What Neck Liposuction Does, And What It Cannot Do
Neck liposuction, also called submental liposuction, takes out fat. That’s the whole job. It does not tighten skin that has stopped snapping back, and it does not touch the muscle underneath.
Most people arrive assuming their neck has one problem. It usually has two or three at once, mixed in a ratio that decides the entire plan. Fat is the easiest to see and the easiest to fix. Skin quality and muscle position are harder to read in a mirror and matter more to the result.
Get the ratio wrong and you land in one of two places. Either fat is left behind, or fat is taken from a neck whose skin had nothing left to shrink with. The Four-Layer Neck Read
Dr. Grandhige reads four things in a fixed order at every neck consultation. Most necks show aging in more than one layer at once, so knowing the order tells you what he’s actually looking at while you’re in the chair. Each layer has its own answer.
Layer One: Fat
How much fat sits above the platysma muscle, and how much sits below it. Fat below the muscle behaves differently and changes the technique. This layer is what liposuction addresses, and when it’s the dominant issue, liposuction alone usually does the job.
Layer Two: The Platysma Muscle And Platysmal Bands
Vertical cords that appear when you tense your neck. Liposuction does nothing for them. When platysmal bands are the main complaint, the answer is MyEllevate, a permanent suspension suture, paired with platysmaplasty to bring the muscle edges back together. Unlike barbed threads that dissolve over months, the suture is nonabsorbable and can be removed. The lift is present the day it’s placed.
Layer Three: Skin And The Fibroseptal Network
Skin recoil, plus the connective tissue foundation between skin and muscle. This is where energy-based tightening earns its place. FaceTite and Renuvion, a helium plasma device that delivers radiofrequency energy, both reach the deepest layer of the skin and the fibroseptal network beneath it. Surface treatments like lasers and microneedling work from the top down and cannot get to that foundation. When skin hangs at rest rather than sitting loosely, it has passed what any energy device can reach, and the honest answer is surgery for loose neck skin instead.
Layer Four: Chin Projection And Jaw Angle
Bone. A recessed chin or a soft jaw angle limits how sharp any result can get, no matter how much fat comes out. Where projection is the limiting factor, structural filler is the answer: calcium hydroxylapatite such as Radiesse, or a hyaluronic acid such as Juvederm, placed to build the frame rather than fill the surface. Nobody should promise you a superhero jawline your mandible cannot support.
Why More Northeast Community Patients Are Asking About Neck Skin In 2026
Rapid weight loss changed who walks in the door.
GLP-1 medications have helped a lot of people drop weight quickly. Fat under the chin goes first and goes fast. The skin does not always keep pace. In the 2025 annual member survey run by the American Academy of Facial Plastic and Reconstructive Surgery, fielded in December 2025, 67 percent of facial plastic surgeons reported an increase in patients seeking treatment related to rapid weight loss, up 45 percent from the year before. For you, that changes one thing. Somebody 40 pounds down on a GLP-1 often has less fat to remove and more skin to manage than the same person would have had three years ago. That’s a different operation, and sometimes it’s a different procedure entirely.
If your weight is still moving in either direction, Dr. Grandhige will tell you to wait. Operating on a moving target rarely holds up.
What Happens On Procedure Day
Awake, in the office, and usually done inside an hour.
You take an oral sedative. Dr. Grandhige numbs the area with local anesthetic. There’s no general anesthesia, no breathing tube and no recovery bay. The work happens through very small incisions under the chin and behind the ears, placed where they heal to near invisible. Fat comes out around the jaw angle, and a healthy layer stays behind on purpose, because that remaining layer is what keeps the result smooth instead of hollow. When laxity is part of the picture, Renuvion or FaceTite runs in the same visit to tighten the skin over the new contour.
Worth knowing, because almost no neck liposuction page in Tampa says it out loud: Florida regulates office liposuction directly. Under the Florida Board of Medicine office surgery rules, 4,000 cubic centimeters of supernatant fat is the ceiling for the office setting, any office removing more than 1,000 cubic centimeters must register with the Department of Health, and those offices get inspected annually unless they hold accreditation from a nationally recognized agency. A neck case never approaches those volumes. That’s precisely why the office setting is the right one for it. For cases that do need general anesthesia, Formation operates in Quad A accredited facilities, with the same anesthesia provider and the same core team on every case.
The Preparation Window Most Practices Skip
Your health gets assessed before anything is scheduled, not after.
Diet, protein intake and inflammation get looked at first. Nutrition, supplements and peptides are then adjusted in the window immediately before and immediately after the procedure, so your body builds as much structure as it can while it’s healing. Dr. Grandhige has spent years assembling the providers who handle lymphatic work, nutrition and coaching, and he vets them personally rather than handing you a list.
Here’s the part that matters. The cannula in Dr. Grandhige’s hand is the same cannula sold to every practice in this state. The radiofrequency device is the same device. The instrument is identical everywhere. The thinking around it is not.
Recovery, Week By Week
Dr. Grandhige treats surgery as 60 percent of your result and post-operative care as the other 40 percent. Bad aftercare can undo good surgery, which is why recovery here is planned rather than handed over as a pamphlet. That split comes out of a larger idea about how surgery should work, which you can read about in Formation’s philosophy.
- First few days: mild swelling, bruising and tightness. The chin compression garment goes on.
- One week: back to work and social activities. Garment worn 12 to 24 hours a day. Most patients return to training around this point.
- Two to four weeks: swelling keeps improving and the fat removal becomes visible.
- Three months: skin tightening shows, with clear improvement along the jawline.
- Six to nine months: final result, once the tightening has fully matured.
Lymphatic treatment starts at your first post-operative visit. It moves fluid and heads off collections before they form. If a small fluid collection does appear, it gets drained in the office at a follow-up rather than watched. Dr. Grandhige stays with you for a full year, and nearly all Formation patients complete the year.
Where Neck Liposuction Goes Wrong
The failure mode isn’t taking out too little. It’s taking out too much.
Over-resection in the neck produces a scooped, hollow look under the chin, exposes small contour irregularities that were hidden before, and gets worse over the following decade instead of settling. It’s one of the few outcomes in aesthetic surgery that ages badly by design. Aggressive fat removal here can also make a neck look older rather than younger, because there’s nothing left for the skin to drape over. Where skin is already slack, removing the fat beneath it often does more harm than good.
Temporary numbness under the chin is normal and resolves. Fluid collection is uncommon and manageable when somebody is watching for it. The permanent problems come almost entirely from over-treatment, which is why restraint is the technique rather than a personality trait. Men in particular need the jawline and neck read as one unit, since small errors here read as obvious, and that combined read runs through all of Dr. Grandhige’s men’s aesthetic surgery work.
Who This Fits, And When Formation Says Wait
Strong candidates sit at or near a stable weight, are bothered by chin and jaw fat that ignores diet and training, and still have reasonable skin recoil. Skin quality tracks with age, and this procedure works especially well across roughly skin ages 25 to 55.
You’re likely a fit if your weight is steady but the fullness under your chin won’t move, if you dislike your profile in photos and on video calls, if your face reads heavier or more tired than your body does, and if you want a permanent change rather than repeat maintenance visits. The fat cells removed don’t come back, so the result holds as long as your weight stays stable.
You’re in the most common age group for this, too. The American Society of Plastic Surgeons reports liposuction was the most popular plastic surgery procedure of 2024, and patients aged 40 to 55 received 45 percent of all liposuction procedures that year. That’s short enough to keep post-operative visits and lymphatic sessions practical instead of turning them into a project. Plan on your first follow-up inside the first week, then a schedule that thins out as swelling resolves. If a fluid collection shows up, you can be in and out the same day rather than waiting it out. For patients driving in from Orlando or Naples, Formation arranges discounted rates at nearby hotels and Dr. Grandhige visits them in their rooms during the early days. From Northeast Community, you don’t need any of that.
