What A Tummy Tuck Corrects That Liposuction Cannot
Liposuction removes fat. It does nothing for skin that has lost its recoil and nothing for muscle that has pulled apart.
Those are two separate problems, and both are common after pregnancy or a large weight change. Skin stretched past its elastic limit does not shrink back once the volume underneath is gone. And the rectus abdominis muscles, the paired columns running down the front of the abdomen, can separate along the midline in a condition called diastasis recti. When they do, the abdominal wall bulges outward under pressure no matter how strong the muscle itself becomes. Crunches can make that bulge worse.
Abdominoplasty removes the excess skin, closes the separation with internal sutures, and reshapes what remains. The result is structural, not cosmetic alone. Patients frequently report better posture and less low back strain once the front wall is holding tension again.
That is the honest dividing line. If the only issue is a layer of fat over an intact wall, this is the wrong operation.
Getting To Formation From Palma Ceia

The Fat Check
Fat above the muscle can be suctioned. Fat behind the muscle, packed around the organs, cannot be reached surgically at all and only responds to weight loss. A firm, round abdomen that does not pinch usually means the volume is behind the wall. Surgery will disappoint that patient.
The Stability Check
Weight should be steady for about six months, and childbearing should be finished. Repair holds under a stable load and stretches again under a changing one. Dr. Gopal Grandhige, a board-certified general surgeon specializing in aesthetic surgery, will tell patients to wait when this check fails, even when the first three pass cleanly.
What Happens During The Procedure
Abdominoplasty is a longer operation than most patients assume, and the sequence is consistent.
Excess skin is removed through a low horizontal incision positioned to sit under clothing. The abdominal muscles are repaired if they have separated, drawn back to the midline and secured with internal sutures. The remaining tissue is redraped and reshaped to restore a flatter front wall. The navel is repositioned to sit correctly against the new contour.
Liposuction is frequently added to refine the flanks and smooth the transition between the treated abdomen and untouched tissue at the waist. Roughly 6 in 10 Formation abdominoplasty patients have some contouring performed alongside the main repair, because a flat abdomen against unaddressed flanks reads as unfinished.
Dr. Grandhige designs the surgical plan himself and personally performs every operation, working with the same anesthesia provider and the same physician assistant across all his cases. Consistency in the room is a safety argument, not a marketing one.
The Recovery Timeline You Should Plan Around
Recovery from abdominoplasty is measured in months, and the first six weeks carry real restrictions. Plan the calendar before booking the surgery.
First Week
Rest, controlled mobility, and pain management. A nurse is provided for the first 24 hours. Help at home for the following several days is strongly recommended. Walking starts early and short.
Weeks Two And Three
Gradual return to light activity. Compression garments remain in continuous use. Most patients return to desk work between day 10 and day 14. Lymphatic treatment begins in this window and does most of its work here. Formation patients average about 6 sessions across the first month.
Weeks Four Through Six
Swelling drops noticeably. Lymphatic treatment continues. Patients return to exercise without restriction at six weeks, not before. This is the milestone people try to rush, and rushing it is how repairs get stressed.
Three To Six Months
Final shape continues refining as residual swelling resolves. The waistline that shows up at month four is not the one that shows up at month six.
Dr. Grandhige’s position is that surgery accounts for about 60 percent of the outcome and post-operative care accounts for the other 40. Poor aftercare can undo a well-executed operation. Patients are followed for a full year.
Where The Surgery Actually Takes Place

That matters more for this procedure than for smaller ones. Abdominoplasty involves general anesthesia, muscle repair, and a longer operative window, and Florida regulates where procedures at that level may be performed. Office-based surgery in Florida operates under volume and registration thresholds that a full abdominoplasty with added contouring can approach quickly. Performing it in an accredited facility rather than an exam room removes that question entirely.
Quad A accreditation covers infection control and sterilization standards, emergency protocols and redundancy systems, credentialing of all surgical and anesthesia staff, and continuous quality oversight. Facilities are inspected on a recurring basis rather than certified once.
Ask any surgeon you consult where they operate and what that facility is accredited by. If the answer is vague, keep asking.
Where The Surgery Actually Takes Place
Formation turns down abdominoplasty candidates regularly. These are the most common reasons.
You are not finished with weight loss or pregnancy. The repair holds under a stable load. Waiting is not a delay tactic, it is the difference between a result that lasts and one that needs revision.
Your issue is definition, not laxity. Athletic patients at or near goal weight, with intact skin recoil and no separation, are asking for something abdominoplasty does not deliver. High-definition liposuction is the correct procedure for revealing muscle structure that is already there.
The laxity extends well beyond the abdomen. Circumferential skin excess after major weight loss can need lift-level surgery rather than an anterior repair. Dr. Ahmad Saad, Formation’s board-certified plastic surgeon, handles those cases.
You cannot commit to the recovery. Six weeks without unrestricted exercise, roughly two weeks away from physical work, and continuous compression are not negotiable. Patients who cannot clear that window should schedule for a season when they can.
You are an active smoker. Nicotine constricts blood flow to healing tissue and raises the risk of wound complications on a procedure that depends on a long incision closing cleanly.
Where The Surgery Actually Takes Place
The patient walking into an abdominoplasty consultation in Palma Ceia today is often a different person than five years ago.
