintenance is part of deciding whether the plan is worth starting.
What patients report afterward is rarely dramatic. Colleagues say they look rested. Photographs stop being something to avoid. One Formation patient, a professional in his forties, had carried fullness under his chin his entire adult life despite staying fit, and described the change afterward as his face finally matching the rest of his body.
