Most people get PRP in the knee as a short series, usually one to three injections spaced about one to two weeks apart. If that first course helps, a repeat round once a year or so is common. That’s the honest short answer.
How often you can get PRP injections in your knee depends on three things: how much arthritis you have, how well you responded the first time, and what you’re trying to do with the joint. Spacing is usually weekly to every two weeks within a round. Some clinics stretch it to about four weeks. One practical note: skip anti-inflammatory painkillers like ibuprofen around your injections. They blunt the very platelets the treatment relies on.
Between rounds, most people wait about a year. Relief from a good course of knee PRP tends to run six to twenty-four months, with a year being a fair average. When the effect starts to fade and symptoms creep back, that’s usually the window to consider another round.

The same guidance says relief usually runs at least a year, sometimes longer, and rarely less than six months. It also pushes clinicians to reassess after the first injection instead of committing you to a fixed package. That fits how we work. Your second shot should be a decision, not a default.
Three things shape your personal schedule:
A 2025 study of 167 knees found milder arthritis peaked after the fourth monthly shot, while more worn knees needed a fifth, and the gains held at two years. Useful, but the pattern still comes back to the individual knee.
No. This is where I get blunt. More PRP shots don’t reliably mean a better knee.
The data keeps showing diminishing returns past about three to five injections in a round. Three shots and five shots tend to land in the same place. So when a clinic starts you at one shot and keeps upselling you toward five or six, that’s a sales pattern, not a medical one. On the other end, a true single-shot “one and done” pitch is often underpowered for arthritis.
You might feel some soreness and bruising for a day or two after an injection. That flare isn’t a sign the shot is working harder. It’s a needle in a joint, nothing more.
The biggest mistake I see is a patient with bone-on-bone grade 4 arthritis committing to a long series expecting the knee to rebuild. It rarely does. Those rounds are often better replaced by a real conversation about a knee replacement.
Yes, repeating knee PRP is considered safe. Because it’s your own blood, there’s no strict lifetime limit, and side effects are usually mild and short: some soreness, swelling, or stiffness for a few days.
One 2025 review of about 2,750 patients did find more minor reactions with PRP than with a placebo shot, roughly 19 percent versus 9 percent, but almost all were mild and self-limiting, and the rate matched other common knee injections. Compare that to steroid shots, which are usually capped near every three months over concerns they can wear on cartilage. PRP doesn’t carry that same cartilage worry, which changes the repeat math.
A note on status. PRP for knee arthritis isn’t FDA-approved as a treatment. The devices used to prepare it are FDA-cleared, but the joint injection itself is considered off-label and investigational.
One correction, because it matters. “Regenerative” oversells what PRP does to a knee. In practice, PRP tends to ease pain for months and calm the joint. It isn’t regrowing cartilage or reversing arthritis, and imaging rarely shows a structural change. Think of it as buying quality time, not building a new knee.
