What Neck Liposuction Does And What It Leaves Alone
This is a fat procedure. Small, discreet incisions go under the chin and behind the ears. Fat is removed with a fine cannula to sharpen the jawline while the natural jaw angles are left intact. Most cases run under an hour.
Dr. Gopal Grandhige is a board-certified general surgeon specializing in aesthetic surgery, certified by the American Board of Surgery. He trained at the University of Michigan Medical School, matched into the combined plastic surgery residency at Yale, then completed a full general surgery residency with fellowship training in burn surgery and critical care. That background matters here because the submental region sits close to the marginal mandibular nerve and the submandibular gland, and because how skin behaves after fat is removed is a healing question, not a technique question.
What it leaves alone is everything that is not fat. Loose skin stays loose. Muscle bands stay banded. A recessed chin stays recessed. Removing fat from a neck that has other problems can make those other problems more visible, which is why the read matters more than the instrument.
The Four-Layer Neck Read
Most necks show aging in more than one layer at once. Grouping the complaint into four layers is how Dr. Grandhige decides what to do and, more often, what to leave alone. Each layer has its own answer.
Layer One, Fat Under The Chin
Pinch the area under your chin with your head level. If you can gather a soft pad between two fingers, that is submental fat. This is the layer liposuction treats, and fat cells removed here do not come back as long as weight stays stable. Lean patients who still carry this pad are usually carrying it for genetic reasons, which is why it survives every diet they have tried.
Layer Two, The Platysma And Platysmal Bands
Tighten your neck as if you were grimacing. If two vertical cords stand up from the collarbone toward the jaw, those are platysmal bands, and no amount of fat removal will flatten them. This layer needs the muscle addressed. MyEllevate, a permanent suspension suture placed with platysmaplasty, is the answer when bands are the driver. Naming this layer early is what keeps a patient from booking a fat procedure that was never going to fix their complaint.
Layer Three, Skin And The Fibroseptal Network
Skin quality decides whether fat removal reads as sharp or as slack. Under the skin sits the fibroseptal network, the connective tissue scaffold that anchors skin to the layers beneath it. Renuvion, a helium plasma and radiofrequency device, and FaceTite, a radiofrequency-assisted contouring device, both reach the deepest layer of the skin and that scaffold underneath. Both are 510(k)-cleared. Lasers and microneedling work from the surface down and cannot get to that depth. They are a different category of tool, not a cheaper version of the same one. Layer Four, Chin Projection And Jaw Angle
Some necks are not fat problems at all. They are frame problems. A chin that sits behind the lip line shortens the neck visually no matter how clean the fat removal is. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, are used to build projection at the chin and along the jaw angle when the bone is what is missing. This is the layer people are actually chasing when they say they want a superhero jawline.
Who This Works For, And Who Should Wait
The candidate profile is narrow on purpose. Skin recoil in the roughly 25 to 55 range holds up well after fat is removed. Reasonable weight stability matters more than a specific number on the scale. And the complaint has to be mostly the fat pad, not mostly the skin or the muscle.
Here is where the answer is no, or not yet. Heavy skin laxity with poor recoil will not tighten to a sharp line, and forcing it produces a hollow, aged look rather than a defined one. Prominent platysmal bands need the muscle handled first. Patients who have dropped weight quickly, including on GLP-1 medications, often arrive with more laxity than fat. For them the honest answer may be tightening alone, waiting until weight holds steady, or a skin-excision procedure that neither liposuction nor energy tightening substitutes for. Formation stays in contact with the surgeons who do that excisional work well and points patients to the right one.

Getting To South Howard From Maryland Manor

The neighborhood itself is useful during recovery. A third of a square mile of quiet residential grid means a full walking loop without crossing a major arterial, and early walking is encouraged. The first few days bring mild swelling, some bruising, and a tight feeling under the chin, managed with a chin compression garment. By one week, most patients are back at work and back to training, still wearing the garment 12 to 24 hours a day. Between two and four weeks the swelling settles and the fat removal becomes clearly visible. At three months the skin tightening is visible and the jawline improvement is substantial. The final result lands somewhere between six and nine months, once the tightening has fully matured.
Recovery is not a waiting period. Formation runs on a 60/40 Recovery Standard: the surgery accounts for about 60 percent of the result and post-operative care accounts for the other 40 percent. Poor aftercare can undo a well-executed procedure, so the practice keeps aftercare in house rather than handing patients an instruction sheet. Nutrition, supplements, and peptides are then adjusted in the window immediately before and after treatment so the body builds as much structure as it can while it is healing. Protein intake, inflammation control, and lymphatic flow all get dialed in rather than left to chance, and lymphatic treatment begins at the first post-operative visit.
The cannula is the same cannula everywhere. The tightening device is the same device everywhere. The thinking around both is not.
What The Published Numbers Say, And What They Do Not
Liposuction has a well-documented safety profile. A 2024 systematic review and meta-analysis in Aesthetic Surgery Journal pooled 29,368 patients and put the overall complication rate at 2.62 percent, with contour deformity the most common at 2.35 percent and infection, skin necrosis, and venous thromboembolism all well under one percent.
The limitation belongs next to the number. That analysis pools observational studies across all body areas, not submental cases specifically, and the authors note that outcomes are shaped by patient characteristics, operative technique, and how closely patients follow post-operative instructions. A separate 2024 meta-analysis of 21,776 patients reported a pooled complication rate of 12 percent. The gap between those two figures is not a contradiction about safety; it reflects how differently studies define and report a complication. Any page quoting one number without that context is quoting a number it does not understand.
One more piece of 2026 context matters when a procedure happens in an office rather than a hospital. Florida House Bill 1175 took effect on July 1, 2026. It adds a section to the Florida Building Code Act covering office surgery suites, and design and fire safety standards are due from the Florida Building Commission and the State Fire Marshal by January 1, 2027. The practical question for a patient is simple. Ask where the procedure happens, what the room is equipped for, and what accreditation covers it.

The people who do worst were sold a fat procedure for a skin or muscle problem. Getting that read right, before anything is scheduled, is the job.
