Layer One: The Fat
He pinches under your chin and along the jaw border. What he wants to know is whether the fullness sits above the muscle, where a cannula can reach it, or below it, where it cannot. Fat below the platysma behaves differently and does not respond the same way. Plenty of people who look like textbook candidates in photos fail this check in person.
Layer Two: The Skin
He pulls the skin at the jaw border and lets go, then watches how fast it returns. Skin that snaps back holds a new contour. Skin that drifts back slowly will hang in the same place after the fat is gone, only now with less underneath it. Most patients who do well here fall between roughly 25 and 55, though the number on your license matters far less than how your skin behaves in his hand.
Layer Three: The Structure
Jaw angle, chin projection, the position of your submandibular glands, and whether the vertical bands in your neck stand up when you clench. A recessed chin or a prominent gland will read as a soft jaw no matter how much fat comes out. That is an anatomy problem, not a fat problem, and he will tell you so.
The three answers combine into one of three plans. Fat with good skin means neck liposuction on its own. Fat plus moderate laxity with decent recoil means liposuction with radiofrequency tightening in the same visit, which is what most people mean when they say FaceTite. Heavy loose skin, standing bands, or a gland sitting low means neither, and he will say that out loud in the room.
What 72 Jawline And Neck Cases Showed Us In 2025
Formation performed radiofrequency jawline and neck tightening on 72 patients in 2025. The number stays low on purpose. Dr. Grandhige plans every case, performs every case, and follows every patient for a full year, and that ceiling is what makes it possible.

- 44 of the 72 were men. The average age was 47, with the youngest at 31 and the oldest at 61.
- 58 combined tightening with liposuction in the same visit. Only 14 needed tightening alone.

- Median return to work was 6 days.
- 19 more people came in for a jawline consultation that year and were told FaceTite was not the answer.

When Dr. Grandhige Says No
Nineteen people heard no in 2025. The reasons repeat.
Some were more than fifteen pounds above a stable weight. Fat that is actively changing will not hold a contour, so he asks them to come back once the scale stops moving.
Some had skin that failed the second layer badly. He tells them plainly that removing the fat would make the hang more obvious, not less.
Some had a chin that sat too far back. No amount of tightening builds a jaw that is not there, and pretending otherwise produces a patient who feels cheated at six months.
And some just were not bothered enough. He asks what specifically prompted the visit. If the answer is a single unflattering photo taken from below, that is a camera angle, not a diagnosis.
Saying no costs a booking. It is also the thing patients repeat to their friends two years later, and it sits at the center of our philosophy about doing only what is necessary and doing it well.
Your Procedure Day, Hour By Hour
You are in and out in about three hours. Here is the sequence.
Arrive thirty minutes early at the South Howard office. Photos get taken in standardized lighting, then Dr. Grandhige marks your jaw and neck while you are sitting upright, because the tissue falls differently lying down and marking on a reclined patient is how contours end up uneven.
You take an oral sedative. Local anesthetic goes in next and needs about twenty minutes to fully set. You stay awake and you can talk the whole time.
Treatment runs sixty to ninety minutes depending on how many zones are being covered. Dr. Grandhige keeps a second tightening option in the room and chooses between them at the table based on how thick your skin is and how the tissue responds to the first passes. That call gets made with the probe already under the skin, which is why it matters who is holding it.
Your chin garment goes on before you stand up. Then you go home. You will need a driver, because of the sedative, and that is not negotiable.
Every case happens in a Quad A accredited surgical suite with the same anesthesia provider and the same staff Dr. Grandhige works with on all of his cases. Quad A accreditation requires a facility to meet its full standard set and pass an on-site survey, and it covers everything from sterilization to emergency protocols.
Recovery In Tampa Heat, Week By Week
Recovery here is managed, not waited out. The timeline below is what Formation patients actually follow.
First few days. Mild swelling, bruising, and a tight feeling under the jaw. The chin garment stays on. Most people are surprised by how little pain there is and how much the tightness dominates instead.
Week one. Back to work and back to social plans. The garment goes 12 to 24 hours a day this week. It fits under a collared shirt and disappears on video calls, which is how most of our North Tampa professionals get through it without taking time off.
Weeks two to four. Swelling drops steadily and the liposuction result becomes visible. This is the point where people start getting comments.
Three months. Skin tightening shows up. The jaw border gets a clean edge.
Six to nine months. Final result. This is when the skin finishes contracting and what you see is what you keep.
Two things about doing this in Tampa specifically. Buy a second chin garment before your procedure.
