The Three-Zone Jawline Map
A blurred jawline isn’t one problem. Dr. Grandhige reads every jaw in three zones before anything gets chosen, because a fix aimed at the wrong zone does nothing at best.
Zone One: The Mandibular Body
The line running from the chin back along the lower border. When it goes soft, the cause is usually under-projected bone or an early dip just in front of the jowl. Gel placed on or just above the periosteum rebuilds that border directly. Zone one is where filler does its cleanest work, and where you’ll see the change before you stand up.
Zone Two: The Gonial Angle
The back corner, where the jaw turns up toward the ear. Two opposite problems live here. If the corner is flat, structural gel adds width and a defined turn. If it’s already wide because the masseter is overdeveloped, usually from clenching or grinding, more volume makes the whole face read square and heavy. That jaw needs the muscle softened, not built, which is what jaw slimming Botox is for. Botox, a botulinum toxin, takes a few weeks to show its effect there, so it goes first and anything structural waits.
Zone Three: Chin Projection
A jaw that stops short at the front never reads sharp, no matter how well the sides are built. Chin projection sets the geometry for the whole lower face, and it’s the zone most often left out of a plan. Two materials get used here. Juvederm, a hyaluronic acid, dissolves on demand with hyaluronidase if the shape isn’t right. Radiesse, a calcium hydroxylapatite, holds longer and doesn’t reverse, and the FDA notes that calcium hydroxylapatite stays visible on x-rays and may obscure underlying features, which matters if you’ve got dental or facial imaging in your future. The agency’s page on approved uses and risks for dermal fillers also sets the age floor at 22 and older. For a first chin, reversibility usually wins the argument.
The Formation Add-Or-Subtract Rule
The map reads the anatomy. This rule picks the tool. One question comes before any product: does this jaw need something added, or something taken away?
Added means the bone falls short. Flat border, soft corner, chin sitting back, nothing in the way. Filler is the direct answer and the result is same-day.
Subtracted means something is sitting between you and the line you want. A real fat pocket under the chin. Skin that’s begun to descend. A masseter carrying extra width. No injectable removes fat, and volume doesn’t lift skin. A submental pocket needs a small surgical step, which Dr. Grandhige performs himself, so patients in that group usually end up discussing neck liposuction instead of a syringe.
Plenty of jaws answer yes to both. Those get staged, not stacked. Subtract first, add second. Build projection onto a jaw that still carries a pad underneath and you’ve made the lower face heavier, not sharper. When the skin and underlying muscle are past what tightening or contouring can reach, we say so and point you toward a surgeon who does that work well. Why Every Syringe Goes In Your Chart
Filler stops being visible long before it stops being present, and almost nobody counts what’s already in there.
The research is worth knowing before your first placement. A 2024 controlled cohort study in the International Journal of Surgery compared CT scans taken before and after chin injection and concluded that hyaluronic acid may cause bone resorption at the mentum, that patients should be told about the possibility, and that a large randomized trial is still needed to confirm it. That last clause is the honest limit on the finding, and any page quoting it without that is overselling. Earlier CT work pointed the same direction and added the more useful detail: patients injected with a millilitre or more per session were more susceptible to bone erosion than those injected with less, while the number of sessions and the gap between them made no real difference. Volume per sitting is the variable, not how often you come in.
So we keep a running filler record in every chart. How much, where, which product, what date, including anything placed elsewhere before you got here. Three things follow. A ceiling on total jaw volume gets set before the first placement rather than after the third. A planned first volume goes in, then we look again at two weeks instead of filling to the finish line in one sitting. And when the record says a jaw is carrying too much, we dissolve and rebuild instead of adding another layer.
If a provider can’t tell you your running total, they aren’t managing your jaw. They’re refilling it.
Preparation And Recovery Around The Syringe
Your health and diet get assessed before treatment, not after something goes sideways. Nutrition, supplements and peptides are adjusted in the window right before and after, so the body builds as much of its own structure as it can around what’s placed and settles with less swelling. Blood thinners, fish oil, alcohol and hard training get dialed in ahead of the date rather than mentioned on your way out the door.

Stay hydrated, stay out of direct midday sun, and skip alcohol and hard training for several days. If you’ve got a wedding, a shoot or a flight coming, leave yourself a 72-hour buffer.
On longevity, be skeptical of the big numbers. Hyaluronic acid materials run roughly 6 to 12 months depending on the product and the area. In the pivotal trial behind the only hyaluronic acid gel carrying a jawline-specific approval, about 70 percent of treated participants met the primary endpoint for improved jawline definition at six months, and improvement was tracked through one year. One year, against an untreated control group, with only the evaluator blinded. Pages promising 18 to 24 months on a jaw are quoting a brochure.
