One thing worth setting straight early. Getting rid of this fat through dieting alone usually costs you muscle, and the weight comes back. That cycle is the reason most Town ‘n’ Country patients end up in a consultation in the first place. The point of surgery here is to let you go back to building instead of restricting.
The Formation Tighten-Or-Remove Line
Before Dr. Grandhige discusses technique, he runs one test. It has three checks, and it decides whether your problem is fat or skin. Get this wrong and no amount of good liposuction fixes it.
Check one, recoil. Pinch the skin and let go. Skin that returns toward the body will redrape over a new contour. Skin that stays creased won’t.
Check two, the standing fold. Stand up straight and look for skin that hangs at rest, with nothing pulling it. A fold that’s there without help is loose skin, and loose skin has to come out, not be thinned underneath.
Check three, the abdominal wall. Press in. If the muscles have separated after pregnancy or a large weight change, the wall itself is what’s pushing outward. Liposuction can’t reach that.
Clear all three and you’re a liposuction patient. Fail check two or three and the correct operation is abdominoplasty, which removes skin and repairs the wall. Dr. Grandhige performs that himself, along with 360 contouring, lower body lift, and revision work, so nobody gets handed off for it.
He’ll also tell you when the answer is to wait or to fix something else first. Sometimes surgery isn’t offered at all. Patients still leave with a plan.
The Published Risk Data On High-Definition Liposuction
Most pages ranking for this procedure list risks without ever putting a number on them. The largest review of high-definition work specifically, published in Plastic and Reconstructive Surgery in July 2023, pooled 21 studies covering 6,964 patients.
The overall complication rate came out at 14.4 percent. Complications classed as major accounted for 0.2 percent. The most common single issue was transient hyperpigmentation at 3.8 percent, followed by seroma at 3.3 percent and temporary soft-tissue firmness at 2.7 percent. Average patient age was 38, average BMI was 25, and of the patients whose sex was reported, 56 percent were men.
Read those two numbers together. Roughly one patient in seven has something happen, and nearly all of it is discoloration, fluid, or firmness that resolves. The serious column is very small.
The review also reported satisfaction near 93 percent, and here’s the limitation the authors state plainly: satisfaction was only reported for about 1,497 of those 6,964 patients, because reporting across the studies wasn’t uniform. So treat the satisfaction figure as directional and the complication figures as solid.
The risk Dr. Grandhige actually worries about isn’t on that list. It’s contour irregularity, the dents and waviness that come from taking too much or misjudging how a particular person’s skin will settle. Over-resection produces irregular contours, added laxity, and results that age badly. It comes from judgment, not from bad luck, and it’s the hardest thing to correct later.
The Florida Volume Limit That Shapes Your Plan
If you’re considering multiple zones at once, one rule matters more than any marketing claim on any clinic page, and no local page states it. Florida limits liposuction performed in an office setting to the removal of less than 4,000cc of supernatant fat. Any case removing more than 1,000cc requires the office to be registered with the Department of Health, with annual state inspection unless the facility is accredited by a nationally recognized accrediting body. The Florida Board of Medicine publishes the requirement directly.
Two things follow from that. First, Formation operates in Quad A accredited surgical facilities, which is the accreditation route that satisfies the inspection standard. Second, an aggressive multi-zone plan can run into a real ceiling, and the ceiling exists because volume drives risk.

Three Months To One Year
Final contour keeps maturing across three to six months. Skin pliability and normal sensation return over roughly six to twelve months. Dr. Grandhige follows patients for the full year, and nearly all of them stay through it. Most come back for refinement planning rather than because something went wrong.
