What Neck Liposuction Removes, And What It Leaves Alone
Neck liposuction takes out the fat sitting between your skin and the muscle beneath your chin. That’s it. It doesn’t tighten muscle, it doesn’t cut away skin, and it doesn’t build out a small chin.
That narrow job description is the reason the procedure works so well on the right person and disappoints the wrong one. Fat under the chin is heavily genetic. Plenty of people who train hard and eat carefully still carry it, because the body decides where it stores fat and the submental area is one of the last places to give it up. You can lose weight everywhere else and watch that area stay exactly the same.
Remove the fat and the jawline that was already there shows up. People notice something changed but usually can’t name it. They’ll say you look rested. Dr. Gopal Grandhige has performed neck liposuction and jawline contouring as one of his most frequent procedures for years, and he treats it as sculpture rather than removal. The fat that comes out is chosen. The fat that stays is chosen too.
The Four-Layer Neck Read
Most necks show aging in more than one layer at once. That’s the part the average consultation skips, and it’s why some people get liposuction and feel let down.
Formation reads every neck across four layers before anyone commits to anything. Each layer has its own solution. Liposuction only answers one of them.
Layer One: Fat
Fat is what liposuction treats. If you pinch under your chin and get a soft roll that moves, and your skin snaps back when you let go, layer one is your problem and liposuction is your answer. This is the cleanest case in the whole category and it’s the one that produces the results people share.
Layer Two: The Platysma And Platysmal Bands
The platysma is the thin sheet of muscle running across your neck. When its edges separate and loosen, you get vertical cords that show up when you talk or tense your jaw. Those are platysmal bands, and no amount of fat removal touches them. Removing the fat in front of them can actually make them easier to see.
When bands are present, the solution is MyEllevate, a permanent suspension suture, combined with platysmaplasty to bring the muscle edges back together. Dr. Grandhige will tell you at the consultation whether you have bands, because finding out afterward is a bad experience.
Layer Three: Skin And The Fibroseptal Network
Under your skin sits a scaffold of connective tissue called the fibroseptal network. It’s what holds skin against the structures below. When it loosens, skin stops sitting tight over the jaw no matter how much fat you take out.
Renuvion, a helium plasma and radiofrequency device, treats the deepest layer of the skin and the fibroseptal network beneath it. FaceTite, a radiofrequency-assisted contouring device, works the same territory. Lasers and microneedling work from the top down and can’t reach that foundation. They’re doing something different, at a different depth, and comparing them is a category error.
Layer Four: Chin Projection And Jaw Angle
Some people don’t have a fat problem. They have a frame problem. A chin that sits back relative to the lips will read as a weak jawline even at low body fat, because there’s no bone edge for the skin to drape over.
That’s a filler question, not a surgical one. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, both build projection at the chin and along the jaw angle. If you want the superhero jawline you saw on someone else, layer four is usually the layer you’re actually looking at, and taking fat out won’t get you there.
What Happens On Procedure Day
You’re awake. This is the detail that separates Formation’s neck procedure from most of what you’ll read about, and it changes the whole day.
Neck liposuction and jawline contouring at Formation is performed in the office with an oral anxiety medication and local anesthesia. No general anesthesia. No ambulatory surgery center across town. No breathing tube, no recovery bay, no anesthesiologist bill folded into a facility schedule. Dr. Grandhige’s larger cases go to accredited Quad A surgical facilities, but this one doesn’t need to.
The incisions are very small and sit under the chin and behind the ears, where they disappear. Fat comes out with attention to what stays behind, because over-aggressive removal under the chin creates surface irregularities and can age the area faster than doing nothing. Dr. Grandhige’s position on this is firm: restraint produces better contours at year five than aggression produces at week six.
Then the skin gets treated. Renuvion or FaceTite is used to tighten the skin down over the muscle and bone underneath, so the neck doesn’t just have less fat in it, it has skin that sits where the fat used to be. The FDA cleared the Renuvion handpiece in July 2022 for improving the appearance of loose skin in the neck and under the chin, and again in April 2023 for treating soft tissue after liposuction, as documented in the FDA’s device communication. That’s 510(k) clearance, which is not the same thing as FDA approval, and any practice telling you otherwise is being loose with language.
Who This Works For, And Who Should Not Have It
The candidate profile is narrow on purpose.
You’re a good candidate if you’re at a stable weight, you’re bothered by fullness under the chin or a jawline you can’t find, and your skin still has recoil. Skin age roughly 25 to 55 is the working window at Formation. Inside that range, skin retracts over the treated area reliably. Outside it, the answer becomes more conditional.
You are not a good candidate if the main issue is hanging skin rather than fat. Liposuction has nothing to offer loose skin, and energy tightening has a ceiling. When someone’s neck is past what tightening or contouring can reach, Formation refers outward rather than talking them into a smaller procedure that won’t satisfy them. Dr. Grandhige stays in contact with the surgeons who do skin-excision neck work well and points patients to the right one. Being told no at a consultation is a service, not a rejection.
Smokers, patients with uncontrolled diabetes, and anyone with a condition that slows wound healing get a different conversation. So does anyone whose expectation is that a neck procedure will read as weight loss. It won’t.
Recovery, Week By Week
Here’s what the calendar actually looks like.
- First few days. Mild swelling, some bruising, and a tight feeling under the chin. A chin compression garment goes on and stays on.
- One week. Most patients are back at work and back to social plans. The garment is worn 12 to 24 hours a day through this stretch. Return to training is one week.
- Two to four weeks. Swelling keeps dropping and the liposuction result becomes visible. This is the point where people start noticing their profile in photos again.
- Three months. Skin tightening shows up. The jawline improvement people came for is clearly there.
- Six to nine months. Final result. Tightening has finished doing what it’s going to do.
The gap between month one and month three catches people off guard, which is why Formation puts the timeline in front of you before you schedule. If you have a wedding, a reunion, or a photographed work event, you plan backward from it.
What Formation Does Before And After That Most Practices Skip
The cannula is the same at every practice in Tampa. The thinking around it isn’t.
Before your procedure, your health and diet get assessed. Not a form you fill out at the front desk, an actual look at what you’re eating, how you’re sleeping, and what your inflammation looks like going in. Nutrition, supplements and peptides are then adjusted in the window immediately before and immediately after, so your body builds the most structure it can while it’s healing. Poor aftercare can undo good surgery, which is why Formation owns that phase instead of handing you a printout and a follow-up date. Results And How Long They Hold
Fat cells removed during liposuction don’t come back. As long as your weight stays stable, the contour holds.
What you can expect is a sharper jawline, a cleaner line between face and neck, and a profile that photographs the way you think you look. Removing fat in this area can reset the aging clock somewhat and slow how quickly laxity shows up later.
One Formation patient, a man in his forties who was otherwise fit, had a neck that never responded to weight loss and had hated the way he photographed his entire adult life. For context on volume, the American Society of Plastic Surgeons counted 24,000 submental and chin liposuction procedures among its member surgeons in 2024, against 22,445 neck lifts, per its 2024 procedural statistics. Those counts cover ASPS members only, so the real national numbers are higher. The useful signal is that the two procedures run at similar volume, which tells you plenty of people are choosing the smaller operation and getting what they wanted.
The Risk Numbers Most Neck Pages Leave Out
Every clinic page says complications are rare. Almost none of them tell you which ones.
A 2024 systematic review and meta-analysis in the Aesthetic Surgery Journal put the absolute risk of complications after aesthetic liposuction at roughly 2.62 percent. The single most common problem wasn’t infection or clotting. It was contour deformity, at about 2.35 percent prevalence, followed by hyperpigmentation at 1.49 percent, seroma at 0.65 percent, and hematoma at 0.27 percent.
The published limitation matters here: that analysis pools liposuction across the whole body and doesn’t isolate submental cases, so the neck-specific rate could run higher or lower. What it does establish is where the risk concentrates. The most likely thing to go wrong is a surface irregularity, and surface irregularities come from taking too much. That’s the argument for restraint, made with numbers instead of adjectives.
Neck Liposuction For North Tampa Patients
Formation sits on South Howard Avenue, which puts most North Tampa addresses 20 to 30 minutes away down I-275, traffic permitting. Forest Hills and Lowry Park are closer to 20. Tampa Palms and the far end of New Tampa run closer to 30.
That drive matters more than it does for a South Tampa patient, and it’s worth being direct about how it plays out. Because this procedure is office-based, procedure day is one trip. You come down, you have it done, you go home. There’s no separate surgical facility to drive to, no pre-op appointment at a second address, no overnight stay. You’ll need someone to drive you home because of the oral sedative, so plan one person for one half-day.
The return trips are where North Tampa patients should plan ahead. Your first post-operative visit starts lymphatic treatment, and the follow-up schedule runs through the full year. That’s several drives down the same corridor. Patients who book morning appointments consistently report a better experience with the I-275 stretch than afternoon ones.

Different depth and different mechanism. Lasers and microneedling work from the skin surface downward. Renuvion and FaceTite work beneath the skin, at the fibroseptal network. Liposuction physically removes fat rather than breaking it down over multiple sessions.
Roughly 20 to 30 minutes down I-275 depending on where you start. Procedure day is a single trip with a driver. After that you'll return for your first post-operative visit and lymphatic treatment, then follow-ups across the first year.
