PRP hair treatment is done in one office visit, and it comes down to four moves. A small blood draw, a spin in a centrifuge, preparation of the concentrate, then a series of shallow injections across the thinning parts of your scalp. Most visits run 45 to 60 minutes. No general anesthesia, no incision, and you drive yourself home. One thing this article won’t do is decide whether PRP fits your hair loss. That depends on your stage of thinning, your labs, and what else is driving the shed, and it belongs in a consultation. What it will do is walk through the mechanics without skipping the parts most clinics compress into a single line.
PRP hair treatment is a scalp injection procedure built from your own blood. Platelets carry growth factors that signal tissue repair. Spinning blood in a centrifuge separates and concentrates those platelets into platelet-rich plasma, which then gets injected into the thinning zones of your scalp to support the follicles still living there.
You’ll also hear about PRF, a slightly different spin that traps fibrin along with the platelets. The real-world gap between PRP versus PRF is narrower than the marketing makes it sound.
Platelet counts vary a lot between people. A normal range runs roughly 150,000 to 450,000 per microliter, so two patients on the identical protocol can walk out with concentrates of very different strength. That variation is one reason results swing so widely, and almost nobody brings it up during a consult.
No. The centrifuges and kits used to prepare PRP hold FDA clearance, but that clearance covers the equipment, not hair growth as an indication. Johns Hopkins Medicine notes the procedure is still considered investigational, and that FDA clearance means a physician may administer PRP when they judge it right for the patient, not that hair regrowth itself got approved.
Off-label isn’t a scandal. Physicians do it legally, every day. It does mean provider judgment is the safeguard, not a regulatory stamp, so who prepares and places your PRP matters more than it would in a procedure with a fixed, approved protocol.
Nobody has fully mapped the mechanism, and any clinic that tells you otherwise is selling. PRP carries at least six growth factors plus roughly thirty other bioactive compounds, so pinning down which one does what has been hard. The three most studied for hair are fibroblast growth factor, vascular endothelial growth factor, and epidermal growth factor. Together they seem to stretch out anagen, the active growing phase of the hair cycle, and improve blood supply around the follicle.
Good candidates have thinning hair with living follicles still under the skin. PRP wakes up follicles that have gone quiet. It can’t raise follicles that are already dead, which is why a bare, shiny crown is a transplant conversation, not an injection one. Whether PRP can regrow hair in bald areas is the single most common misread, and it costs people months of waiting on the wrong plan.
The patients who do best, in my experience, share a pattern. Loss that started recently, diffuse thinning instead of patches, a widening part or fading density at the crown, and no untreated thyroid, iron, or hormonal problem underneath. Scarring alopecia is a different disease, and PRP won’t touch it.

You’ll feel pressure and small stings. Most people call it annoying, not painful. No real downtime, though the scalp stays tender a day or two.
Plan on three to four sessions spaced four to six weeks apart, then maintenance every four to six months. One session does nothing you can measure. If a clinic sells you a single treatment and implies you’re finished, walk out.
Maintenance isn’t a revenue trick either. The gains fade once you stop PRP for hair, so treat this like blood pressure management, not a one-time haircut.
Leave your scalp alone on day one. No shampoo, no product, no scratching, no hat pressure. After about 24 hours you can shampoo gently and comb lightly.
Skip anti-inflammatory medication for a couple of weeks. Ibuprofen and aspirin blunt the exact inflammatory signal PRP relies on. Sauna, alcohol, and hard training all wait 48 hours too, and there’s more nuance to when you can exercise after PRP than most people expect, since spiking your heart rate too early works against the treatment.
The honest summary is moderate evidence for density, weaker evidence for thickness. A 2025 systematic review and meta-analysis in Dermatology and Therapy pooled 43 randomized controlled trials covering 1,877 participants. It found that activated PRP improved hair density and cut recurrence against placebo, and it reported no meaningful pooled benefit for hair thickness.
Read that twice, because it contradicts most marketing copy in this space, including copy I’ve seen from clinics I respect.
Individual trials look rosier. A 2025 Phase I trial in the Journal of Cosmetic Dermatology, average patient age 40.2 years, reported hair count up 62.4% and thickness up 58.6%, with 58% of patients rating their results excellent. Patient-reported data runs higher still. Roughly 93% of about 231 RealSelf reviewers call PRP for hair worth it, a figure carrying obvious selection bias, since people who quit early rarely write reviews.
Actually, calling this a contradiction isn’t quite right. Pooled meta-analyses average away protocol differences, and protocol is exactly what varies. Activated versus non-activated prep, single versus double spin, injection depth, concentration reached. The meta-analysis measures PRP as a category. A good clinic runs one specific version of it.
PRP is one of the lowest-risk injectables in aesthetics because the material comes from you. No foreign product, no allergy risk in the usual sense, no sourcing question. Johns Hopkins lists the common effects as soreness and bruising at the injection site, with bleeding, infection, and nerve irritation as uncommon possibilities.
The side effect that scares people most isn’t on that list. Some patients shed a little more for a few weeks after the first session. That’s usually follicles cycling, not damage, and the panic calls about it come in every month.
This is the part of the talk that separates a real clinic from a shop selling vials. PRP, peptides, exosomes, stem cells: none of them do the work. They fine-tune work that’s already happening and make it more efficient. Your lifestyle does the work.
So when we build a hair loss plan, the injections are one line item. The rest targets the same biology from other directions. Protein high enough to actually supply keratin. Ferritin and vitamin D checked and corrected, not guessed at. Sleep, because most follicle repair signaling runs overnight. Scalp inflammation handled. Nicotine gone, since it constricts the microvasculature PRP is trying to improve. Getting the fundamentals right is most of how you maintain hair after PRP, and biotin isn’t the fix unless you’re actually deficient.
The body compensates. Push hard on one pathway and it routes around you through another, so the result comes out worse than the effort deserved. That’s the single most useful thing I’ve learned in regenerative medicine, and it’s why we chase a goal through several pathways at once instead of betting everything on the syringe.
One more thing, because this field has an ugly underbelly. Formation uses human grade compounds only. Research grade molecules are a separate category, and it’s illegal to administer, prescribe, buy, or dispense them. We don’t carry them. Formation also holds SSRP certification from the Society for the Study of Regenerative Practices and ISSCA certification from the International Society for Stem Cell Application, both of which set standards for peptides, exosomes, and stem cells. The FDA has warned for years about unapproved regenerative medicine products, and those warnings exist because clinics earned them.
Reduced shedding usually shows up first, somewhere between weeks two and six. Visible density comes later, generally months four through six, once new fine hairs mature into terminal strands. Johns Hopkins Medicine puts the most noticeable point for scalp injections at around six months, and it notes the results aren’t permanent.
Your phone is the best tracking tool you own. Same lighting, same angle, same time of day, every four weeks. Memory is terrible at gradual change and photos aren’t, which is really the whole trick to knowing how long PRP takes to show anything on your scalp.
Understanding how PRP hair treatment is done matters less than understanding what it is. It’s a maintenance therapy for early-stage hair loss with living follicles, run as a series, supported by everything you do between sessions. Come in early, track it honestly, and treat the injections as one pathway among several. If you’re weighing where to get PRP, the clinic’s protocol and who holds the syringe should decide it, not the discount. Patients who show up expecting a rescue are the ones who leave let down.
