You’re awake during FaceTite, and you can hold a conversation the whole time. I numb the area with a local anesthetic first, then work through two or three access points smaller than a grain of rice. No breathing tube, no general anesthesia, no anesthesiologist watching a monitor. In the largest published review of the procedure, 240 of 247 cases were done exactly that way. Most of my Tampa patients spend the hour telling me about their kids.
Awake FaceTite is radiofrequency skin tightening performed under local anesthesia while the patient stays conscious. A thin probe passes just beneath the skin and delivers controlled heat. Most clinics sell the awake part as a benefit. I’d argue it’s closer to a byproduct, and knowing why changes how you should judge the whole procedure. The same logic applies to being awake during neck liposuction, which I perform the same way in the office.
The numbing solution shuts down sensation in the treated tissue before any energy gets delivered, so there’s nothing to sleep through. You can stay awake because the procedure is small, not because anyone is cutting corners on comfort.
I inject a dilute mix of lidocaine, epinephrine, and buffered saline into the layer directly under the skin. It does three jobs instead of one. The lidocaine blocks pain signals. The epinephrine tightens blood vessels, which cuts bleeding and bruising. The added fluid builds a physical cushion between the heated probe and the nerves I want nowhere near it.
That third job matters more than patients realize. The marginal mandibular nerve runs along the jaw and controls part of your lower lip. Tumescent fluid lifts it clear of the heat, which is the same principle behind awake body contouring.
Pressure, warmth, and a tugging feeling along the jaw, like someone pulling gently at the skin from underneath. No sharp pain once the area is numb. Some people hear a faint clicking from the handpiece.
I give most patients an oral sedative about thirty minutes beforehand. Not because FaceTite hurts. Because lying still while somebody works on your face is easier when you’re relaxed. In a review of 247 patients, laxity scores improved by 1.4 points on a standard grading scale and 93 percent said they would do it again.
Plan on two hours door to door. Energy delivery averaged 58 minutes in the published series. Numbing adds another 20 to 30 minutes before that, plus time for marking, photos, and letting the sedative settle in.
A few practical things nobody asks until the day before. Arrange a driver, because the sedative rules out driving yourself. Eat something light rather than fasting, since you aren’t going under. Stop aspirin, fish oil, and anything else that thins your blood a week ahead. How you prepare for FaceTite affects bruising more than most patients expect, so show up with a clean face and wear a shirt that buttons.
Staying awake removes a risk category. It does not improve your result.
General anesthesia carries small but real risks, and for a procedure this limited, those risks would be the biggest hazard in the room. Skipping them is a genuine win. That’s the honest case for awake FaceTite, and it holds up.
But being awake tells you nothing about whether the tightening will work on your face. Your skin decides that, along with your fat distribution and whoever is holding the probe. The marketing pages also leave out one detail. A systematic review of subsurface radiofrequency found no prospective randomized studies with a control group, and the median study size was 25 patients.
Actually, let me sharpen that. The evidence isn’t weak. It’s uncontrolled. Patients in these series did improve. Nobody has run the trial where half the group gets nothing and we compare.
The right candidate has loose skin that still snaps back plus a modest fat layer under the chin. That window is narrower than the marketing suggests. Too little fat and there’s nothing to contour.
Pinch the skin under your chin, pull it down half an inch, and let go. If it springs back fast, you have elasticity to work with. If it drifts back slowly or stays where you left it, you’re looking at excision rather than energy, and FaceTite will disappoint you.
I run this test in every consultation and it overrides everything else on the chart. Age matters less than people assume. I’ve treated 62-year-olds with better recoil than 45-year-olds who spent two decades in the Florida sun.
Here I disagree with how the industry sells this. Almost every clinic frames FaceTite as a facelift alternative. It isn’t one. Facelifts move tissue. Radiofrequency shrinks it. Those are different mechanical problems, and the reported volumes climbed for both surgical options in 2024, with neck lifts up 2 percent to 22,445 procedures and facelifts up 1 percent to 79,058. Two rising categories usually means two different groups of patients.
Competitor pages claim only a board-certified plastic surgeon can operate this device. That’s marketing, not regulation. Device use follows state scope-of-practice law and manufacturer training, and how FDA clearance works for these platforms surprises most patients. What predicts your result is how many of these the person has performed and whether they can manage a complication when one appears.
I’m a board-certified general surgeon with fellowship training in burn surgery, critical care, and minimally invasive procedures. Controlled thermal injury under a jawline sits squarely in that wheelhouse. Formation also has a plastic surgeon on staff, Dr. Ahmad Saad, for cases that call for a different skill set. Give it three to five days before you look presentable and one to two weeks before most swelling clears. Pages promising you’ll return to normal activity immediately are wrong, and that gap is exactly where disappointment comes from.
You’ll wear a chin compression garment 12 to 24 hours a day through the first week, and the aftercare instructions we send home cover when to start massage and how hard to press. Swelling peaks around day two or three. Bruising along the jaw is common and usually fades inside ten days.
The timeline that matters runs longer than the one people ask about. Skin retraction keeps improving for months as new collagen lays down. I tell patients to expect visible jawline change around three months and the final shape somewhere between six and nine months. Anyone promising a finished neck in two weeks is selling you something.
Firm lumps are the complaint I hear most. In that 247-patient series, 4.8 percent had swelling past six weeks and 3.2 percent had hardened areas past twelve weeks. Every case resolved on its own.
Temporary numbness along the jaw is normal and expected after FaceTite. Nerve irritation shows up in roughly 1 percent of cases, and in the published data every episode resolved without treatment. Full sensation usually returns over weeks to a few months.
Loose skin isn’t one problem. Several aging processes land in the same place, which is why one lever rarely fixes it.
The Hallmarks of Aging framework names the culprits. Collagen and elastin degrade, which stiffens the matrix that used to hold everything taut. Senescent cells build up in the dermis and stop pulling their weight. Stem cell reserves thin out, so repair slows down. Controlled heat reverses none of that. What it does is trigger a wound-healing response that recruits whatever repair capacity you still have, which explains why the same treatment gives two people the same age different results.
Rapid weight loss shifts the equation hard. GLP-1 medications strip fat faster than the matrix can adapt, and patients arrive with skin their frame no longer fills.
This is the Rhythm of Longevity in practice, the seesaw between building and renewal. Sleep, protein intake, sun protection, and stable weight do the foundational work of keeping that matrix intact. FaceTite refines what your habits already preserved. It can’t stand in for them. Patients who tighten a neck and keep smoking will be back in my office.
Heavy skin excess, muscle banding, and a small chin. Energy does nothing for any of the three.
Loose vertical cords down the front of your neck are platysma muscle, and heat won’t tighten muscle. A soft jawline caused by a recessed chin needs projection, not shrinkage. And if you can gather two inches of skin under your chin, that skin needs removing.
When I turn someone down for FaceTite, I tell them what would work instead. Muscle bands need suturing. A weak chin needs projection built in. Heavy skin needs excision, and even the easiest neck lift surgery is still surgery with a real recovery attached. None of those are consolation prizes. They’re the correct answer for that anatomy, and they cost less than paying for the wrong procedure first.
I say no more often than I say yes. Not false modesty. A treatment that can’t reach someone’s problem means they pay twice, once for the failure and again for the fix.
Everyone asks whether you’re awake during FaceTite. The better question is whether your skin will respond at all. Do the pinch test before your consultation, bring the answer with you, and ask the person holding the probe how many jawlines they’ve treated. If those answers hold up, being awake will be the least interesting part of your morning.
