Past that window, the threads pull against tissue that cannot hold the load, and the lift fades early while the barbs stay put a while longer.
Layer Four: Chin Projection And Jaw Angle
A short chin makes every other layer look worse than it is. Pull the jawline back and the same amount of fat and skin reads as a heavier lower face. Threads do not project bone. What The Published Data Shows About Thread Complications
A meta-analysis published in Frontiers in Surgery on March 31, 2026 pooled 26 studies and 2,827 thread lift patients. Swelling appeared in 34 percent of cases, bruising in 26 percent, and pain in 11 percent. Beyond four weeks, visible or palpable threads showed up in 10 percent, dimpling or asymmetry in 7 percent, altered sensation in 6 percent, thread exposure in 5 percent, and infection in 2 percent.
The authors’ own caveats carry as much weight as the numbers, so here they are. Heterogeneity ran high, reaching 92 percent for swelling and bruising, which means the studies disagreed with each other a great deal. The review did not separate results by thread material, so polydioxanone sits in the same pool as polycaprolactone, poly-L-lactic acid and permanent polypropylene. And the authors state directly that swelling, bruising and pain inside the first four weeks may be expected tissue responses to the passes rather than true complications, which means those early figures likely overstate the rate of anything clinically meaningful.
The division that should shape your expectations is early versus late. Swelling generally settled within two to four weeks across the reporting studies, and bruising within one to three. The events that arrive or persist past four weeks are the ones that need a hand: massage, repositioning, or removal of a thread.
The Three-Route Screen For A Softening Jawline
Route one is threads. They fit early laxity in intact skin, a patient who wants a change now, and a patient who accepts that this is maintenance rather than a fix. Route two is energy-based tightening. Where skin needs to shrink rather than move, a radiofrequency device works from beneath the skin and reaches depth that surface treatments cannot. Route three is referral. When the skin envelope has to come out, no thread and no energy device will get you there, and Formation says so and points you to the surgeons who do that work well.
One piece of accuracy belongs inside route one, stated once, because it belongs in your decision rather than in a sales pitch. PDO threads come to market as 510(k)-cleared absorbable sutures. Cleared, not approved, and the two words mean different things. Read the FDA clearance summary for the one polydioxanone family carrying a facial suspension indication and two details stand out. The cleared language describes temporary fixation. And the manufacturer’s own bench data put breaking strength at roughly half by six weeks, too degraded to measure at ten to twelve weeks, with the suture fully absorbed somewhere between 180 and 220 days.
Hold that against the 12 to 18 months you will read on nearly every thread lift page in the Brandon and Valrico search results. At month twelve the threads are gone. Whatever lift you still have is your own collagen response to them, which is real, and which is smaller than what you saw on day one. Anyone quoting you a year and a half of hold is quoting the corpus, not the clearance file.
Threads are the wrong tool if a fold of skin hangs at rest with no clenching involved, if platysmal bands are your main complaint, if you sit past the elasticity window, or if you want to do this once and never again. There is one more case worth naming. Whether thread tracks leave enough scarring to complicate a facelift years later is not settled in the published literature, and clinicians argue both sides in good faith. If surgery is on your five year horizon, Dr. Grandhige raises that himself rather than waiting to see whether you ask.
Preparation And Recovery At Formation
Health and diet get assessed before anything is placed. Nutrition, supplements and peptides are adjusted in the window immediately before and after, so the tissue builds as much structure as it is capable of building while the threads are still doing mechanical work. Lymphatic support starts early rather than after swelling has settled in. Follow-up runs a full year, and nearly every patient completes it.
Formation treats the procedure as 60 percent of the result and the recovery as the other 40. Return to training is one week. That figure is not a marketing number, it is the standard applied across the practice, and it holds because the recovery is managed rather than left to the patient to guess at.
