A PRP joint injection usually lasts 6 to 12 months in an arthritic knee. In a chronic tendon, closer to one to two years. Some patients get 18 solid months out of one series. Others get four.
That spread isn’t luck and it isn’t marketing. It tracks how far the arthritis has already gone, how concentrated the platelets were, whether you got one shot or three, and what you did with your body in the 12 weeks after.
I’ll give you the numbers by area, the trials behind them, and the part most clinics skip. PRP doesn’t hold its ground on its own.
Two things this article won’t cover. PRP for hair and facial work, since that’s different tissue with a different evidence base. And stem cells and exosomes, which sit beside PRP in our regenerative program but answer a different question.
A PRP joint injection is a shot of your own concentrated platelets into a painful joint or tendon. A small blood draw spins in a centrifuge, which separates the platelet-rich plasma layer from red cells and most of the plasma. That layer carries growth factors that signal repair. Nothing synthetic gets added.
The material leaves your arm and goes back into your body the same visit.
What it doesn’t do is regrow cartilage. No high-quality trial has shown reliable cartilage regeneration, and any clinic promising you a new knee is overselling a tool that works fine on its own terms.
Six to 12 months for most arthritic joints. One to two years for stubborn tendon problems. A few months at best in a joint that’s already bone on bone.
Those last three sets are what we see in our own patients rather than trial output, and I’d rather label that than dress it up.
Past two years, the research thins out and starts contradicting itself. One 2026 cohort tracked benefit out to roughly four years in early-stage knees treated with a three-injection series, fading toward baseline by year six. A separate five-year real-world analysis found no lasting edge over conservative care at all. Both groups declined together. That gap is why maintenance dosing is now a live conversation instead of an afterthought.
Because they do different jobs. Cortisone suppresses inflammation fast and fades fast. PRP starts slow and leaves a longer tail.
I’ll say where I land. Putting a 44-year-old with an early arthritic knee on a repeating cortisone schedule is a bad trade. You buy six weeks at a time, and repeat steroid exposure isn’t neutral for cartilage. If that patient can commit to the slower option, it’s the better one. If they can’t, I’ll say so plainly instead of pretending PRP is the only defensible answer.
Partly, and the split matters more than any single headline. Any clinic that hides this is selling you something.
The American College of Rheumatology, writing with the Arthritis Foundation, recommends against PRP for hip and knee arthritis. OARSI goes further and recommends against intra-articular PRP outright. The American Academy of Orthopaedic Surgeons gives it only a limited recommendation, and its recent ankle osteoarthritis guidance recommends against routine PRP in that joint specifically.
Then April 2026 happened.
The American Academy of Physical Medicine and Rehabilitation released formal guidance saying PRP should be considered for mild to moderate knee arthritis, Kellgren-Lawrence grades I through III, in patients still symptomatic after weight management, physical therapy, and analgesics. Not for grade IV. It’s the most concrete standardization step this field has taken.
The disagreement comes down to preparation. “PRP” isn’t one product. Platelet concentration, white cell content, spin protocol, volume, and injection count vary wildly between clinics, so trials get pooled that were never testing the same thing. The 2025 analysis in the American Journal of Sports Medicine found high-platelet preparations held their functional gains at 12 months while low-platelet preparations did not. Average a strong prep with a weak one and you land on “unclear.” That’s a measurement problem, not proof that platelets don’t work.
Good candidates share a pattern. Mild to moderate wear, a clear pain generator, and a body already doing some of the work.
Strong fits include active adults in their 30s and 40s with early joint wear, patients over 50 who’ve been told to live with it, chronic tendon cases that haven’t responded to therapy, and anyone trying to delay a joint replacement they’re not ready for.
Poor fits include grade IV bone-on-bone arthritis, active infection, certain blood and cancer histories, and patients who want a single injection to replace rehab. We also postpone in specific situations, including PRP during pregnancy, where the right call is to wait rather than experiment.
The biggest mistake I see isn’t choosing PRP. It’s committing to a full series in a joint that’s too far gone, then arriving at the surgical conversation a year later than you should have.
The AAPM&R guidance statement names most of these directly, including dosing, technique, and image guidance. Patients almost never ask about them. Providers almost never volunteer them.
Now a correction, because I’ve been sloppy above. I keep writing that PRP “lasts” 6 to 12 months, like a filler dissolving on schedule. That framing isn’t quite right. The platelets are consumed within days. What you’re buying is a repair window, and the tissue that forms inside that window is what carries you for a year. The clock isn’t running down on a drug. It’s running on your joint, which keeps aging either way. That distinction changes what you should be doing in month two.
The appointment runs about an hour. The injection itself takes minutes.

Before anything gets prescribed here, we work through five questions. Is this person a fit. What’s the goal. What needs ruling out first. What gets monitored. When do we stop. It’s the same filter we apply to whether someone qualifies for any regenerative protocol.
Regenerative medicine got big on social media, and the DIY version showed up right behind it.
Wanting faster recovery, better body composition, and more comfortable years isn’t the problem. Those are reasonable goals. The problem is what happens when that interest meets an influencer funnel instead of a clinician.
You can’t order PRP online, since it’s your own blood. You absolutely can order peptides, and almost every one of those vials is labeled not for human use. They skip purity, potency, and sterility testing entirely, and the red flags are easy to spot once you know them. Administering or dispensing research-grade material for people is illegal. We work only with human-grade compounds from licensed pharmacies.
Remember that insulin is a peptide. Insulin isn’t a trend. It’s a powerful molecule the body makes that saves lives used correctly and causes real harm used wrong. None of these tools are good or bad on their own. What matters is whether they’re used on purpose, for the right person, inside a plan. A vial is not a strategy. Self-experimentation is not the same thing as clinical care.
PRP fine-tunes the repair you’re already driving. It doesn’t do the work. Your habits do.
Then the point most people miss. The body compensates through many pathways at once. Push hard on one and your physiology routes around it, so the result comes out weaker than promised. The injection, the food, the sleep, and the training all have to point the same direction. Some patients also layer in peptides that support repair, though that’s a separate clinical decision rather than an automatic add-on.
Reassess at three months, then plan the repeat before your pain fully returns rather than after.
Most of our joint patients do one series up front, usually two or three injections spaced four to six weeks apart, then a single maintenance injection somewhere between month nine and month 15. Waiting until you’re back to square one means restarting on a joint that’s had another year of wear. Tendon cases often don’t need a repeat at all.
Satisfaction data tracks that curve. One pilot series reported 90% of patients satisfied at six months and 75% still satisfied at twelve, which is roughly what a maintenance schedule is built to protect.
If the conversation moves past PRP toward cell-based options, the rules change. Those aren’t autologous, so sourcing and legal status matter far more, and state stem cell laws now govern what can be offered and what has to be disclosed. We use only labs certified under the applicable Florida statute, and safety sits ahead of speed on that list every time.
Stop asking how long a PRP joint injection lasts and start asking how much comfortable, active time it buys you per needle. Twelve months of a knee that lets you train, sleep, and walk your dog is a different outcome than twelve months of slightly less ache while you sit still. The first one requires you to show up. The second is what people get when they treat the injection as the entire plan.

Expect 4 to 8 weeks before joint pain shifts, and 6 to 12 weeks for tendon problems. PRP opens a repair window rather than suppressing pain directly, so nothing happens on day two. Two to four days of ache in the treated area right after the injection is normal.
Yes, and it's the most under-discussed variable in the field. The 2025 analysis in the American Journal of Sports Medicine found that high-platelet preparations held their functional gains at 12 months while low-platelet preparations did not. Ask any clinic what concentration they deliver before you book.
Generally no. The April 2026 AAPM&R guidance recommends PRP for Kellgren-Lawrence grades I through III and specifically not for grade IV. In advanced disease, relief tends to be measured in weeks rather than months, and the better conversation is usually a surgical one.
It depends on the timeframe. OARSI guidance supports corticosteroid for acute relief of 1 to 2 weeks and short-term relief of 4 to 6 weeks, while PRP typically holds 6 to 12 months. A 2024 meta-analysis on tennis elbow found steroid better under two months and PRP better at six months and beyond.
