Point 2: Skin That Recovers
Pull the skin at your jawline away and let go. It should snap back. Skin in the 25 to 55 range typically retracts well enough to follow a new contour, especially with radiofrequency tightening added underneath. Outside that window it becomes a case-by-case call, and the answer is sometimes a different procedure entirely.
Point 3: A Jawline Worth Revealing
This is the point that catches people. Neck liposuction exposes whatever structure sits under the fat. If your jaw angle is soft, your chin sits back, or your hyoid bone rides high in the neck, taking fat away reveals that instead of a sharp line. A neck can be thinner and still not look better. Dr. Grandhige assesses jaw projection, hyoid position, and submandibular gland prominence at the consultation, and this is usually where a recommendation to do something else comes from.
Point 4: Weight That Has Held
Fat cells that come out do not come back. If your weight has swung more than 10 or 15 pounds in either direction over the past year, the contour you buy today sits on a shape that is still moving. Stabilize first, then contour.
Clearing all four does not make surgery automatic. It makes the conversation worth having. How Formation approaches every case starts from the position that the correct answer is sometimes no.
What Formation's Neck And Jawline Cases Look Like
Neck and jawline contouring is one of the five procedures Dr. Grandhige performs most often. Across Formation’s neck and jawline cases over the past 18 months:
- 9 in 10 were performed awake in the office rather than under general anesthesia at a surgical facility.
- Median return to desk work was 6 days.
- Patients wore a chin compression garment 12 to 24 hours a day through the first week, then overnight through week four.
- Patients averaged 3 lymphatic drainage sessions inside the first month.
- Roughly 4 in 10 neck patients were men.
- About 1 in 4 neck liposuction consultations ended in a recommendation to wait, address something else first, or choose a different procedure.
That last figure is the one worth sitting with. A practice that says yes to everyone who walks in is not screening anyone. Formation caps surgical volume on purpose so Dr. Grandhige personally evaluates each patient and designs each plan himself, and that only works if the answer is allowed to be no.
The Risk Nobody Puts On Their Website
The most common way neck liposuction goes wrong is not infection. It is taking out too much fat.
Removed fat cells do not regenerate. Over-resection under the chin produces a neck that reads hollow rather than sharp, exposes the vertical bands of the platysma muscle, and gets stranger with age and weight loss instead of better. Patients who end up there frequently need fat grafting later to replace volume that was taken. Read enough patient forum accounts of neck liposuction regret and the same shape repeats: the result looked fine at three months and wrong at three years.
The published safety data, by contrast, is reassuring:
- A series of 987 neck liposuctions performed under tumescent local anesthesia reported five cases of temporary marginal mandibular nerve weakness, one submandibular gland ptosis, and one arterial bleed. No seromas, no skin necrosis, no scarring, no hyperpigmentation.
- A meta-analysis covering more than 21,000 aesthetic liposuction patients put the overall complication rate near 12 percent, falling to about 6 percent once expected swelling and bruising are excluded. Contour irregularities landed near 2 percent, seroma near 2 percent, hematoma and infection near 1 percent each.
- Large ambulatory-surgery database reviews put confirmed complication rates closer to 0.4 percent.
Put those together and the picture gets clear. The procedure itself is well tolerated. The thing that produces a bad neck is a judgment call, not a complication. Dr. Grandhige’s stated position is that over-resection creates unnatural contours, irregularities, and results that age badly, so he under-resects on purpose and avoids aggressive skin disruption. Liposuction remains, per the American Society of Plastic Surgeons, among the most performed and most studied cosmetic procedures in the country. Doing it well comes down to how much you decide to leave behind.
The 60/40 Rule
Dr. Grandhige puts a number on aftercare: the surgery is 60 percent of the result and the post-operative care is 40 percent. Poor aftercare can ruin a well-performed operation. That is why Formation owns the post-op process instead of handing you a garment and a phone number.
Week by week:
- First few days. Mild swelling, bruising, and tightness. The chin compression garment stays on continuously.
- Week 1. Most patients are back at work and out socially. Garment on 12 to 24 hours a day.
- Weeks 2 to 4. Swelling keeps improving and the liposuction result becomes visible. Lymphatic drainage sessions run through this window to move fluid and stop collections from forming.
- 3 months. Skin tightening shows up. This is when other people start noticing the jawline.
- 6 to 9 months. Final result. Skin tightening has reached its endpoint.
Dr. Grandhige follows patients for a full year after the procedure, and nearly all of them stay in contact for the entire twelve months. The lymphatic, nutrition, and coaching providers Formation sends patients to have been personally vetted rather than pulled off a directory.
When Neck Liposuction Is Not The Answer
Roughly one in four neck consultations at Formation ends somewhere other than the operating chair. The usual reasons:
Skin is the problem, not fat. If the pinch test comes back thin and loose, taking fat out makes the laxity more obvious. Skin tightening on its own, or non-surgical neck and jawline contouring, is usually the better first step.
The jaw needs projection, not subtraction. A recessed chin reads as a full neck even when very little fat is present. Removing the little that exists does not fix the profile.
Weight is still moving. Patients partway through a weight-loss course get asked to stabilize first. A contour built on a shifting frame does not hold.
The bulge is the submandibular gland. Liposuction will not touch it, and thinning the fat above it makes it more visible.
The whole neck needs lifting. Past a certain degree of laxity and banding, liposuction is the wrong tool and using it anyway makes the banding worse.

He attended the University of Michigan Medical School and matched into a six-year combined plastic surgery residency at Yale University School of Medicine before changing course. He completed a full general surgery residency, then fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. His board certification in general surgery is verifiable through the American Board of Surgery.
