A separate multicenter retrospective review of 2,000 randomly selected PDO cases across 105 clinics put the overall complication rate at 12.4%, with skin retraction at 6.35% and asymmetry at 2.9%. The part worth memorizing is what predicted those complications. Two variables did, independently:
- Thread count. Each additional thread raised the odds of a complication by about 12%.
- Age. Compared with patients under 40, the odds ran about 2.1 times higher at 40 to 49, about 3.4 times higher at 50 to 59, and about 5.1 times higher at 60 and above.
A related review of 1,500 patients found formal dissatisfaction, meaning patients returning because the result was not enough, at 3.4%. The same two predictors showed up, with the inflection points sitting near age 43 and near 10 threads.
Read that together and the conclusion is uncomfortable for the industry: the more threads a provider sells you, and the older you are, the worse the math gets. A clinic paid per thread has no incentive to say that. Formation does not sell threads by volume, and this data is the reason.

The 510(k) summaries for several commonly used PDO thread devices state the product is indicated for soft-tissue approximation and is not intended for lifting and supporting tissues. Cosmetic facial lifting sits outside the cleared indication for a number of the threads currently in circulation.
That does not make a thread lift illegitimate. Off-label use of cleared devices is routine and lawful in medicine, and physicians exercise that judgment constantly. It does mean any page telling you threads are “FDA-approved for facial lifting” is either careless or selling. Ask your provider which device they use and read its 510(k) summary yourself. You are entitled to know what is going into your face.
The Trade-Off Nobody Mentions: Threads And Future Surgery
Threads leave something behind, and it can complicate a facelift later. This is the single most under-reported item in thread-lift marketing.
Reviews published across 2025 and 2026 note that the fibrosis and scar tissue threads generate can make a later surgical facelift technically harder and less predictable. Tissue planes that would normally separate cleanly do not. The temporary nature of the lift is advertised everywhere. The possible downstream cost to a future operation is not.
