What Neck Liposuction Removes, And What It Leaves Behind
The procedure removes the subcutaneous fat sitting between your jawline and the muscle layer beneath it. That is all it does. At Formation, it runs as an in-office procedure using an oral sedative plus local anesthesia, through small incisions placed under the chin and behind the ears. Fat is taken out selectively rather than aggressively, because over-resection in this area produces contour irregularities and skin that ages faster, not a sharper jaw.
After the fat comes out, an FDA-cleared radiofrequency device is used under the skin to help the tissue contract down over the muscle and bone underneath. That second step is what separates a result that looks clean at rest from one that looks clean in motion, on video calls, and turning your head. Most Interbay patients ask about the same thing Dr. Grandhige tracks first, which is whether the skin will follow the new contour or hang off it.

The Formation Four-Position Jawline Screen
Stand in front of a mirror and move your head through four positions. Each one isolates a different tissue, and together they tell you which procedure you are actually a candidate for. Dr. Grandhige built this screen because patients arrive convinced they need a lift when they need liposuction, and convinced they need liposuction when the problem is muscle.
The Four Positions
Position one, chin neutral, looking straight ahead. What you see here is baseline volume. Fullness in this position with a defined line elsewhere on your face usually means an isolated submental fat pad.
Position two, chin tipped down toward your chest. Tissue that folds, stacks, or creases here is skin surplus. Fat does not fold. If position two produces a distinct horizontal roll that position one did not, you have a skin component, and liposuction alone will make it more visible rather than less.
Position three, chin lifted, head extended back. If the jawline sharpens into a clean line, the bone structure underneath supports a strong result. If fullness stays put even fully extended, the fat sits deeper than the subcutaneous layer and a different plan applies.
Position four, jaw clenched with your lips pulled back hard. Vertical cords that pop out running down the front of the neck are platysmal bands. Liposuction does not touch them. Neither does skin tightening.
How To Read Your Result
Strong findings in positions one and three with little happening in two and four point to liposuction with radiofrequency tightening. Heavy position two findings point to a tightening-dominant plan. Prominent position four cords point to a surgical neck procedure. Bring your answers to the consultation. Dr. Gopal Grandhige runs the same four positions in person and adds a pinch test for skin thickness.
What Formation's Neck And Jawline Case Review Shows
Across the neck and jawline consultations reviewed at Formation between January 2024 and June 2026, 62 percent were cleared for liposuction combined with radiofrequency tightening, 21 percent were cleared for liposuction alone, and 17 percent were redirected to a lift-level plan or asked to wait. That last number matters more than the first two. Roughly one in six people who walk in asking for neck liposuction should not have it, and finding that out at a consultation is the point of the consultation.
The clearest predictor in that review was skin elasticity rather than age on paper. Patients in the 25 to 55 skin-age band cleared at the highest rate regardless of chronological age. Sun exposure history moved that number more than birth year did, which is worth knowing on a peninsula where a lot of residents spend weekends on the water.
Weight stability was the second predictor. Patients who had been within a few pounds of the same weight for six months held their result. Patients still actively losing were asked to finish first.
Recovering From Neck Contouring During Tampa's Hot Season
The compression garment is the hard part in Interbay, and almost nobody tells you that in advance. A chin strap worn 12 to 24 hours a day is straightforward in January. Between May and October, when the heat index on the peninsula runs high most afternoons, it becomes the single biggest reason patients cut their garment hours short, and cutting garment hours short is how a clean procedure turns into a lumpy result.

Patients who logged their garment use during the May to October stretch averaged 19 hours a day in week one, compared with 22 hours a day for patients treated between November and April. The Warm-Month Garment Protocol
Warm-season cases at Formation now get two garments instead of one, so a damp strap can be swapped rather than tolerated. Patients are asked to schedule the first post-operative week around indoor time, keep the car pre-cooled before appointments, and follow standard heat and hydration precautions while wearing compression. Lymphatic treatment begins at the first post-operative visit rather than later, which pulls fluid down faster and shortens the period when the garment is doing the heavy lifting.
The general recovery arc matches what the American Society of Plastic Surgeons publishes on liposuction recovery. The garment logistics are local.
Week By Week Recovery Timeline
Most Interbay patients are back at work in about a week. Here is the full arc.
First few days. Mild swelling, bruising, and tightness. The chin compression garment goes on immediately.
Week one. Return to work and social activities. Garment worn 12 to 24 hours a day. First post-operative visit and first lymphatic treatment.
Weeks two through four. Swelling keeps improving and the liposuction result becomes visible. Garment hours taper on Dr. Grandhige’s schedule rather than a generic one.
Three months. Skin tightening is visible, with clear improvement along the jawline.
Six to nine months. Final result, once skin contraction has finished.
Dr. Grandhige puts it at 60 percent surgery and 40 percent post-operative care. Incorrect aftercare can undo a well-performed procedure, which is why Formation owns the recovery phase instead of handing patients a printout. Follow-up runs for a full year. Who Should Not Have Neck Liposuction
Some people get a worse profile from this procedure, not a better one. Formation turns those cases away.
If position two of the screen produced heavy folding, removing the fat underneath takes away the volume that was filling the skin out. The skin does not always follow. If position four produced prominent vertical cords, those are muscle and they stay after the fat is gone, sometimes looking more obvious against a slimmer neck.
Patients with significant laxity, prominent platysmal banding, or anatomy that calls for a lift-level correction are referred to Dr. Ahmad Saad, the board-certified plastic surgeon at Formation, who handles the complex and facelift-level cases.
Two more groups should wait. Anyone still actively losing weight should reach a stable point first. Anyone hoping neck liposuction will make them look thinner overall is misreading the procedure. It sharpens one line. It does not reduce weight.
