Florida has three places named some version of Palmetto, and search engines flatten all three into one.
Palmetto Beach is a Tampa neighborhood in the 33605 ZIP, pinned between the Ybor Channel and McKay Bay, with DeSoto Park at its center and Ybor City directly north. Palmetto is a separate city in Manatee County, about 45 minutes south across the bay. Palmetto Bay sits in Miami-Dade, four hours away.
Type the neighborhood name into a search bar and you’ll get med spas in Bradenton, wellness clinics built for Palm Beach County, and South Miami practices targeting Palmetto Bay. None of them can tell you how long the drive actually takes from Bermuda Boulevard.
Here’s what’s true on the ground: there is no dedicated injector operating inside the neighborhood boundaries. Residents drive out. Most head toward South Tampa or SoHo, and the short trip means a Palmetto Beach patient can pick on judgment rather than proximity, because everything is close. That changes the question from “who’s nearest” to “who should be holding the syringe.”

How To Confirm Your Injector Can Legally Treat You In Florida

The Two-Minute License Check
Do this before your appointment, not after.
Ask for the injector’s full legal name and license type. Not the clinic name. The person’s name.
Open the Florida Department of Health license verification portal and search by last name and profession. It’s free and public.
Confirm three things: the status reads Active, the license type actually permits injecting, and the discipline section is empty. A record you can’t find at all is the loudest possible answer.
Then ask where the product is sourced. A practice buying through authorized channels will answer that question without hesitating.
The Still-Face Screen: Three Questions Before Anyone Injects You
Formation runs the same three-question screen on every face before a needle comes out. You can run it on yourself in a mirror right now.
One: is the line there when your face is completely still? A relaxer softens lines your muscles are actively creating. If a crease sits in your skin at total rest, muscle isn’t the whole problem, and quieting the muscle won’t erase it.
Two: is the problem muscle, volume, or skin? Only the first one responds to a neurotoxin. Hollowing is volume. Crepey texture along the jaw is skin. Treating the wrong layer produces a result that looks off and costs you months.
Three: does treating this now protect what you want in two years? Over-relaxing a forehead drops the brow. Chasing one line teaches you to chase the next. Restraint early is what keeps a face looking like itself at 55.
Roughly one in five people screened at Formation leave without an injection that day. Sometimes the answer is a different treatment. Sometimes it’s wait six months. Dr. Grandhige built the practice around being willing to say that out loud, which is the same reason he limits how many procedures he takes on.
Men Get A Different Injection Plan
A male face treated with a plan copied from a female patient produces one of two failures: nothing visible, or an arched brow that reads as feminized.
The reasons are anatomical. Men carry more muscle mass in the glabellar complex, so an identical dose lands with less effect. Men also have a naturally flatter, lower-set brow, which means the injection pattern that lifts a woman’s arch actively works against a masculine brow line. The forehead has to be treated conservatively for the same reason, because dropping a brow that already sits low reads as heaviness rather than rest.
About four in ten faces treated at Formation belong to men, which is unusual for an aesthetic practice and reflects where the clinic’s surgical work already concentrates. Male body contouring and male facial refinement are the two things this practice is known for, and the same rule governs both: do only what’s necessary, then stop.

When A Wrinkle Relaxer Is Not The Answer
A neurotoxin relaxes muscle. That’s the entire mechanism. If the thing bothering you lives in a different layer of your face, no amount of product will move it.
The most common mismatch is the jawline. Patients arrive convinced injections will sharpen a soft jaw or erase a double chin. Fullness under the chin is fat. A loose line along the mandible is skin. Neither one is muscle, and neither responds.

Before You Come In
Bring a list of what bothers you and when you notice it. Photos of yourself from a few years ago help more than you’d expect. If you’ve been injected before, note roughly when and where, because residual effect changes what can be assessed accurately.
The Day Of
Dr. Grandhige evaluates every patient personally. He’ll photograph your face at rest and in motion, because a still photo hides exactly the information that matters. You’ll be walked through the Still-Face Screen against your own anatomy, told which lines will respond and which won’t, and given a plan. If the plan is to do nothing yet, he’ll say so.
The Two-Week Check
Full effect lands somewhere around day 10 to 14, so that’s when the result gets reviewed rather than at the moment you walk out. Small refinements happen at that visit. This follow-through is part of our philosophy across every service here, surgical or not.
How Injectables Fit Around Surgery
If you’re weighing a facial or neck procedure in the next year, injection timing stops being cosmetic and becomes clinical.
Relaxers near a planned surgical field are generally paused beforehand and restarted once swelling settles. The reason is judgment, not safety. Injecting into tissue that’s still resolving makes it genuinely difficult to tell what the surgery accomplished and what the product is doing, and that ambiguity is how patients end up chasing a result they already have.

A new northbound I-275 exit into Ybor opens in fall 2026, and the full project wraps in spring 2027. If 22nd Street is your usual on-ramp, that habit has an expiration date. The Selmon route to SoHo avoids the work zone entirely.
